[Nasal positive pressure ventilation (nCPAP) in therapy of obstructive sleep apnea: acceptance by 50 patients].

In a prospective study between 1989 and 1993, 50 patients (45 males, 5 females, mean age: 58.4 years; mean AHI = 37.8/h) suffering from obstructive sleep apnoea were examined. They underwent nocturnal polysomnography and were provided nightly treatment with nasal CPAP (Respironics Sleep-Easy III und REMSTAR, Respironics Inc, Monroeville, PA). 41 patients were given primary treatment with nCPAP. Nine patients were secondarily treated with a nCPAP mask after other modalities of therapy (weight reduction, mandibular positioning appliance, nasal surgery, uvulopalatopharyngoplasty) had been applied…

Clinical diagnoses in 216 insomnia patients using the International Classification of Sleep Disorders (ICSD), DSM-IV and ICD-10 categories: a report from the APA/NIMH DSM-IV Field Trial.

Three diagnostic classifications for sleep disorders have been developed recently: the International Classification of Sleep Disorders (ICSD), the Diagnostic and Statistical Manual, 4th edition (DSM-IV), and the International Classification of Diseases, 10th edition (ICD-10). No data have yet been published regarding the frequency of specific diagnoses within these systems or how the diagnostic systems relate to each other. To address these issues, we examined clinical sleep disorder diagnoses (without polysomnography) in 257 patients (216 insomnia patients and 41 medical/psychiatric patients)…

Postoperative risk following uvulopalatopharyngoplasty for obstructive sleep apnea.

A study was conducted to assess oxygenation and respiratory changes on the first and second postoperative nights after uvulopalatopharyngoplasty (UPPP) for obstructive sleep apnea (OSA). Twelve patients were postoperatively evaluated with 8-hour nocturnal polysomnography on four occasions: (1) PREOP--night before UPPP, (2) POPN1--first postoperative night, (3) POPN2--second postoperative night, and (4) 3MOS--3-month follow-up study. Results demonstrate that apnea index (AI) and respiratory disturbance index (RDI) were significantly improved at 3MOS from PREOP levels: AI (p less than 0.01) and …

Cassette EEG sleep recordings in Gilles de la Tourette syndrome.

Tourette syndrome (TS) patients often complain of sleep problems, and questionnaire studies indicate that sleep disturbance is frequent. Decreased slow wave sleep and increased awakenings have been reported in laboratory polysomnography in TS patients, and a serotoninergic disorder of arousal has been postulated. We recorded outpatient sleep in 20 patients newly diagnosed with TS utilizing a 4-channel cassette EEG system. The newly-diagnosed patients were predominantly male, and ranged in age from 10 to 36 years. Some had taken psychotropic medications in the past, but none had been treated sy…

[Neurophysiological studies on group A xeroderma pigmentosum in early childhood].

Neurophysiological studies were performed on 8 patients with group A xeroderma pigmentosum during early childhood. EEG, ABR and NCV were normal during this period. In contrast, various sleep parameters detected by polysomnography showed abnormal findings even in the neurologically normal patient. Decreased % sleep REM was seen in a case, and decreased frequency of REMs were seen in another. Body movements were extremely high or low in frequency in 3 cases in whole night sleep. The distribution of body movements were abnormal; in control subjects, the frequency was higher in SREM and stage 1 th…

Physiological and Sleep Profiles in Insomnia, OSA and COMISA: A Comparative Study in Portugal and Spain.

Insomnia and obstructive sleep apnea (OSA) are common sleep disorders that often co-occur as comorbid insomnia and sleep apnea (COMISA), worsening health outcomes. This study characterized and compared the physiological and sleep profiles of insomnia, OSA and COMISA in population-based datasets from Portugal (n = 147) and Spain (n = 264), excluding patients with cardiovascular disease. Classification followed clinical guidelines, and analyses included polysomnography and heart rate variability (HRV) data. Conventional HRV metrics in the time and frequency domains, along with condition-specific…

Pediatric obstructive sleep apnea hypopnea diagnosis using a video recording score: A pilot study.

OBJECTIVES: Because access to sleep recordings is limited, there is a need for new reliable tools for pediatric obstructive sleep apnea-hypopnea syndrome (OSAHS) diagnosis. A score calculated from a 30 minutes-home sleep video recording (VR) has already been proposed in 1996 with interesting results. The main objective of this pilot study was to assess the reliability of a similar score applied to reference PSG VR and calculated on two different time windows (30 and 10minutes). METHODS: A retrospective monocenter study was made on 16 children suspected of OSAHS, that underwent VR during overn

Nocturnal Sleep Breathing Patterns in Healthy Adolescents Residing at Very High Altitudes in Bolivia.

Data on sleep and respiratory patterns among adolescents residing at very high altitude (> 3500 m) remain scarce, and altitude-related physiological differences may influence these parameters. Studying adolescents at different very high altitudes is crucial, as subtle environmental variations could affect sleep-related oxygenation and respiratory function. This study aimed to characterise sleep-related oxygenation and respiratory parameters in healthy adolescents native to two distinct very high-altitude environments. Overnight sleep polygraphy was performed in 163 healthy adolescents aged 13.

Evaluating the role of inpatient sleep studies in hospitalized adults in relation to 1-year mortality and healthcare utilization: a population-based cohort study.

RATIONALE/RESEARCH OBJECTIVE: Sleep-disordered breathing (SDB) is common among hospitalized individuals and may contribute to increased health utilization and mortality after discharge, yet the optimal timing and setting for sleep testing remain unclear. We compared 1-year healthcare utilization and mortality among hospitalized individuals undergoing (1) inpatient polysomnography (PSG), (2) delayed post-discharge PSG within 1 month of discharge (primary analysis), or (3) no PSG during hospitalization or within one year after discharge (matched non-PSG controls); secondary analyses examined del…

First night positive airway pressure therapy does not impair sleep quality in obstructive sleep apnea.

BACKGROUND: Positive airway pressure (PAP) therapy is the gold standard treatment for obstructive sleep apnea (OSA), yet patients often express concern about its impact on sleep quality during the initial phase of treatment. This study investigated whether the first night of PAP therapy adversely affects objective sleep architecture. METHODS: In this retrospective study, polysomnographic data from N = 176 patients with OSA undergoing first-night PAP therapy (continuous, auto titrated, or bilevel) were compared with a healthy sleep control group (N = 73). Sleep efficiency (SE), proportions of
