INTRODUCTION: The difficulties of managing bone loss in the distal tibia are well known in the literature. The various therapeutic options available to us include custom prosthetic replacement, talocrural arthrodesis with allograft, vascularised or non-vascularised autograft, bone transfer according to Illizarov and insertion of a metal augment. In the case of non-conservation of the talocrural joint, osteosynthesis is performed using adapted plates and screws or, more conventionally, transplanted centromedullary nailing. We report on a clinical case of bone loss in the distal tibia in an infe

Aseptic loosening and infection are still the main reasons for revision surgery. The contact of the implant is made through the surface, which is coated with calcium phosphate ceramics for better integration into the bone. The aim of the present work is to optimize these coatings by making them thinner (20 μm) and doping them with copper to add antimicrobial functionality. Four different coating materials were used: hydroxyapatite, Bioglass, GB14, and beta tricalcium phosphate. The coatings were applied by high velocity suspension flame spraying. The titanium rods with the coatings were then i

Comminuted fractures, characterized by multiple irregular bone fragments, present significant challenges for traditional fixation methods like plates and screws, often resulting in complex surgeries and delayed healing. Injectable hydrogel adhesives have been applied for comminuted fractures, but face challenges such as excessive fluidity before curing and curing processes that involve high temperatures or ionic release, which can result in material leakage, localized inflammation and reduced adhesion. To address these challenges, we developed a humidity-responsive hydrogel adhesive system, co

BACKGROUND: Reconstruction of the skeletal defects resulting from the resection of bone tumors remains a considerable challenge and one of the possibilities is the orthotopic replantation of the irradiated bone autograft. One technical option with this technique is the addition of a vital autologous fibular graft, with or without microvascular anastomosis. The aim of our study was to evaluate the clinical results of the treatment of our patient cohort with a specific view to the role of fibular augmentation.
METHODS: Twenty-one patients with 22 reconstructions were included. In all cases, the

INTRODUCTION: Mucopolysaccharidosis I (MPS I) is a lysosomal storage disorder characterized by deficient α-l-iduronidase activity leading to accumulation of poorly degraded dermatan and heparan sulfate glycosaminoglycans (GAGs). MPS I is associated with significant cervical spine disease, including vertebral dysplasia, odontoid hypoplasia, and accelerated disk degeneration, leading to spinal cord compression and kypho-scoliosis. The objective of this study was to establish the nature and rate of progression of cervical vertebral bone disease in MPS I using a canine model.
METHODS: C2 vertebra

INTRODUCTION: Lateral locked plating (LLP) development has improved outcomes for distal femur fractures. However, there is still a modest rate of nonunion in fractures treated with LLP alone, with higher nonunion risk in high-energy fractures, intra-articular involvement, poor bone quality, severe comminution, or bone loss. Several recent studies have demonstrated both the safety and the biomechanical advantage of dual medial and lateral plating (DP). The purpose of this study was to evaluate the clinical outcomes of DP for native distal femoral fractures by performing a systematic review of t

BACKGROUND CONTEXT: Vertebral osteolysis has been reported as a complication of off-label recombinant human bone morphogenetic protein-2 (rhBMP-2) use in transforaminal lumbar interbody fusion (TLIF). It has been postulated that end plate violation during disc space preparation, rhBMP-2 overdosing, or a combination thereof can contribute to the development of vertebral osteolysis when rhBMP-2 is used in the lumbar interspace.
PURPOSE: To present two cases of vertebral osteolysis that occurred after TLIF in which rhBMP-2 was used. In each case, the osteolysis originated from subchondral cysts

Background: Distal radioulnar joint (DRUJ) instability associated with forearm bone deformity is difficult to treat because residual osseous malalignment may impair forearm rotation even after triangular fibrocartilage complex (TFCC) reconstruction. This study evaluated the clinical usefulness of preoperative three-dimensional (3D) computer simulation and patient-matched instruments (PMIs) for corrective osteotomy in deformity-related DRUJ instability. Methods: Three patients with chronic DRUJ instability associated with forearm bone deformity underwent corrective osteotomy based on 3D compute

INTRODUCTION AND IMPORTANCE: Pathological mandibular fractures, defined as fractures that occur in regions where bone has been weakened by an underlying pathological process, are rare, accounting for fewer than 2 % of all fractures of the mandible. Mandibular pathological fractures can have several aetiologies including osteomyelitis, osteoradionecrosis (ORN), surgical interventions, bisphosphonate-related osteochemonecrosis of the jaw and tumoral lesions.
CASE PRESENTATION: We present a case of a 13-year-old male patient following a tooth infection with a right facial swelling and limited mo

