Unexpected hemoglobin electrophoresis results following red cell exchange in a sickle cell anemia patient with acute chest syndrome.

Acute chest syndrome is a well described complication of sickle cell anemia. It is characterized by fever, pulmonary infiltrates, pleuritic chest pain and abnormal pulmonary auscultation. Transfusion therapy, either simple transfusion of red blood cells or a total red blood cell exchange, is a cornerstone therapy for these patients. Exchange transfusion is preferred when an acute reduction of the hemoglobin S (HbS) concentration is the therapeutic goal since it allows one to rapidly reduce the percent HbS without increasing blood viscosity or volume (Wayne, Kevy and Nathan, Blood 1993; 81:1109

Efficiency and safety of a technique for drawing blood from the hamster cranial vena cava.

The authors evaluated the efficiency and safety of a technique for drawing blood from the cranial vena cava of the hamster (Mesocricetus auratus). They collected blood from Golden Syrian hamsters (n = 15) and from epileptic hamsters (GASH/Sal strain; n = 10) that were anesthetized with isoflurane. Four epileptic hamsters constituted a control group that underwent anesthesia but not blood collection. For blood sampling, a needle connected to a syringe was inserted at a site between 0.2 and 0.6 cm from the manubrium; the depth of insertion was 0.3-1.0 cm. Blood collection was successful in all h

Complex flow patterns in the great vessels: a review.

The article reviews the applications of magnetic resonance velocity mapping based on phase shifts in the protons to quantify blood flow velocity and blood flow volume. The method can be used to study normal physiology of blood flow in the aorta and its major branches, including forward and backward flow, to measure the aortic valve function in aortic valvular disease, stenosis and regurgitation, as well as pulmonary artery flow velocities in pulmonic insufficiency and regurgitation. Superior vena cava flows, pulmonary vein flows, left-to-right shunts, atrial and ventricular pulmonary conduit f

[Erythropoietin and hypertension].

Recombinant human erythropoietin (rhEpo) has been demonstrated in several studies to be effective in correcting the anemia of regular dialysis patients. This was accompanied by a significant improvement of the physical work capacity shown by exercise testing. The main side effect of rhEpo treatment has been the development or aggravation of hypertension in approximately 30% of the treated patients. In 2% hypertensive encephalopathy and convulsions occurred. Data obtained by measurements of regional blood flow indicate the peripheral resistance did increase probably due to rise of blood viscosi

Effective antihypertensive therapy: blood pressure control with moxonidine.

Stimulation of the imidazoline I1-receptor represents a new mode of antihypertensive action, inhibiting peripheral alpha-adrenergic tone by a central mechanism. Moxonidine is an imidazoline I1-receptor modulator. Acute hemodynamic studies indicate that moxonidine results in an acute fall of both blood pressure and systemic vascular resistance, whereas heart rate, cardiac output, stroke volume, and pulmonary artery pressures are not affected. The ejection fraction is not significantly affected. Left ventricular end-systolic and -diastolic volumes are reduced. There is regression of left ventric

MRI of Potassium and Sodium Enables Comprehensive Analysis of Ion Perturbations in Skeletal Muscle Tissue After Eccentric Exercise.

OBJECTIVES: The aims were to investigate if potassium ( 39 K) magnetic resonance imaging (MRI) can be used to analyze changes in the apparent tissue potassium concentration (aTPC) in calf muscle tissue after eccentric exercise and in delayed-onset muscle soreness, and to compare these to corresponding changes in the apparent tissue sodium concentration (aTSC) measured with sodium ( 23 Na) MRI. MATERIALS AND METHODS: Fourteen healthy subjects (7 female, 7 male; 25.0 ± 2.8 years) underwent 39 K and 23 Na MRI at a 7 T MR system, as well as 1 H MRI at a 3 T MR system. Improved Hemodynamic Recovery and 72-Hour Survival Following Low-Volume Resuscitation with a PEGylated Carboxyhemoglobin in a Rat Model of Severe Hemorrhagic Shock.

