Acute ischaemic stroke in pregnancy.

Stroke is a neurological emergency that can be either ischaemic or haemorrhagic. Hypertensive disorders of pregnancy are an important risk factor for haemorrhagic stroke, while thrombophilia (acquired or congenital) could increase the risk of ischaemic stroke. Pregnancy itself is a thrombogenic condition, and pregnancy-associated conditions like hyperemesis, pre-eclampsia and ovarian hyperstimulation in assisted conception may further increase this risk. Maternal stroke during pregnancy is an infrequent but debilitating condition that can lead to long-term disability and death. Early diagnosis

Management of osteoporosis in chronic kidney disease.

Patients with chronic kidney disease (CKD) face an excessive burden of bone disease and fracture risk compared with the general population due to metabolic derangements related to renal disease as well as traditional fracture risk factors, osteoporosis and advancing age. Fragility fracture incidence increases with progressive renal impairment and age, and fractures in this group are associated with exaggerated morbidity and mortality. Despite this, the diagnosis and management of osteoporosis in CKD is not well understood and patients are frequently undertreated. This review summarises the cur

Risks and safety of biologics: A practical guide for allergists.

Biologic agents are a rapidly expanding class of medications, and several options are now available for the management of allergic and immunologic disorders. The risks of biologic therapy need to be understood in order to adequately counsel patients and appropriately monitor for potential adverse events. We sought to provide a comprehensive review of the risks and adverse effects reported for the current FDA-approved biologics used in management of allergic and immunologic disorders, including omalizumab, benralizumab, dupilumab, mepolizumab, reslizumab, tezepelumab and tralokinumab. Our revie

Diagnosis and management of bone fragility in diabetes: an emerging challenge.

Fragility fractures are increasingly recognized as a complication of both type 1 and type 2 diabetes, with fracture risk that increases with disease duration and poor glycemic control. Yet the identification and management of fracture risk in these patients remains challenging. This review explores the clinical characteristics of bone fragility in adults with diabetes and highlights recent studies that have evaluated bone mineral density (BMD), bone microstructure and material properties, biochemical markers, and fracture prediction algorithms (i.e., FRAX) in these patients. It further reviews

[Cardiovascular risk of smoking by gender].

In France, the increase in the prevalence of women's smoking has been major over the past 30 years. The consequences are a significant increase and an unusual precocity of all the women's cardiovascular (CV) pathologies (myocardial infarction, stroke and aortic pathologies). Associated factors (combined oral contraception, unequal distribution of other CV risk factors by sex) increase the CV impact of women's smoking. For the same tobacco use, women have a 25% higher risk of coronary heart disease than men. The risk awareness is lower in women than in men and medical management insufficient. F

Valvular heart disease in pregnancy.

In women with valvular heart disease, pregnancy-associated cardiovascular changes can contribute to maternal, foetal and neonatal complications. Ideally, a woman with valvular heart disease should receive preconception assessment and counselling from a cardiologist with expertise in pregnancy. For women with moderate- and high-risk valve lesions, appropriate risk stratification and management during pregnancy will optimise outcomes. Pregnancy in women with high-risk lesions, such as severe aortic stenosis, severe mitral stenosis and those with mechanical valves, requires careful planning and c

Between a Rock and a Hard Place: Hip Fracture Repair in an Elderly Patient with Severe Cardiac Comorbidities and Limited Options: A Problem-Based Learning Discussion.

Perioperative right ventricular dysfunction can be life-threatening. Anesthetic management of elderly patients with hip fractures and advanced cardiac disease poses considerable perioperative risks. The problem-based learning discussion examines the management of an 87-year-old female with severe pulmonary hypertension, right ventricular dysfunction, and recent anticoagulation scheduled for urgent hip fracture repair. A tailored management approach included peripheral nerve blocks, intraoperative transesophageal echocardiographic monitoring, goal-directed hemodynamic support, and management of

Two different management modalities in a two sibling case report of Adams Oliver syndrome.

The authors report two-sibling case of Adams Oliver syndrome. One was managed with early surgical intervention after a period of failed conservative management and the other conservatively managed until reconstruction and surgical correction of alopecia. The goal of treatment is to achieve a complete closure of the defect without encountering any fatal complications such as haemorrhage and infection or causing any long-term neurological morbidity. Various papers have been published related to ACC, yet there is no consensus on the therapeutic approach. The management decision hinges upon balanc

Knowledge Graphs for Cervical Cancer Across the Care Continuum: From Vaccination and Screening to Personalized Therapy.

Cervical cancer remains one of the leading causes of cancer-related death among women worldwide, with a disproportionately high burden in low- and middle-income countries. Infection with human papillomavirus (HPV) is the central etiological factor. Although HPV vaccination and standardized screening can substantially reduce incidence and mortality, major bottlenecks persist in real-world settings, including inadequate coverage, loss to follow-up, data silos, and limited interpretability of algorithms. Knowledge graphs (KGs), characterized by their core "entity-relation" structure, can integrat

Metabolic, Atherogenic, and Anthropometric Markers for Discrimination of Premature Coronary Artery Disease in a Predominantly Statin-Treated Cohort.

PURPOSE: Premature coronary artery disease (PCAD) is increasingly recognized in younger adults and requires improved risk stratification. Conventional lipid markers may have limited discriminative value, particularly when lipid levels reflect ongoing statin therapy. This study evaluated the associations and discriminatory performance of metabolic, atherogenic, and anthropometric indices for PCAD and developed internally validated dynamic nomograms for PCAD case discrimination. PATIENT AND METHODS: This multicenter retrospective case-control study used data from the Iran Premature Coronary Art

From Legal Indeterminacy to Risk-Governed Continuity: Operationalizing Risk Governance for China's Family Doctor Contract Services.

China's family doctor contract services (FDCS) are an important instrument for strengthening Primary Health Care, chronic disease management and continuity of care, but their legal and institutional foundations remain uneven. This Perspective treats legal indeterminacy not simply as a drafting defect but as a governable source of risk. It uses a structured narrative and legal-policy synthesis of Chinese FDCS policy and implementation materials and peer-reviewed studies principally from 2016-2025, relevant current Chinese laws and administrative rules, and limited functional reference points fr

Clinical Outcomes and Treatment-Related Complications in Triple-Negative Breast Cancer: A Review of 24 Case Reports from 2020 to 2025.

Triple-negative breast cancer (TNBC) is clinically heterogeneous, and selected patients experience rare complications, unusual responses, or biomarker-discordant treatment trajectories that are not fully represented in aggregate trial evidence. This review aims to synthesize published case-report evidence on clinical outcomes and treatment-related complications in TNBC, with an emphasis on host vulnerability, tumor biology, biomarker-guided treatment, and therapy-associated toxicity. A descriptive review of PubMed-indexed case reports was performed for records published from January 1, 2020, t

Immune Checkpoint Inhibitors in Breast Cancer: Mechanisms, Biomarkers, and Future Therapeutic Strategies.

The landscape of breast cancer treatment has been fundamentally transformed by the emergence of immune checkpoint inhibitors (ICIs), representing a paradigm shift from traditional cytotoxic approaches to precision immunotherapy. This comprehensive review examines the current state and future directions of ICIs in breast cancer, with particular emphasis on triple-negative breast cancer (TNBC) where these therapies have shown the most promise. We explore the mechanistic foundations of checkpoint inhibition targeting CTLA-4, PD-1, and PD-L1 pathways, examine resistance mechanisms and predictive b
