Carpal tunnel release using the MANOS CTR system: preliminary results in 52 patients.

PURPOSE: To describe a carpal tunnel release technique using the MANOS Carpal Tunnel Release device, with preliminary results in 52 patients. METHODS: The MANOS Carpal Tunnel Release device is a blade that divides the transverse carpal ligament using wrist and palm skin punctures. The awake patient provides feedback as the surgeon navigates a 2.1-mm-diameter blunt probe across the undersurface of the ligament from a wrist incision with standard disposable nerve stimulator monitoring. The leading tip of the blunt probe is uninsulated and conducts 2 mA. The surgeon converts the blunt insulated

Intrathoracic dislocation of the inferior pole of the scapula following thoracotomy.

Intrathoracic dislocation of the scapula is extremely rare. We present the case of a 64 year old man who underwent a lung transplant for emphysema via a standard posterior thoracotomy approach. Four weeks later, following a bronchoscopy, he experienced severe pain and restriction of movement in his shoulder. CT scans revealed intrathoracic dislocation of the inferior angle of the scapula. Two manipulations under anaesthesia were unsuccessful and formal exploration required with closure of the intercostal defect. We describe our surgical technique.

High prevalence of heterotopic ossification after cervical disc arthroplasty: outcome and intraoperative findings following explantation of 22 cervical disc prostheses.

OBJECT: Cervical disc arthroplasty (CDA) has been increasingly used for the treatment of cervical disc herniations. However, the impact of CDA on adjacent-segment degeneration and the degree of heterotopic ossification (HO) of the treated segment remain a subject of controversy. Due to a product failure of the Galileo-type disc prosthesis, 22 of these devices were explanted. The radiological and clinical course in each case was investigated in detail with an emphasis on the incidence of HO and facet joint degeneration 18 months following the operation. Intraoperative findings regarding ossific

Improved ankle push-off power following cheilectomy for hallux rigidus: a prospective gait analysis study.

BACKGROUND: There is limited objective scientific information on the functional effects of cheilectomy. The purpose of this study was to test the hypothesis that cheilectomy for hallux rigidus improves gait by increasing ankle push-off power. METHODS: Seventeen patients with symptomatic Stage 1 or Stage 2 hallux rigidus were studied. Pre- and postoperative first metatarsophalangeal (MTP) range of motion and AOFAS hallux scores were recorded. A gait analysis was performed within 4 weeks prior to surgery and repeated at a minimum of 1 year after surgery. Gait analysis was done using a three-dim

A lethal case of meningitis due to Lactobacillus rhamnosus as a late complication of anterior cervical spine surgery.

We reported a lethal case of meningitis due to Lactobacillus rhamnosus as a late complication of anterior cervical spine surgery in an 80 years old lady. The anterior plate graft was responsible for a fistula between the oesophagus and meningeal space. This case illustrated the difficulty of early diagnosis of such infrequent complication and suggested that dysphagia following anterior cervical spine surgery should prompt cervical imaging.

Early treatment of scoliosis with growing rods in children with severe spinal muscular atrophy: a preliminary report.

BACKGROUND: Spinal muscle atrophy (SMA) is a progressive neuromuscular disease predominantly presenting in infancy and early childhood. Scoliosis is the most common spinal deformity in these patients and treatment in SMA patients is controversial. Treatment is usually definitive fusion. The purpose of this study is to evaluate a novel growing rod technique used to treat more involved children with SMA types I and II with scoliosis at an earlier age. METHODS: An Institutional Review Board approved retrospective medical record review was performed of children with SMA who were treated for scoli

Surgical management of U-shaped sacral fractures: a systematic review of current treatment strategies.

PURPOSE: U-shaped sacral fractures usually result from axial loading of the spine with simultaneous sacral pivoting due to a horizontal fracture which leads to a highly unstable spino-pelvic dissociation. Due to the rarity of these fractures, there is lack of an agreed treatment strategy. METHODS: A thorough literature search was carried out to identify current treatment concepts. The studies were analysed for mechanism of injury, diagnostic imaging, associated injuries, type of surgery, follow-up times, complications, neurological, clinical and radiological outcome. RESULTS: Sixty-three cas

Noninfectious wound complications in clean surgery: epidemiology, risk factors, and association with antibiotic use.

BACKGROUND: Noninfectious wound complications are frequent and often are confused with and treated as infection. METHODS: We assessed the epidemiology, impact, risk factors, and associations with antibiotic use of noninfectious wound complications in clean orthopedic and trauma surgery. We report a single-center, prospective, observational study in an orthopedic department. RESULTS: Among 1,073 adult patients, 630 (59%) revealed clinically relevant postoperative noninfectious wound complications, leading to a significant prolongation of hospital stay (14 vs. 12 days; Wilcoxon rank-sum test;

Novel anticoagulants: new evidence for emerging drugs and their potential application in major lower limb surgery.

