Fatal cerebral toxoplasmosis in a patient with multiple sclerosis treated with natalizumab.

Natalizumab is a highly effective therapy but may impair central nervous system (CNS) immune surveillance, leading to rare opportunistic infections. We report a fatal case of cerebral toxoplasmosis under natalizumab. A 61-year-old man with stable multiple sclerosis (MS) developed acute hemiparesis on natalizumab therapy. Magnetic resonance imaging (MRI) showed multiple ring-enhancing lesions. Brain biopsy and polymerase chain reaction (PCR) confirmed reactivated Toxoplasma gondii. A low-risk IgG κ monoclonal gammopathy of undetermined significance (MGUS) was identified. Despite antiparasitic t

Extracellular vesicles engineered to directly target encephalomyocarditis virus ameliorates multi-organ viremia in a lethal infection model.

The outbreak and prevalence of encephalomyocarditis virus (EMCV) causes significant global mortality and morbidity to the pig industry. Though the current and most effective approach to control EMCV outbreak are done through inactivated vaccines, we have yet to see an effective antiviral agent that directly targets EMCV. Here, we present a molecular therapy consisting of extracellular vesicles (EVs) decorated with EMCV-specific single-chain variable fragment (scFv), engineered on the external loop of the EVS transmembrane domain CD63. These EMCV-scFv enriched EVs directly neutralizes infectiou

Rapid and specific detection of Mycoplasma pneumoniae using Pyrococcus furiosus argonaute combined with recombinase polymerase amplification.

Mycoplasma pneumonia (MP) is a common pathogen of human respiratory infections and one of the leading causes of community-acquired pneumonia. Early and rapid diagnosis of its infection is crucial for clinical treatment decisions. In this study, we innovatively combined recombinase polymerase amplification with Pyrococcus furiosus Argonaute protein (PfAgo) to establish a novel molecular diagnostic method for MP. This assay demonstrated high specificity with no cross-reactivity with other common respiratory pathogens. The limit of detection was 2 × 104 copies μl-1. Furthermore, evaluation using

Corynebacterium striatum-Induced Meningitis in a Non-immunocompromised Patient.

We present the case of a 46-year-old immunocompetent male diagnosed with meningitis caused by Corynebacterium striatum, a rare pathogen typically found in immunocompromised individuals. The diagnosis was confirmed by cerebrospinal fluid (CSF) culture, which grew C. striatum, and was supported by the exclusion of other common pathogens causing meningitis, including viral etiology, using polymerase chain reaction (PCR) testing. This case highlights the potential of C. striatum to cause central nervous system (CNS) infections in otherwise healthy individuals. The patient had a history of prosthet

Imported human babesiosis in the Republic of Korea, 2019: two case reports.

Human babesiosis is a tick-borne disease induced by the genus Babesia and has been significantly reported in the Republic of Korea. This report shows the cases of 2 patients with human babesiosis who traveled to the USA in 2019. The 2 patients experienced fever and had travel histories to babesiosis-endemic regions. The diagnoses of both cases were verified by the identification of Babesia-infected red blood cells on blood smears. One patient was found to be infected with Babesia microti using polymerase chain reaction (PCR) for 18S rRNA, which discovered the phylogenetic link to the B. microt

Deletion of the first glycosylation site promotes Lassa virus glycoprotein-mediated membrane fusion.

The Lassa virus (LASV) is endemic in West Africa and causes severe hemorrhagic Lassa fever in humans. The glycoprotein complex (GPC) of LASV is highly glycosylation-modified, with 11 N-glycosylation sites. All 11 N-linked glycan chains play critical roles in GPC cleavage, folding, receptor binding, membrane fusion, and immune evasion. In this study, we focused on the first glycosylation site because its deletion mutant (N79Q) results in an unexpected enhanced membrane fusion, whereas it exerts little effect on GPC expression, cleavage, and receptor binding. Meanwhile, the pseudotype virus bea

Emerging role of γδ T cells in protozoan infection and their potential clinical application.

γδ T cells are thymus derived heterogeneous and unconventional T- lymphocyte expressing TCR γ (V γ9) and TCRδ (Vδ2) chain and play an important role in connecting innate and adaptive armaments of immune response. These cells can recognize wide ranges of antigens even without involvement of major histocompatibility complex and exert their biological functions by cytotoxicity or activating various types of immune cells. In recent past, γδ T cells have emerged as an important player during protozoa infection and rapidly expand after exposure with them. They have also been widely studied in vaccin

A Case Report of Renal Tuberculosis With Associated Unusual Pulmonary Findings.

Tuberculosis (TB) is a wide-reaching chronic inflammatory disease predominantly infecting the lungs. When it infects other sites, it is termed extrapulmonary TB. Among the extrapulmonary forms, genitourinary TB (GU-TB) accounts for 30%-40% of cases. We report a case of pulmonary-renal TB with unusual pulmonary findings. Subsequent investigation of a frank haematuria case revealed positive Mycobacterium TB culture and acid-fast bacillus polymerase chain reaction (AFB-PCR) samples of urine, with abdominal imaging findings suggestive of GU-TB. Pulmonary involvement was evident on chest imaging as b

Anosmia and the Need for COVID-19 Screening during the Pandemic.