Introduction The radial bone and the radioulnar joint are vital for the physiological and physical stability of the elbow. The prostheses and plates used in cases of radius fracture are designed based on the morphology of the Western population. This could result in a bone-implant mismatch when applied to the Indian population, resulting in complications. Hence, the study aimed to record the normal values of radius morphology in the Indian population. Methods A total of 30 (eight male and seven female) freshly frozen cadaveric bilateral upper limbs were chosen. Cadavers with previous surgical

BACKGROUND: Pseudoarthrosis, or failure to achieve bony union, is a well-known complication of spinal fusion operations. Rates range from 5% to 40% and are influenced by both patient and technical factors. Patients who do not achieve complete fusion may experience a return or worsening of their preoperative pain. For patients with complicated pathologies, vascularized bone grafts (VBGs) have been shown to provide better outcomes than nonvascularized bone grafts (N-VBGs).
OBJECTIVE: To enhance an instrumented spinal fusion by the innovative technique presented herein that utilizes a rotated, p

OBJECTIVE: Nitric oxide (NO) has been implicated in the local regulation of bone metabolism. However, the contribution made by specific nitric oxide synthase (NOS) enzymes to skeletal development is unclear. The objective of this study was to examine the effects of inactivation of neuronal nitric oxide synthase (nNOS) on cartilage development in mice.
DESIGN: Mice carrying a null mutation in the nNOS gene were used to address our objectives. Histological staining, immunohistochemistry and in situ analyses were employed along with real-time reverse transcriptase - polymerase chain reaction (RT

Enzyme replacement therapy is currently available for three of the mucopolysaccharidoses (MPSs) but has limited effects on the skeletal lesions. We investigated the involvement of the Toll-like receptor 4 (TLR4) signaling pathway in the pathogenesis of MPS bone and joint disease, and the use of the anti-TNF-alpha drug, Remicade (Centocor, Inc.), for treatment. TLR4 KO (TLR4(lps-/-)) mice were interbred with MPS VII mice to produce double-KO (DKO) animals. The DKO mice had longer and thinner faces and longer femora as revealed by micro-computed tomography analysis compared with MPS VII mice. Hi

OBJECTIVE: To study the surgical reconstruction of secondary fracture deformities of the nasal-orbital ethmoid area.
METHODS: Typical bicoronal and subciliary incisions or the adjacent scar incision were employed to expose the fractured area. The flattened nasal bone was narrowed by curved osteotomy along the medial orbital rims and trimed with a bur. 2-3 pieces of cranial outer table were used to reconstruct the nasal framework, which were fixed to the frontal bone with mini-plates. After the medial orbital wall and orbital floor were exposed, the herniated orbital contents were released and

MR imaging is the modality of choice for the detection, staging, and differential diagnosis of inflammatory disorders of the spine. Infectious spondylitis is characterized by the involvement of two adjacent vertebrae and the intervening disk with severe BME and early destruction of the end plates. The disk space is narrowed and typically exhibits water-equivalent signal intensity on T2-weighted or STIR images. Prevertebral and epidural extensions, abscess formation, enhancement of the BME, the disk space, and the surrounding granulation tissue are well demonstrated by gadolinium-enhanced image

Achondroplasia, associated with gain-of-function mutations in FGFR3, causes growth plate cartilage dysfunction, resulting in short-limb dwarfism. However, its precise molecular and cellular mechanisms remain unclear. To address this, we aimed to generate knock-in mice (Fgfr3Ach) harboring the achondroplasia mutation (p.Gly380Arg). In addition to previously reported abnormalities, we observe an expansion of the resting zone. EdU labeling and lineage tracing analyses indicate that disruption of turnover and impairment of stem cell-like behavior of resting zone chondrocytes results in accumulatio

Complex intra-articular injuries like proximal tibial fractures need validated patient-reported functional outcomes. Functional outcomes of 30 adults undergoing locking plate ORIF for closed proximal tibia fractures were assessed via KOOS at 1.5, 3 and 6 months postoperatively (September 2022-February 2024). The cohort consisted of middle-aged males (76.7%, mean age 44.33 ± 13.19 years) with the most common fracture patterns being Schatzker Type-5 (46.7%) and Type-6 (30%). Patients experienced bone union at 15.03 ± 1.61 weeks without complications. Overall KOOS scores improved significantly fr

INTRODUCTION: Giant cell tumors of the bone (GCTB) are challenging neoplasms, particularly in the proximal humerus.
CASE REPORT: We report the case of a 46-year-old male with proximal humerus GCTB who was managed with extended curettage, proximal humerus internal locking system (PHILOS) plating, and fibular strut grafting. The patient presented with shoulder pain and restricted movement. Imaging revealed a lytic lesion in the metaphyseal-epiphyseal region, and a biopsy confirmed the diagnosis. Extended curettage was performed using the deltopectoral approach, followed by cryotherapy and recon