For decades, parenteral drugs, such as the low molecular weight heparins and unfractionated heparins or vitamin K antagonists, have been used as anticoagulants for prevention of venous thromboembolism following major lower limb surgery. However, these regiments have limitations that rendered the quest for new anticoagulants mandatory. Recently, research has been focused on the development of orally active small molecules that directly target thrombin or activated factor X (FXa). These regiments exhibit a number of characteristics that an "ideal" anticoagulant should possess. Currently, two age

Comparison of a novel bone-tendon allograft with a human dermis-derived patch for repair of chronic large rotator cuff tears using a canine model.

PURPOSE: This study tested a bone-tendon allograft versus human dermis patch for reconstructing chronic rotator cuff repair by use of a canine model. METHODS: Mature research dogs (N = 15) were used. Radiopaque wire was placed in the infraspinatus tendon (IST) before its transection. Three weeks later, radiographs showed IST retraction. Each dog then underwent 1 IST treatment: debridement (D), direct repair of IST to bone with a suture bridge and human dermis patch augmentation (GJ), or bone-tendon allograft (BT) reconstruction. Outcome measures included lameness grading, radiographs, and ult

Tendoscopic treatment of intrasheath subluxation of the peroneal tendons.

BACKGROUND: Intrasheath subluxation of the peroneal tendons does not have an injury of the proximal retinaculum. The aim of this study was to describe the tendoscopic technique and preliminary results. METHODS: Six patients with intrasheath subluxation of the peroneal tendons were treated tendoscopically. All patients had preoperative pain and a clicking sensation at the lateral retromalleolar area. Mean followup was 18.3 (range, 14 to 24) months. These six patients included three males and three females, with a mean age of 23.5 (range, 18 to 33) years. The AOFAS and Visual Analog Score for p

Popliteal angle as an indicator for successful closed reduction of developmental dysplasia of the hip.

PURPOSE: To evaluate the use of the popliteal angle as an indicator for successful closed reduction of developmental dysplasia of the hip (DDH) in children with delayed presentation. METHODS: 29 patients aged 6 to 18 months underwent successful closed reduction for unilateral DDH under general anaesthesia. Using a graduated goniometer, the popliteal angle was measured with the hip abducted within the safe zone (to avoid redislocation and injury to the femoral capital epiphysis) before and after reduction and after 6 weeks of spica casting. RESULTS: The mean popliteal angles before and after

Repair of complex and avascular meniscal tears and meniscal transplantation.

A functional meniscus is critical to the long-term health of the knee joint. The repair of meniscal tears that extend into the central avascular region requires understanding the appropriate indications, contraindications, surgical techniques, and postoperative rehabilitation protocols. An inside-out repair technique using multiple vertical divergent sutures with an accessory posteromedial or posterolateral incision is recommended for optimal stability. In young, active patients, the risk of repair failure and the need for revision are outweighed by the benefit of meniscal preservation. Althou

Meniscal root suturing techniques: implications for root fixation.

BACKGROUND: Meniscal root tears have attracted increasing interest in recent years. Fixation is an important factor for rehabilitation and avoidance of early failure. Suture fixations have been the most commonly used techniques. The current study aimed to evaluate the maximum failure load of the native meniscal roots (anteromedial, posteromedial, anterolateral, and posterolateral) and of 3 commonly used meniscal root fixation techniques (2 simple stitches, modified Kessler stitch, and loop stitch). HYPOTHESES: (1) There will be no difference in maximum failure load between the native meniscal

A new three-stage spinal shortening procedure for reduction of severe adolescent isthmic spondylolisthesis: a case series with medium- to long-term follow-up.

STUDY DESIGN: A case series of eight consecutive patients with severe (Meyerding Grade ≥ 3) adolescent isthmic spondylolisthesis (SAIS) who underwent reduction and stabilization by using a new surgical technique. OBJECTIVE: To report the results of a safe three-stage spinal shortening procedure in a single operative session in eight patients with SAIS. SUMMARY OF BACKGROUND DATA: The treatment of SAIS is controversial and the opinion continues to remain divided between in situ fusion and reduction followed by stabilization. We reported a new surgical technique to facilitate safe reduction an

Management of traumatic anterior shoulder dislocation in the 17- to 25-year age group: a dramatic evolution of practice.

BACKGROUND: Anterior shoulder dislocation in the young patient is a common problem, yet there is no universal agreement on its management. METHODS: In this study, we comprehensively surveyed all members of the British Elbow and Shoulder Society (BESS) to ascertain their preferred methods of treating young patients with traumatic, anterior shoulder dislocation. We then repeated exactly the same survey 7 years later to find out whether practices had changed and if any consensus of opinion had been reached. RESULTS: The number of surgeons indicating their preferred stabilization procedure was a
