The Müller acetabular support ring. A preliminary review of indications and clinical results.

A series of 41 total hip arthroplasties with acetabular reinforcement by a Müller support ring is reviewed. The postoperative management was the same as that used in conventional cemented total hip arthroplasty. The overall clinical results and the radiological findings were most satisfactory after one to six years. Of the 30 hips available for functional assessment using the Merle d'Aubigné scale, 16 were excellent, 10 good, 2 fair, 1 poor and 1 bad. These results indicate that, regardless of the primary disorder, the Müller support ring has been helpful when dealing with severe osteoporosis,

[Injuries of the proximal humeral epiphysis. Indications for surgical therapy and results].

Lesions of the proximal humeral epiphyseal plate occur most frequently as epiphysiolysis with metaphyseal fragment between the age of 12 and 16 years. The conservative treatment with Desault plaster cast is recommended for stable fractures. In unstable fractures the percutaneous K-wire osteosynthesis after reduction is the adequate procedure. Hypertrophic scars were found after open reduction in half of all patients treated. Only when closed reduction was impossible, the fracture should be exposed and stabilised with K-wires and/or lag screws. All 21 patients who were followed-up have been fre

Chronic Thigh Pain in an Adolescent: A Case Report and Literature Review of Desmoplastic Fibroma, A Rare Benign Tumor.

INTRODUCTION: Desmoplastic fibroma (DF) is a very rare, benign but aggressive bone tumor. It makes up <0.1% of all bone tumors and is often mistaken for other conditions, such as bone cysts or giant cell tumors. DF can occur in different bones, but it is uncommon in the proximal femur. Because it looks like other bone diseases on X-rays, diagnosing it correctly can be difficult. Proper treatment is needed to prevent it from coming back. However, due to its rarity, treatment recommendations are largely based on individual case reports, limiting the ability to establish standardized protocols.

The impact of demineralized bone matrix characteristics on pseudarthrosis and surgical outcomes after posterolateral lumbar decompression and fusion.

OBJECTIVES: The objectives of our study were to compare the fusion rates and surgical outcomes of lumbar fusion surgery based on the (1) type of demineralized bone matrix (DBM) carrier allograft, (2) the presence/absence of a carrier, and (3) the presence of bone fibers in DBM. METHODS: Patients >18 years of age who underwent single-level posterolateral decompression and fusion (PLDF) between L3 and L5 between 2014 and 2021 were retrospectively identified. We assessed bone grafts based on carrier type (no carrier, sodium hyaluronate carrier, and glycerol carrier) and the presence of bone fibe

Deltoid Osteomuscular Transfer Suppresses Humeral Head Translation Caused by Massive Irreparable Rotator Cuff Tears: A Cadaveric Biomechanical Study.

BACKGROUND: The optimal treatment for massive irreparable rotator cuff tears (MRCTs) remains unclear. The deltoid flap transfer has been described in the literature as an option for MRCT; however, it has a high retear rate and therefore its effectiveness is limited. PURPOSE: To describe a modification of the deltoid flap transfer, termed the deltoid osteomuscular transfer (DOT), and assess its biomechanical effectiveness in the management of irreparable MRCT in a cadaveric model. STUDY DESIGN: Controlled laboratory study. METHODS: Eight fresh-frozen cadaveric shoulders were tested. Segmenta

A rat model for microimplant-assisted rapid palatal expansion.

BACKGROUND: Microimplant-assisted rapid palatal expansion (MARPE) has been widely used to correct maxillary transverse deficiency. To gain valuable insights into the biological processes and mechanisms involved in palatal suture expansion and develop approaches to reduce relapse rates, researchers have turned to animal models. However, no small-animal models are currently available for MARPE. This study aimed to establish a MARPE rat model and investigate the bone-remodeling mechanisms underlying this biological process. METHODS: Three group sets were analyzed: 1) fifteen Sprague-Dawley rats

Anterior Decompression via a Single Posterior Approach Using the Ultrasonic Bone Scalpel for the Treatment of the Thoracic Segmental Ossification of Posterior Longitudinal Ligament: A Report of Three Cases.

INTRODUCTION: Thoracic myelopathy due to ossification of the posterior longitudinal ligament (T-OPLL) is an indication for surgical treatment because the symptom is usually progressive. The surgery for T-OPLL is technically challenging for several reasons. Various operational procedures were developed for dealing with T-OPLL. The anterior decompression through a single posterior approach is a procedure to achieve the complete decompression via the direct resection of the ossified lesion, especially for the beak-type OPLL. Previous reports showed better postoperative outcomes using this method

Unstable nonunions of the distal part of the humerus.

BACKGROUND: Some nonunions of the distal part of the humerus are so unstable that the hand and the forelimb cannot be supported against gravity. The purpose of the present retrospective study was to analyze the results of open reduction and internal fixation, joint contracture release, and autogenous bone-grafting in the treatment of these unstable nonunions of the distal part of the humerus. METHODS: Fifteen patients (average age, sixty years) with an unstable nonunion of the distal part of the humerus were treated with excision of fibrous and synovial tissues, opening of sclerotic fracture

Radiographic marginal bone level evaluation around two different tissue-level implant systems: a one-year prospective study.