In this commentary, we briefly summarize the available data from Iran and other countries on the sudden increase in anosmia, hyposmia, and hypogeusia that has coincided with the COVID-19 pandemic. Alarmingly, a high proportion of patients with severe COVID-19 had isolated anosmia as the sole initial presenting symptom, which is likely due to the direct neuropathic effect of the virus rather than being secondary to nasal congestion and obstruction. Since isolated anosmia is not yet considered a prerequisite for screening for COVID-19, we wish to raise awareness on the association of anosmia wit

Urinary capillariosis in a cat from Rio de Janeiro, Brazil - Clinical, morphological and phylogenetic characterization.

Capillaria spp. infections of the urinary tract of domestic carnivores are uncommon worldwide. Infections are rarely diagnosed and are typically asymptomatic. This study aimed to evaluate a case of capillariosis in a cat from the state of Rio de Janeiro, Brazil. A seven-year-old female cat with apathy and reduced appetite was presented. Urine analysis revealed C. plica eggs in urine sediment, and cystitis was evidenced by the presence of bacteria, pyuria, proteinuria and hematuria. The subject was treated with 50 mg/kg fenbendazole for five days. Urine samples were frozen for molecular analysi

Recurring and emerging questions related to management of HIV-related opportunistic infections.

The incidence of HIV-related opportunistic infections (OIs) has dramatically declined with the ability to achieve viral suppression and immune reconstitution with potent antiretroviral therapy. However, a large number of patients remain at risk for OIs because they are diagnosed at late stages of HIV disease, fail to stay in treatment, or fail to maintain viral suppression. Clinicians should remain vigilant for OIs and for changes in recommended management strategies. Issues that often arise in this regard include how to interpret polymerase chain reaction diagnostic results in individuals wit

Emergence of human West Nile Virus infection among pediatric population in Madhya Pradesh, India.

Human infections caused by West Nile virus (WNV) mostly remain subclinical and self-limited. However, nearly 20% infected people suffer from febrile illness and very few of them (<1%) may get neuroinvasive illness. Mortality has been reported among children. India somehow has reported very less number of WNV cases in the past. We collected cerebrospinal fluid (CSF) samples from 75 pediatric age group patients clinically suffering from acute encephalitis syndrome. Three of these samples were positive by reverse transcriptase polymerase chain reaction using pan flavivirus primers. On sequencing

Mycotic pseudoaneurysm of a pulmonary artery branch caused by Cladosporium.

We report the case of a 53-year-old male with a history of acute myelogenous leukemia, who suffered the rupturing of a right-sided pulmonary artery pseudoaneurysm combined with pneumonia. He underwent a right-sided lower lobectomy. The resected lung tissue demonstrated a mycotic pseudoaneurysm of a pulmonary artery branch together with a filamentous fungal infection. Pseudoaneurysms are caused by the breaching of all layers of a blood vessel wall. The extravasated blood is trapped by the surrounding extravascular tissue or clots. Cladosporium was detected during a polymerase chain reaction-bas

Visceral pentastomiasis caused by Armillifer armillatus in a captive striped hyena (Hyaena hyaena) in Chiang Mai Night Safari, Thailand.

Visceral pentastomiasis (porocephalosis) caused by Armillifer armillatus larvae was incidentally diagnosed in a female striped hyena (Hyaena hyaena) of unknown age which died unexpectedly in 2013. The hyena had been imported from Tanzania 8years earlier and have been since then in a zoo in Chiang Mai, northern Thailand. Pathological examination revealed visceral nymph migrans of pentastomes throughout the intestine, liver, diaphragm, omentum and mesentery, spleen, kidneys, and urinary bladder. Polymerase chain reaction and sequencing that targeted the pentastomid-specific 18S rRNA gene determi

Lymphogranuloma venereum serovar L2b in Portugal.

Chlamydia trachomatis, serovar L2, is the causative agent of lymphogranuloma venereum (LGV), which during recent years has been responsible for various outbreaks reported among men who have sex with men (MSM) in Western Europe, America, Canada and Australia. Samples from nine patients with chronic proctitis, seen at a local hospital were sent to us for identification of C. trachomatis serovar L2. The presence of C. trachomatis serovar L DNA was identified by realtime polymerase chain reaction (PCR) in two patients. They both had high positive C. trachomatis antibody titres (>/=10,000) and were

2010 ACVIM small animal consensus statement on leptospirosis: diagnosis, epidemiology, treatment, and prevention.

This report offers a consensus opinion on the diagnosis, epidemiology, treatment, and prevention of leptospirosis in dogs, an important zoonosis. Clinical signs of leptospirosis in dogs relate to development of renal disease, hepatic disease, uveitis, and pulmonary hemorrhage. Disease may follow periods of high rainfall, and can occur in dogs roaming in proximity to water sources, farm animals, or wildlife, or dogs residing in suburban environments. Diagnosis is based on acute and convalescent phase antibody titers by the microscopic agglutination test (MAT), with or without use of polymerase
