A Rotator Cuff Tear Concomitant With Shoulder Stiffness Is Associated With a Lower Retear Rate After 1-Stage Arthroscopic Surgery.

BACKGROUND: Few studies have reported on the radiological characteristics and repair integrity of coexistent rotator cuff tears (RCTs) and shoulder stiffness after simultaneous arthroscopic rotator cuff repair and capsular release. PURPOSE: To evaluate the radiological characteristics and repair integrity of 1-stage arthroscopic surgery of RCTs concomitant with shoulder stiffness. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Among patients who underwent arthroscopic repair of full-thickness RCTs, the stiff group underwent simultaneous capsular release for shoulder stiffness, a

Articular surface partial-thickness rotator cuff tears.

Between 1987 and 1992, one hundred eleven articular surface partial-thickness rotator cuff tears were diagnosed in 106 patients; 90 were men, and 16 were women. The average age was 42.5 years. Average follow-up was 32.3 months, ranging from 26 to 84 months. Patients were separated into three groups. Group 1 consisted of 85 shoulders in which impingement was believed to be the primary cause; these shoulders were treated with debridement of the tear and arthroscopic subacromial decompression. Group 2 consisted of 14 shoulders with instability treated with debridement of the partial tear and ante

Revision after anatomical total shoulder arthroplasty : the influence of age on outcomes in a large nationwide registry study.

AIMS: Primary anatomical total shoulder arthroplasty (aTSA) is generally the preferred treatment for active young patients with increased functional demands who need a shoulder arthroplasty. There is little information, however, about the survival of this implant in patients aged < 65 years. The aim of this study was to investigate the differences in revision rates after primary aTSA in patients aged < 65 years compared with those aged ≥ 65 years, within ten years, hypothesizing that younger patients would have higher revision rates due to increased functional demands and activity levels. Clinical and radiological outcomes of Inlay versus Onlay humeral stems in reverse shoulder arthroplasty with a 145° neck shaft angle: a multicentre retrospective study with a minimum follow-up of three years, an analysis from the registry of the shoulder friends institute.

BACKGROUND: While the original Paul Grammont Inlay design had a 155° neck-shaft angle (NSA), developments in humeral stem designs have led to the emergence of the Onlay design with a more vertical angle. The purpose of the study is to compare three year clinical and radiological outcomes of two humeral stem designs, namely Inlay versus Onlay designs, with a 145° NSA and identical glenoid component. METHODS: In this multicentric retrospective study, 227 patients (141 Inlay versus 86 Onlay) that underwent primary reverse shoulder arthroplasty (RSA) between March 2019 and April 2020, were review

Reverse shoulder arthroplasty in proximal humerus fractures: What is the optimal neck-shaft-angle?

PURPOSE: Limited evidence exists for functional outcomes of neck-shaft-angle (NSA) in reverse shoulder arthroplasty (RSA) for proximal humerus fractures (PHFs). We evaluated the functional outcomes of 155° versus 145° NSA RSA, to determine superiority for acute PHFs. METHOD: Retrospective analysis of 66-patients with displaced 3/4-part, head-split or fracture-dislocation PHFs, managed with acute RSA, within 4-weeks of injury at single trauma unit. 33-patients underwent 155° NSA RSA cemented stem; 33-patients 145° NSA RSA uncemented stem. All had same glenoid components with equivocal humeral s

Mid- to long-term outcome of reverse total shoulder arthroplasty as revision procedure for failed hemiarthroplasty after proximal humerus fracture.

BACKGROUND: Insufficient tuberosity healing is the most common reason for poor outcome after treatment of proximal humerus fractures (PHFs) using hemiarthroplasty (HA). In these cases, revision to reverse total shoulder arthroplasty (RTSA) can improve function and reduce pain in the short term, however, long-term results remain scarce. Aim of this study was to evaluate the clinical and radiological mid- to long-term results in patients with a revision RTSA after failed HA for PHF. METHODS: In this retrospective study all patients that received a revision to RTSA after failed fracture HA betwe

Does improved resection plane coverage during shoulder arthroplasty influence proximal humeral bone stress? A comparison of circular vs. elliptical humeral heads.

BACKGROUND: Stress shielding remains a concern following total shoulder arthroplasty using press-fit short humeral stems. While the effect of alterations in implant geometry, positioning, and sizing on stress shielding have been investigated, the effects of coverage of the cortical boundary of the resection plane have not yet been fully explored. The purpose of this study was to quantify the effect of improved cortical coverage using elliptical vs. circular humeral heads on changes in bone stress and resorbing potential. We hypothesized that better cortical coverage would reduce stress shieldi

Primary reverse total shoulder arthroplasty for fractures requires more revisions than for degenerative conditions 1 year after surgery: an analysis from the Dutch Arthroplasty Register.

BACKGROUND: Although reverse total shoulder arthroplasty (RTSA) is considered a viable treatment strategy for proximal humeral fractures, there is an ongoing discussion of how its revision rate compares with indications performed in the elective setting. First, this study evaluated whether RTSA for fractures conveyed a higher revision rate than RTSA for degenerative conditions (osteoarthritis, rotator cuff arthropathy, rotator cuff tear, or rheumatoid arthritis). Second, this study assessed whether there was a difference in patient-reported outcomes between these 2 groups following primary rep

Glenohumeral and scapulothoracic strength impairments exists in patients with subacromial impingement, but these are not reflected in the shoulder pain and disability index.

BACKGROUND: Pain and loss of function are cardinal symptoms associated with Subacromial impingement syndrome (SIS), while the presence and magnitude of deficits in strength and range of motion (ROM) are largely undescribed in non-athletic patients with SIS. Moreover, the relevance of impairments in strength and ROM to patient-reported shoulder function is not well described, even though testing of strength is recommended in clinical guidelines. The purpose of this study was, first, to investigate impairments in glenohumeral and scapulothoracic strength and in abduction and internal rotation RO

Range of motion as a predictor of clinical shoulder pain during recovery from delayed-onset muscle soreness.

CONTEXT: Athletic trainers use clinical pain and range of motion (ROM) to gauge recovery after musculoskeletal injury. Limited evidence to date suggests which shoulder ROM measures can predict symptomatic relief and functional recovery after delayed-onset muscle soreness (DOMS). OBJECTIVE: To determine whether shoulder passive internal rotation, passive external rotation, active abduction, and active flexion and evoked pain with abduction are associated with resting pain experienced after exercise-induced DOMS. DESIGN: Descriptive laboratory study. SETTING: Controlled research laboratory.

A new morphological classification for greater tuberosity fractures of the proximal humerus: validation and clinical implications.

In this study, we describe a morphological classification for greater tuberosity fractures of the proximal humerus. We divided these fractures into three types: avulsion, split and depression. We retrospectively reviewed all shoulder radiographs showing isolated greater tuberosity fractures in a Level I trauma centre between July 2007 and July 2012. We identified 199 cases where records and radiographs were reviewed and included 79 men and 120 women with a mean age of 58 years (23 to 96). The morphological classification was applied to the first 139 cases by three reviewers on two occasions us

Measuring shoulder injury function: common scales and checklists.

The increasing shift towards patient-centred healthcare has lead to an emergence of patient-reported outcome instruments to quantify functional outcomes in orthopaedic patients. Unfortunately, selecting an instrument for use in a shoulder trauma population is often problematic because most shoulder instruments were initially designed for use with chronic shoulder pathology patients. To ensure an instrument is valid, reliable, and sensitive to clinical changes, it is important to obtain psychometric evidence of its use in the target population. Four commonly used shoulder outcome instruments ar