BACKGROUND: Marginal bone loss, is a frequently reported variable in the evaluation of dental implants. The primary objective of this study was to evaluate radiographic marginal bone level changes around the two different tissue-level implant systems placed adjacently in the posterior maxilla or mandible. The influence of implant macro-geometry and vertical soft tissue thickness on marginal bone loss were also evaluated. METHODS: Seven patients were included in the study and a total of 18 implants were analyzed. Each patient received two different implants placed adjacently in the maxilla or

Modified Kouvalchouk technique for recurrent posterior instability of the shoulder: A retrospective multicenter review of 31 cases with an average follow-up of 5 years.

BACKGROUND: Surgical treatment of recurrent posterior shoulder instability gives good results in cases of isolated posterior labral lesion and relies on arthroscopic capsulo-labral repair. In cases of associated posterior glenoid bony lesions or for revision surgery, iliac crest bone block procedures seem to yield good short-term results, but there might be a concern for long-term results with resorption of the bone block, recurrence, and late arthritis. The Kouvalchouk technique, described in 1993, is using a bone block harvested from the posterior acromion and pedicled to a band of posterior

New Technique for Mandibular Symphyseal Distraction by a Double-Level Anchorage and Fixation System: Advantages and Results.

INTRODUCTION: A surgical technique to widen the mandible is the mandibular midline distraction: the most common indications for mandibular midline distraction are severe mandibular anterior crowding, severe mandibular transverse deficiency, uni- or bilateral crossbite, impacted anterior teeth with inadequate space, and tipped teeth. Commonly used distraction devices can be divided into 2 systems: bone-borne distraction system appliance, dental-borne distraction systems. Each system has peculiar advantages, disadvantages, and different indications. To combine advantages of both systems we devel

Measurement of the principal compressive trabeculae in the midcoronal plane of proximal femur and its significance in lag screw fixation.

BACKGROUND: With the development of osteoporosis, the principal compressive trabeculae (PCT) of proximal femur is preferentially retained and regarded as the main loadbearing structure of the femoral head. However, the significance of PCT in lag screw fixation has not been clarified. MATERIALS AND METHODS: The CT data of intact proximal femurs from 93 elderly patients were reconstructed with a personalized threshold segmentation method. The midcoronal plane of proximal femur was established, in which the PCT contour was extracted, and the analytical screw-tracts were created. The relative CT

Helical Blade Versus Lag Screw Fixation in the Cephalomedullary Nailing of Geriatric Hip Fractures.

BACKGROUND: While some prior research has shown helical blades to have higher risks of fixation failure and cut-out than lag screws in the cephalomedullary nailing of intertrochanteric femoral fractures, other studies have not demonstrated any such differences. The purpose of this study was to compare the performance of helical blade and lag screw fixation among older patients with a hip fracture treated with cephalomedullary nailing and to determine whether the relative performance of these 2 fixation methods varies on the basis of patient characteristics. METHODS: This retrospective cohort

Risk factors for screw breakage in Lisfranc joint injuries: A multicenter retrospective study.

BACKGROUND: Lisfranc joint injuries are rare but significantly impact foot biomechanics. Open reduction and internal fixation (ORIF) using screws is common, but screw breakage is a frequent issue. While previous studies have reported breakage rates, few have investigated the association between screw type and breakage risk. PURPOSE: This study aims to identify risk factors for screw breakage, analyze the timing and patterns of failures, and evaluate their impact on clinical outcomes. STUDY DESIGN: Multicenter retrospective cohort study. METHODS: This multicenter retrospective study analyzed

Angular position of proximal screw fixation during minimally invasive hallux valgus surgery in the sagittal plane.

Minimally invasive surgery for hallux valgus correction has evolved from early techniques, which used minimal or no hardware, to the current generation of more advanced, fixated osteotomies. These modern techniques often incorporate various forms of fixation, with recommendations on screw placement to optimize outcomes. Specifically, it is recommended that the proximal screw in the transverse plane should purchase two cortices of the first metatarsal to ensure maximum stability. While the ideal position of the screw in the anteroposterior view has been well documented in the literature, there

Extensor Mechanism Damage With Proximal Phalanx Antegrade Intramedullary Screws: A Cadaveric Study.

BACKGROUND: Antegrade intramedullary screw instrumentation for treatment of proximal phalangeal fractures has become increasingly common. To date, there has been little investigation describing and quantifying extensor mechanism damage incurred with this treatment option. METHODS: Thirty-two fresh cadaveric proximal phalanges (thumbs excluded) underwent percutaneous antegrade screw insertion. Radial-based and ulnar-based intramedullary screws were inserted per phalange, and the sagittal band and/or extensor tendon defect were evaluated. The mean sagittal band violation was measured and calcul
