Predictors of incomplete adherence, virologic failure, and antiviral drug resistance among HIV-infected adults receiving antiretroviral therapy in Tanzania.

BACKGROUND: Access to antiretroviral therapy is rapidly expanding in sub-Saharan Africa. Identifying the predictors of incomplete adherence, virologic failure, and antiviral drug resistance is essential to achieving long-term success. METHODS: A total of 150 subjects who had received antiretroviral therapy for at least 6 months completed a structured questionnaire and adherence assessment, and plasma human immunodeficiency virus (HIV) RNA levels were measured. Virologic failure was defined as an HIV RNA level >400 copies/mL; for patients with an HIV RNA level >1000 copies/mL, genotypic antivi

Health status ten years after pediatric liver transplantation--looking beyond the graft.

BACKGROUND: Little is known about long-term health after pediatric orthotopic liver transplantation (OLT). This study aimed to characterize the health status of recipients 10 years after OLT, with an emphasis on transplant-related morbidity and quality of life. METHODS: We performed a retrospective database review of 32 children who underwent OLT before October 1992 at one center and were alive after 10 years. Outcome measures were assessed 10 years after OLT. Cantril's self-anchoring scale was used for global quality of life assessment. RESULTS: Synthetic liver function at 10 years was pres

Treatment of dental anxiety disorders. Outcome related to DSM-IV diagnoses.

In this study we evaluated treatment effects in 70 consecutively admitted patients in a specialized university clinic for treatment of dental fear. Thirty-three (47%) of the patients fulfilled the diagnostic criteria for Specific Phobia alone (Dental Phobia, DP), 24 (33%) had severe dental anxiety without fulfilling the criteria for phobia (ND), and 13 (19%) fulfilled the criteria for multiple DSM-IV diagnoses (MD). Dental anxiety was measured prior to, immediately after and at follow-up (mean = 19 months) using Corah's Dental Anxiety Scale and Dental Fear Survey. Dental attitudes were measure

Scales, diagnoses, and child psychopathology: I. CBCL and DISC relationships.

OBJECTIVE: To clarify the relationship between scales and structured diagnostic interview diagnoses, the authors used a two-stage screening method to study 201 military families with one or more children ages 5 to 17. METHOD: Parents and children were interviewed with the Diagnostic Interview Schedule for Children (DISC 2.1); parents also completed the Child Behavior Checklist (CBCL) while the children completed other self-report symptom scales. RESULTS: Results indicate only a modest ability of scales to discriminate among discrete DISC-derived DSM-III-R diagnoses. Inclusion of diagnostic i

Comparison of simulation training on self and using a doll for teaching generalized menstrual care to women with severe mental retardation.

This study examined the effects of two simulation methods of general case instruction in teaching generalized menstrual care; long-term maintenance was assessed up to 18 months. Within a split multiple-baseline design across subjects, three middle-school students and one adult woman with severe or profound mental retardation initially were assigned either to on-self instruction or to instruction using a doll. The two learners receiving doll training also participated in instruction on self. A modified forward chaining procedure with identical task analysis steps was used with each method. The

Subjective cognitive decline predicts longitudinal neuropsychological test performance in an unsupervised online setting in the Brain Health Registry.

BACKGROUNDS: Digital, online assessments are efficient means to detect early cognitive decline, but few studies have investigated the relationship between remotely collected subjective cognitive change and cognitive decline. We hypothesized that the Everyday Cognition Scale (ECog), a subjective change measure, predicts longitudinal change in cognition in the Brain Health Registry (BHR), an online registry for neuroscience research. METHODS: This study included BHR participants aged 55 + who completed both the baseline ECog and repeated administrations of the CANTAB® Paired Associates Learning

E-Cigarette Dependence in Youth.

INTRODUCTION: The majority of e-cigarette vaping youth use nicotine when vaping. Some then become dependent on the nicotine, which can result in subsequent health effects. There has been limited evaluation of convergent validity of e-cigarette dependence measures for use specifically in youth. The aim of this study was to investigate and validate various e-cigarette dependence measures for use in youth populations. AIMS AND METHODS: One thousand two hundred and five Canadian youth aged 16-24 who completed a cross-sectional online survey reported vaping at least monthly and were thus included

The Association Between Self-Care and Health Literacy in Patients With Chronic Diseases: A Systematic Review and Meta-Analysis.

BACKGROUND: Chronic diseases are a major global health burden, contributing to morbidity, mortality and healthcare costs. Self-care is essential for effective disease management, with health literacy (HL) and digital health literacy (eHL) playing a role in enabling individuals to engage in health-promoting behaviours. However, the relationship between HL and self-care remains inconclusive, necessitating further investigation to clarify its impact. OBJECTIVE: To synthesise evidence on the association between HL and self-care in chronic diseases and identify mediating and moderating factors inf

Self-Sampling for Cervical Cancer Screening in Transmasculine and Nonbinary People.

Nearly all cases of cervical cancer are caused by persistent infection with high-risk human papillomavirus (HPV), and many cases of invasive cancer have been attributed to lack of screening. Transmasculine and nonbinary patients are more likely to be underscreened than cisgender women for a variety of reasons, and high-risk HPV self-sampling has been proposed as a method to increase screening uptake in this population. This narrative review is among the first to assess HPV self-sampling as a cervical cancer screening modality in transmasculine and nonbinary populations. Self-sampling for HPV d

Exploring the integration of thin-ideal internalization and self-objectification in the prevention of eating disorders.

Analyses of thin-ideal internalization and self-objectification were conducted within the context of a cognitive dissonance based eating disorder prevention program implemented in an undergraduate sorority. Participants completed self-report assessments at baseline (n=177), post-intervention (n=169), 5-month (n=159), and 1-year follow-up (n=105). Cross-sectional path analysis indicated that thin-ideal internalization and self-objectification predict each other and both predict body dissatisfaction, which in turn, predicts eating disorder symptoms. A longitudinal examination conducted using hie

Predicting adherence to prophylactic medication in adolescents with asthma: an application of the ASE-model.

An explanatory framework, referred to as the attitude/social influence/self-efficacy-model (ASE-model), was utilised to explain future self-reported adherence of adolescents to daily inhaled prophylactic asthma medication. The objective was to investigate the long-term influence of these earlier reported cognitive variables and other psychological and medical determinants on self-reported adherence 1 year later. Data were collected, via a questionnaire, from 86 adolescents with asthma (aged 11 through 18 years) recruited from outpatient clinics. Adherence was assessed by asking the patients to

Cerebellum and self-esteem in children and adolescents: the moderating effect of depression.

INTRODUCTION: Self-esteem, a critical determinant of psychological well-being, develops concurrently with brain maturation during childhood and adolescence. Emerging evidence has identified the involvement of the cerebellum in self-related processing and its structural links to self-esteem. Depression, highly prevalent in this developmental period, affects both self-esteem and cerebellar functions, but their interaction remains understudied in pediatric populations. METHODS: A community sample of 103 children and adolescents (mean age = 12.23 years) underwent structural magnetic resonance ima
