Radiation-induced bilateral optic neuropathy in cancer of the nasopharynx. Case failure analysis and a review of the literature.

CASE REPORT: A case history of unanticipated radiation-induced bilateral optic neuropathy, 18 months after induction chemotherapy and radiation therapy for a locally advanced nasopharyngeal carcinoma, is presented. Retrospective reanalysis of the radiation therapy technique, with emphasis on the doses received by the optic pathway structures, was performed. These re-calculations revealed unexpectedly high doses in the range 79 to 82 Gy (cumulative external and brachytherapy dose) at the level of the optic nerves, which explained the observed radiation injury. CONCLUSION: Routine implementatio

Optic nerve and susceptibility imaging at asymptomatic stage of multiple sclerosis: impact and predictive value.

Optic nerve is now included as a fifth typical location in multiple sclerosis diagnosis criteria. The radiologically isolated syndrome represents the earliest stage of multiple sclerosis. Previous optical coherence tomography studies in this asymptomatic context reported no or slight retinal thickness difference compared to healthy subjects. Frequency of asymptomatic optic nerve lesions has never been evaluated at this stage of the disease. Susceptibility-weighted imaging findings on brain MRI are incorporated in the recent revised multiple sclerosis diagnostic criteria (2024) through the 'cen

Discriminant analysis models for early detection of glaucomatous optic disc changes.

AIM: To evaluate and compare four different mathematical formulas for the early detection of morphometric optic nerve head changes in chronic open angle glaucoma. METHODS: The optic nerve heads of 161 patients with perimetrically defined glaucomatous optic nerve damage and of 194 normal subjects were examined by confocal laser scanning tomography. Using four formulas of linear discriminant analysis and the optic cup shape measure as the single optic disc variable, the predictive power of each of these methods was examined to differentiate between the normal eyes and the glaucoma eyes. Macular ganglion-cell-complex layer thinning and optic nerve integrity in drug-naïve Parkinson's disease.

To reveal the macular inner retinal change linked to axonal degeneration in Parkinson's disease (PD), we performed macular optical coherence tomography scan and diffusion tensor imaging of the retrobulbar optic nerve on both eyes of 36 drug-naïve PD patients. Thicknesses of inner retinal layers were automatically measured, and correlation analysis was conducted between the retinal thickness and diffusion parameters of the optic nerve. PD patients showed thinning of the inner retinal layers compared to control data. Thicknesses of the ganglion cell and inner plexiform layers were both correlate

Anterior ischemic optic neuropathy associated with udenafil.

We report a case of anterior ischemic optic neuropathy associated with udenafil. A 54-year-old male presented with an acute onset visual field defect of the right eye after udenafil use. Examination revealed a relative afferent pupillary defect and a swollen disc. Automated visual fields revealed an enlarged blind spot and a narrowed visual field. Fluorescein angiography revealed both an inferior choroidal filling delay and an inferior sector filling delay of the optic disc in the arteriovenous phase as well as diffuse leakage of the optic disc in the late phase. Optical coherent tomography re

Blindness after maxillofacial blunt trauma. Evaluation of candidates for optic nerve decompression surgery.

Clinical and computerized tomography (CT) findings in 10 patients with post-traumatic blindness after maxillofacial blunt trauma were evaluated as potential candidates for optic nerve decompression surgery. The total number of blind eyes was 14. All patients suffered from midfacial fractures (isolated zygomatic fracture in 4 and Le Fort fracture in 6 cases) predominantly caused by road traffic accidents. CT scanning and clinical findings allowed location of damage to the optic nerve region in all patients. The most common CT finding was swelling of the optic nerve. Fracture in the optic canal

[Combined assessment of early-stage primary open-angle glaucoma progression].

A total of 92 patients (92 eyes), including 52 patients with early-stage primary open-angle glaucoma and 40 glaucoma suspects, were enrolled in the study and followed up during 25.5 +/- 3.3 months on the average by means of computer perimetry, optical coherence tomography and Heidelberg retinal tomography. The prognoses for POAG progression made on the basis of these tests using both conventional (conservative) and extended (liberal) evaluation criteria demonstrated poor correspondence. It is suggested to consider glaucoma progression confirmed if characteristic changes are registered by at le

Adaptive optics retinal imaging: emerging clinical applications.

