Use of the FLARe Smartphone App in a Trauma-Exposed Sample: A Feasibility Study.

Laboratory-based paradigms allow for the collection of multimodal assessments. A fear conditioning (FC) paradigm, capturing one key risk factor for posttraumatic stress disorder (PTSD) and alcohol use disorder, has been administered in a lab-based format. Recently, a smartphone-based application (app, specifically called "Fear Learning and Anxiety Response, FLARe") of this FC paradigm has been administered within control and anxiety samples. However, this app has not been administered to trauma-exposed individuals. Thus, the primary goal of this study was to test the feasibility of this app am

Comparing dropout from cognitive processing therapy versus prolonged exposure: Results from a randomized clinical trial.

OBJECTIVE: Dropout from evidence-based psychotherapies for posttraumatic stress disorder (PTSD; e.g., cognitive processing therapy [CPT] and prolonged exposure [PE]) remains a problem, with rates around 38%-51% in routine care settings. Research is needed to characterize predictors and timing of dropout to inform intervention efforts. Using data from the largest randomized controlled trial of CPT and PE to date, we examined the timing and predictors of dropout, including sociodemographic and psychosocial variables. METHOD: Participants (N = 916; average age = 45.2; 79.9% male; 64.4% White) we

A mixed-methods study of the development and evaluation of an Internal Family Systems (IFS)-based group intervention for post-traumatic cognitions and fear of compassion among trauma-exposed adolescents in Out-of-Home Care (OOHC).

BACKGROUND: Adolescents in Out-of-Home Care (OOHC) often suffer multiple Adverse Childhood Experiences, leading to high rates of distorted Post-Traumatic Cognitions (PTC) and Fear of Compassion (FC). Tailored, evidence-based psychological interventions are essential for this group. OBJECTIVE: This mixed-methods study aimed to design, validate, and empirically evaluate a Internal Family Systems (IFS)-based group intervention for trauma-exposed adolescents in Out-of-Home Care (OOHC). PARTICIPANTS AND SETTING: Fifty trauma-exposed adolescents (27 female, 23 male) were purposively selected from

Mechanisms of change in mobile app-delivered acceptance and commitment therapy for posttraumatic stress disorder in China: a secondary analysis of a randomized controlled trial.

Background: Research has demonstrated the effectiveness of a mobile app-delivered Acceptance and Commitment Therapy (IACT) programme for individuals with posttraumatic stress disorder (PTSD) in their PTSD symptoms. However, there are still few studies on its mediating mechanism. Objective: This secondary analysis aimed to examine whether six components of psychological flexibility - acceptance, present-moment awareness, self-as-context, cognitive defusion, valued living, and committed action - mediate the effects of an IACT programme for individuals with PTSD.Method: Data were obtained from a r

Cracks in the foundation: disrupted epistemic trust as a link between trauma, PTSD and disturbances in self-organization.

Traumatic experiences profoundly impact mental and physical health, alongside an individual's trust in their social environment. Disruptions in epistemic trust - the capacity to evaluate and trust socially communicated information - are theorized to increase vulnerability to psychopathology by impairing social learning. This cross-sectional study investigated associations between trauma characteristics and epistemic stance using self-report data from a representative German sample (N = 2 519). We examined relationships among trauma type, age at trauma, epistemic stance, PTSD and cPTSD. Neither

Repairing Attachment Insecurities with God and PTSD Symptomology in Adult Survivors of Childhood Trauma: An Exploratory Study Using Spiritually Integrated Cognitive Processing Therapy (SICPT).

Childhood trauma occurs when an individual under the age of 18 survives or witnesses a life-threatening event that surpasses one's ability to cope. To circumvent future danger, numerous developmental changes occur in response to fear, manifesting in a multitude of symptoms that can culminate in posttraumatic stress disorder (PTSD). This debilitating condition arises from the brain and body's heightened sensitivity to environmental stimuli, including interpersonal interactions, which affect attachment styles with others and with God. Research has indicated that high levels of self-reported spir

Mapping moral injury: Development and feasibility of the Clinical Interview for Moral Injury-military version (CIMI-M).

OBJECTIVE: There is currently no clinical interview to assess exposure to potentially morally injurious experiences (PMIEs) and moral injury, the potential clinical outcome associated with PMIEs. To enable a person-centered assessment of PMIEs and moral injury in military veterans, we developed a structured, non-diagnostic clinical interview to support descriptive assessment, case conceptualization, and treatment planning, and conducted an initial evaluation of its feasibility, acceptability, and clinical utility. METHOD: The Clinical Interview for Moral Injury-Military Version (CIMI-M) was d

A self-guided digital mental health intervention for Syrian refugees in Germany and Sweden: effects from two pragmatic randomized controlled trials.

BACKGROUND: Syrian refugees across diverse host countries, including high-income European countries, face increased mental health needs. Digital interventions can scale support, but global scalability limits human guidance and contextual adaptations. We evaluated the effectiveness of a potentially scalable digital intervention (Step-by-Step; SbS) with minimal contact-on-demand (COD) in reducing psychological distress and functional impairment among Syrian refugees in Germany and Sweden. These trials were conducted in parallel with SbS studies in Egypt and Lebanon, using the same content to tes

Deepfake perpetrator conversation for adults with sexual abuse-related posttraumatic stress disorder: intervention development and multiple baseline study protocol.

Background: A conversation between a victim and a perpetrator of sexual abuse has the potential to reduce posttraumatic stress disorder (PTSD) symptoms in the victim. However, an actual conversation may not always be possible or feasible. Deepfake technology may offer a way to simulate a conversation in a clinical context. Therefore, a feasible, acceptable and potentially effective intervention protocol is required. Objective: We aimed to develop an intervention protocol for a deepfake victim-perpetrator conversation to be used in the treatment of patients with sexual abuse-related PTSD, that w

Distinct and shared neurobiological patterns in PTSD and CPTSD: evidence from resting-state fMRI.

BACKGROUND: Complex Post-Traumatic Stress Disorder (CPTSD) is a diagnosis newly introduced in the ICD-11. However, its status as a distinct disorder from Post-Traumatic Stress Disorder (PTSD) remains debated, and there is a notable lack of neuroimaging evidence derived from resting-state functional magnetic resonance imaging (rs-fMRI) to clarify this distinction. This study aimed to address this gap by providing integrated neuroimaging evidence to elucidate their unique and shared neural bases. METHODS: By collecting resting-state functional brain imaging data from patients with PTSD, CPTSD,

Experiential Avoidance as a Mediator of the Relationship Between Posttraumatic Stress Disorder Symptoms and Intimate Partner Abuse Perpetration.

Past research has established trauma and posttraumatic stress disorder (PTSD) as important risk factors for intimate partner violence (IPV) perpetration, focusing on hyperarousal and cognitive biases as explanatory mechanisms. The current study investigated whether experiential avoidance (EA), the tendency to avoid unwanted private experiences, helps explain the association between PTSD symptoms and IPV perpetration. Seventy-four men receiving IPV intervention services completed assessments of trauma exposure, PTSD symptoms, and IPV perpetration, and responded to hypothetical distressing relat

The impact of patients suicide on the mental health of psychotherapists.

A great number of psychotherapists and psychiatrists experience at least one patient suicide during their career. This study aims to build upon previous research findings concerning the impact of experienced patient suicide on the mental health of practicing and trainee psychotherapists in a German sample. A central aspect involves examining the temporal development of perceived distress and identifying short- and long-term emotional effects of suicide through a mixed methods approach to derive corresponding implications for postvention. The online survey revealed relatively low levels of symp