INTRODUCTION: Hemorrhage is a leading cause of death from potentially survivable civilian and military trauma. As projected conflicts move from settings of tactical and logistical supremacy to hyper-dynamic tactical zones against peer and near-peer adversaries, protracted medical evacuation times are expected. Treatment at the point-of-injury is critical. Although crystalloids like Lactated Ringer's (LR) are ubiquitous, whole blood (WB) is the preferred resuscitation fluid following hemorrhage; however, logistical constraints limit the availability of WB in prehospital settings. Hemoglobin-bas

[Prospective randomized controlled trial on the effectiveness of low-dose and high-dose intravenous tranexamic acid in reducing perioperative blood loss in single-level minimally invasive transforaminal lumbar interbody fusion].

OBJECTIVE: A prospective randomized controlled trial was conducted to study the effectiveness and safety of intravenous different doses tranexamic acid (TXA) in single-level unilateral minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF). METHODS: The patients treated with single-level unilateral MIS-TLIF between September 2019 and October 2020 were enrolled and randomly classified into low-dose TXA (LD) group (n=39), high-dose TXA (HD) group (n=39), and placebo-controlled (PC) group (n=38). The LD, HD, and PC groups received intravenous TXA 20 mg/kg, TXA 50 mg/kg, the same vo

Protocol for profiling circulating T cell subsets, their homing and regulatory markers, in pediatric samples using a 4-laser BD LSRII.

Here, we present a protocol for profiling T cell subsets, regulatory phenotypes, and gut- and skin-homing markers of unstimulated cells or those stimulated with staphylococcal enterotoxin B (SEB), lipopolysaccharide (LPS), or zymosan (ZYM). We describe steps for isolation, cryopreservation, thawing, stimulation, and multiparameter flow cytometric analysis of human peripheral blood mononuclear cells (PBMCs). Use of the multicolored panel detailed in this protocol allows full utilization of the valuable samples with the minimal possible volume of collected pediatric blood. For complete details o

Postoperative Myocardial Infarction in Acute Type A Aortic Dissection: A Case Report.

Postoperative myocardial infarction (POMI) in acute type A aortic dissection rarely has been reported, we report a case of postoperative myocardial infarction in acute type A aortic dissection, and the clinical presentation and possible mechanisms are described. This case illustrates that postoperative myocardial infarction in acute type A aortic dissection is a rare fatal complication in patients without coronary lesions or coronary malperfusion before aortic repair. Type 2 myocardial infarction may account for POMI. Effective treatment may include anticoagulation therapy, volume adjustment,

Orthostatic Hypotension in Parkinson Disease.

Orthostatic hypotension (OH) is a sustained fall in blood pressure on standing that can cause symptoms of organ hypoperfusion. OH is associated with increased morbidity and mortality and leads to a significant number of hospital admissions. OH can be caused by volume depletion, blood loss, cardiac pump failure, large varicose veins, medications, or defective activation of sympathetic nerves and reduced norepinephrine release upon standing. Neurogenic OH is a frequent and disabling problem in patients with synucleinopathies such as Parkinson disease, multiple system atrophy, and pure autonomic

Repeated phlebotomies decrease body iron storage in adult dogs.

The aim of the present study was to determine changes in body iron storage in adult dogs following phlebotomy. We performed repeated phlebotomies by removing 1% body weight (approximately 10% of the total blood volume) weekly for a total of 12 times using adult beagle dogs without an iron-restricted diet. After treatment, stored iron was decreased, as demonstrated by gradual reductions in serum ferritin levels and hepatic iron contents. Anemia and abnormalities in blood chemistry analysis were not observed; therefore, this method was considered safe and useful for control of stored iron levels…

Concomitant Takayasu arteritis and Cushing syndrome in a child undergoing open adrenalectomy: An anaesthetic challenge.

Takayasu's arteritis (TA) is a rare, chronic progressive panendarteritis involving the aorta and its main branches. Anaesthesia for patients with TA is complicated by their severe uncontrolled hypertension, end-organ dysfunction, stenosis of major blood vessels, and difficulties encountered in monitoring arterial blood pressure. In a patient with Cushing's syndrome (CS), the anaesthesiologist needs to deal with volume overload, hyperglycaemia, hypokalaemia, difficult airway and ventilation. Anaesthetic management of a patient with concomitant TA and CS undergoing adrenalectomy has hardly ever

Recurrent unconsciousness due to bilateral middle cerebral artery stenosis.