The human retina is a uniquely accessible tissue. Tools like scanning laser ophthalmoscopy and spectral domain-optical coherence tomography provide clinicians with remarkably clear pictures of the living retina. Although the anterior optics of the eye permit such non-invasive visualization of the retina and associated pathology, the same optics induce significant aberrations that obviate cellular-resolution imaging in most cases. Adaptive optics (AO) imaging systems use active optical elements to compensate for aberrations in the optical path between the object and the camera. When applied to

Microcystic inner nuclear layer abnormalities and neuromyelitis optica.

IMPORTANCE: Microcystic abnormalities involving the inner nuclear layer of the retina occurs in a subset of patients with multiple sclerosis, most commonly in eyes previously affected by symptomatic optic neuritis. Acute optic neuritis is a cardinal manifestation of neuromyelitis optica (NMO). To our knowledge, microcystic inner nuclear layer abnormalities have not been investigated in NMO. OBJECTIVE: To establish whether microcystic inner nuclear layer abnormalities occur in NMO. DESIGN: Observational, retrospective study. SETTING: University of California at San Francisco Multiple Scleros

[Optic neuropathy induced by thinner sniffing].

A case of optic neuropathy induced by thinner sniffing is reported. A 17-year-old girl, who had been sniffing a lacquer thinner for three months, suffered acute blindness followed by optic atrophy. Brain computed tomography revealed symmetrical low attenuation areas in the bilateral putamen. An analysis of the thinner by gas chromatography showed that its major components in a vaporized state were methyl alcohol and metyhl acetate. The optic neuropathy was induced by these solvents. In the diagnosis of the intoxication of mixed organic solvents, the measurement of each solvent in its vapor pha

Tuberculous Subretinal Abscess in a Non-HIV Patient with Miliary Tuberculosis.

This case report describes the features of a tuberculous subretinal abscess in a non-HIV patient. It includes the characterization of the lesion with spectral-domain optical coherence tomography (SD-OCT) and fundus autofluorescence examination. SD-OCT and autofluorescence imaging showed external retinal involvement during development of the tuberculous subretinal abscess. Patients with tuberculous lesions need a multidisciplinary approach, since the disease may involve any part of the body. In this case in particular, incidental findings at computed tomography scan led to the detection of this

Non-invasive and invasive imaging of vulnerable coronary plaque.

Vulnerable plaque is characterized by a large necrotic core and an overlying thin fibrous cap. Non-invasive imaging modalities such as computed tomography angiography (CTA) and magnetic resonance imaging (MRI) allow for the assessment of morphological plaque characteristics, while positron emission tomography (PET) enables the detection of metabolic activity within the atherosclerotic lesions. Invasive imaging modalities such as intravascular ultrasound (IVUS), optical-coherence tomography (OCT), and intravascular MRI (IV-MRI) display plaques at a high spatial resolution. Near-infrared spectro

The pull of the macula: Optic disc dragging in large macular coloboma.

Macular coloboma is an excavation at the macula with defective or rudimentary retinal tissue. Here, we present an unusual case of large bilateral macular coloboma with dragging of optic disc. A 48-year-old diabetic male patient presented to our outpatient department with a history of diminution of vision and squint since childhood. Dilated fundus examination revealed bilateral large well-defined excavation at the macula with optic disc displaced temporally and temporal arcade vessels appeared straightened. Rest of the retina was normal. Optical coherence tomography and B-scan ultrasonography f

Bilateral Optic Nerve Head Drusen Complicated by Choroidal Neovascularization Misdiagnosed as Papilledema and Neuroretinitis.

We report a case of bilateral optic nerve head drusen complicated by choroidal neovascularization (CNV) in the left eye at presentation. The presence of optic disc and macular edema in addition to exudation led to the misdiagnosis of neuroretinitis at an outside medical center. Swept-source optical coherence tomography (SS-OCT) and SSOCT angiography were critical in establishing the diagnosis and follow-up in a noninvasive manner. Secondary CNV associated with optic nerve head drusen responded well to intravitreal injections of anti-vascular endothelial growth factor in the left eye. Asymptoma

Use of ganglion cell layer analysis for diagnosing anti-glycoprotein neuromyelitis optica of oligodendrocyte myelin.

The case concerns a 26-year-old patient with bilateral recurrent optic neuritis episodes in the context of suspected neuromyelitis optica. In the first outbreak, she had greatly impaired visual acuity of the left eye, as well as seeing ganglion cell layer damage in both eyes in the optic coherence tomography, with evidence of a possible extensive lesion in the optic chiasma. Likewise, MRI with contrast showed a great involvement of the left optic nerve that compromises the chiasma increasing the suspicion of a neuromyelitis origin. Althogh the anti-myelin oligodendrocyte glycoprotein (MOG) and
