Congenital hernias in the lumbar region are very uncommon. Approximately 10 per cent of all lumbar hernias are congenital and the vast majority are unilateral. We report our experience with seven congenital lumbar hernias in six infants treated during a five year period. All patients presented in the first year of life with abnormal protrusions in the lumbar region. Biological prostheses of human umbilical vein and synthetic prostheses of ePTFE were comparatively studied in the microsurgical anastomosis of the uterine horns in twenty rats. Patency rate after 30 days was 60% for umbilical vein and 75% for the ePTFE prostheses. Gross and histological examination of umbilical vein graft specimens showed minimal alteration with a smooth intimal surface. On the contrary tissue covering the inner surface from the adjacent ends of the uterine horns was noted in ePTFE prostheses.

We have to date installed artificial spermatoceles made of silicone in a total of 33 patients. Artificial insemination of husband (AIH) was performed in 1-9 times using semen collected from 11 of the 33 patients in whom spermatocele was installed, and pregnancy was achieved in 2 cases. In 24 cases scutiform polytetrafluoroethylene patches were implanted to correct saddle malformation and irregularities of the nasal dorsum. For smaller tissue defects of the nasal bone and cartilage, the patches are very suitable in rhinosurgery. One of the advantages of the implant is that it makes the reconstructed area appear as natural as possible.

Many different materials have been used to provide augmentation of the nasal dorsum. This includes both autogenous and homogenous materials, as well as allografts. Many times autogenous material is not readily available and homogenous materials have recently demonstrated significant absorption. Artificial urinary sphincters (AUS) were implanted around intestinal segments to achieve urinary continence in 8 patients and faecal continence in 1. The long-term results achieved in 31 joints by means of discoplasty or (temporary) implantation of silastic for treatment of internal derangements are presented. These data support the philosophy of preserving the disc whenever possible. If, however, discectomy is inevitable this fibrous plate should be replaced. A technique for controlling intraocular pressure in the immediate postoperative period with Molteno implants is described. Filtration tubes were closed with polypropylene sutures in 13 patients. Filtration tubes closed with 7-0 sutures provided the most satisfactory partial to complete lumen closure. The ligatures in six patients later were opened by argon laser.

A modified Stamey technique was applied to nineteen women to control their stress incontinence. Needle puncture was reduced to one time for each side of the urethra by placing the Dacron sleeve parallel to the urethra. Postoperatively, the urethrovesical angle was reduced to be within normal limit, and functional urethral length and maximum urethral closure pressure increased significantly. All patients receiving the modified operation restored urinary continence without any significant complications.

In 14 out of 94 patients with erectile dysfunction a venous insufficiency was suspected. Localization and securing of the venous leak were done by dynamic cavernosography. We report a new technique for the management of the complications of vesicorectal fistulas. The patient we present had a fistula and severe skin excoriation. The fistula was caused by carcinoma of the prostate that had been treated by radiation therapy. The fistula was patched with a rectal prosthesis similar to that used to patch esophageal-tracheal and esophageal-bronchial fistulas.

The tissue response of two low-gold alloys (Midas and Rajah), two silver-palladium alloys (Albacast and Alba V), and one type III gold alloy (JSC) was studied after subcutaneous implantation of cylindrical implants in 24 guinea pigs. The purpose of this study was to review the long-term results in anophthalmic patients treated for enophthalmos. The patients in our series had augmentation of the volume of their orbital contents by subperiosteal placement of room-temperature vulcanized silicone.