Unconsciousness may be caused by transient ischemia of the bilateral hemisphere or brain stem. We report a case of recurrent unconsciousness in which angiography confirmed an occluded left middle cerebral artery (MCA) with well-established collateral compensation, along with a severely stenotic right MCA with poor collateral compensation, which was consistent with decreased cerebral blood flow and cerebral blood volume seen on a computed tomography perfusion study. A stent was placed in the patient's right MCA, after which the symptom disappeared completely. This case demonstrates that bilater…

High output cardiac failure in a parturient with hereditary haemorrhagic telangiectasia.

Hereditary haemorrhagic telangiectasia is a genetic condition which results in arteriovenous malformations involving the skin, mucous membranes, lung, brain, gastrointestinal tract, liver and spinal canal. The shunting of blood through arteriovenous malformations, especially in the liver,; leads to maldistribution of cardiac output. In order to supply blood to vital organs, cardiac output is increased through vasodilation, elevated stroke volume and elevated heart rate. Pregnancy can worsen the effects of the arteriovenous malformations. We present the peripartum management of a woman with her

Complications of fluid therapy.

The intravenous administration of fluids is one of the most important aspects of patient care in hospitalized animals. Intravenous fluids are administered to replace or prevent dehydration, treat hypovolemic shock and intravascular volume depletion, correct acid-base and electrolyte abnormalities, and maintain vascular access for administration of drugs, blood product components, and parenteral nutrition. Intravenous catheterization also can provide a means of blood sample collection, thus avoiding frequent and uncomfortable venipunctures in critically ill animals. Although the benefits of int

A dual contrast injection technique for multidetector computed tomography angiography of Fontan procedures.

Extracardiac and lateral tunnel Fontan palliations result in caval venous return to the branch pulmonary arteries without prior blood mixing in the right atrium. Single contrast injection techniques result in suboptimal pulmonary computed tomography angiography since blood streaming results in asymmetric pulmonary flow of injected contrast. We used a dual injection technique--dorsal foot vein and upper extremity vein to achieve optimal simultaneous opacification of the inferior and superior venae cavae and branch pulmonary arteries. The resulting dataset allowed for excellent 3D volume renderi

[Localized right atrial tamponade after aortic valve replacement].

A 78-year-old man developed isolated right atrial tamponade 15 hours following aortic valve replacement. There were excessive postsurgical bleeding, low blood pressure, and low cardiac output. Volume expansion and inotropic therapy did not increase blood pressure. There were no cardiomegaly and echo-free space. Diagnosis was made by appearance of pulsus paradoxus and transthoracic echocardiography and confirmed by surgery. The clinical picture was improved dramatically after surgical removal of the hematoma. Right atrial tamponade leads to a unique clinical conglomeration of hemodynamic and ec

The authors had 32 patients with gastroduodenal hemorrhage of ulcerous etiology under observation. Enteral correction of the blood loss with an electrolyte-monomer solution in such patients increases systemic oxygen transport (SOT) through improvement of central hemodynamics in the posthemorrhagic and postoperative periods. In respect of percentage SOT increased equally both in enteral correction and in the traditional infusion-transfusion therapy, oxygen consumption did not reduce in the periods of examination (1st, 3rd, 7th day). [Diagnosis of chronic fetal anoxia].

The intrauterine fetal status was assessed in women at 34-42 week gestation; the pregnancies were complicated by obstetric and extragenital disease (nephropathy, prolonged gestation, hypertension, diabetes mellitus, renal disease). Computer-assisted analysis of the diagnostic value of fetal cardiotocography, uterine-placental hemodynamics, and acid-base status of maternal capillary blood showed that the following parameters are most informative for the diagnosis of chronic fetal hypoxia: stable rhythm duration; oscillation amplitude; acceleration/deceleration number, amplitude, and duration; v
