Monocyte count is associated with the severity of human adenovirus pneumonia in hospitalized children aged less than 6 years.

BACKGROUND: Human Adenovirus (HAdV) pneumonia is common in young children and infants. Overall, 7-8% of all viral respiratory illnesses among children for less than 5 years are induced by HAdVs. Unfortunately little is known about the role of monocyte count in the disease severity. METHODS: Data were gathered from 595 children (age < 6 years) who were diagnosed with HAdV infection at the 1st People's Hospital (Changde City, China) between January 2019 and December 2019. There were 181 cases of severe adenovirus pneumonia. RESULTS: The correlation between the patients' monocyte count and the

Factors Associated With Hospitalization in Children and Adolescents With SARS-CoV-2 Infection.

In this retrospective study of 319 children with laboratory-confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, we assessed whether age, asthma, obesity, diabetes, and socioeconomic status were associated with hospitalization for coronavirus disease 2019 (COVID-19). Demographic and clinical characteristics were assessed using univariate statistics, excluding incidental or unrelated positives. There was a bimodal distribution of age among hospitalized children. Obesity (P < .001) and a past medical history of diabetes (P = .001) were significantly more prevalent in

Nonenteric Adenoviruses Associated with Gastroenteritis in Hospitalized Children.

The object of this study was to investigate the frequency of human adenovirus (HAdV) infections in hospitalized pediatric patients. Stool samples were collected during a 1-year period (February 2018 to January 2019). HAdV was detected by a broad-range PCR and genotyped by sequencing and phylogenetic analysis. Demographic characteristics and detailed clinical information were analyzed for each patient. HAdV was detected in 7.1% of stool samples (34/476). Among these patients, 23.5% were coinfected with other enteric viral or bacterial pathogens. The majority (85.2%) of HAdV positives were detec

Pediatric Emergencies: Imaging of Pediatric Head Trauma.

Pediatric head trauma is an important cause of morbidity and mortality in children and may be seen in the setting of accidental or abusive injuries. Although many of the patterns of head injury are similar to adults, the imaging manifestations of head injury in children are more complex due to the developing brain and calvarium. Additionally, there are unique considerations for mechanisms of injury in children, to include abusive head trauma and birth-related injuries. The primary role of the radiologist is to identify and characterize the type and severity of head injury to help guide appropr

Challenges of autism in the inpatient setting.

Julie is a 4-year-old girl with autism spectrum disorder (ASD) who presented to the emergency room with severe unilateral hip pain and limping. Initial evaluation indicated increased inflammatory markers and blasts on a blood smear. A bone marrow biopsy revealed acute lymphoblastic leukemia (ALL), and Julie was admitted for induction chemotherapy. Julie was diagnosed with ASD 1 year before this presentation. Her parents, who had immigrated to the United States from China before her birth, indicated that it took them some time to accept the diagnosis of ASD but they were feeling more confident

Assessment and comparative analysis of a rapid diagnostic test (Tubex®) for the diagnosis of typhoid fever among hospitalized children in rural Tanzania.

BACKGROUND: Typhoid fever remains a significant health problem in many developing countries. A rapid test with a performance comparable to that of blood culture would be highly useful. A rapid diagnostic test for typhoid fever, Tubex®, is commercially available that uses particle separation to detect immunoglobulin M directed towards Salmonella Typhi O9 lipopolysaccharide in sera. METHODS: We assessed the sensitivity and specificity of the Tubex test among Tanzanian children hospitalized with febrile illness using blood culture as gold standard. Evaluation was done considering blood culture c

Mycoplasma pneumoniae pneumonia in hospitalized children diagnosed at acute stage by paired sera.

BACKGROUND: Mycoplasma pneumoniae (M. pneumoniae) is a frequent cause of respiratory tract infections. However, there is deficient knowledge about the clinical manifestations of M. pneumoniae infection. We described the clinical and laboratory findings of M. pneumoniae pneumonia in hospitalized children who were all diagnosed by a ≥ fourfold increase in antibody titer. METHODS: M. pneumoniae antibodies were routinely detected in children admitted with acute respiratory infection during a one-year period. The medical history was re-collected from children whose M. pneumoniae antibody titer inc

Effects of sexual and physical abuse: a comparison of adolescent inpatients.

The present study was designed to collect descriptive data on a sample of hospitalized adolescents who were victims of sexual and/or physical abuse and to compare these groups to patients without a history of abuse. A questionnaire was administered to 51 inpatient teenagers at Fort Logan Mental Health Center, a state psychiatric hospital in Denver, Colorado. Information was obtained about family background, type, extent and duration of abuse, drug and alcohol abuse and self-reported mental health and general adjustment problems. In addition, clinical staff completed the Revised Behavior Proble

Childhood hospitalization and chronic intractable pain in adults: a controlled retrospective study.

Three groups of patients have been studied in order to elucidate the relationship between childhood hospitalization and chronic intractable pain in adults. The groups were: patients referred to a pain clinic, psychiatric patients with a depressive illness, and patients attending a rheumatology clinic. The findings suggest that early hospitalization is related to the genesis of both depressive illness and intractable pain: in the former occurring in the preschool years and in the latter, during school age. The significance of these relationships is discussed.

Disease Severity of Respiratory Syncytial Virus Infection in Hospitalized Children.

Background: Respiratory syncytial virus (RSV) is a leading cause of hospitalization for acute respiratory tract infection (ARTI) in young children. Respiratory viral coinfections are frequently identified in RSV-related ARTIs, yet their impact on disease severity remains controversial and may vary according to the co-pathogen involved. In the context of evolving RSV prevention strategies, a clearer understanding of RSV coinfection phenotypes is needed. Methods: We conducted a multicenter retrospective cohort study of children aged ≤ 5 years hospitalized for ARTI at two Italian tertiary-care pe

Intranasal fentanyl and impact of an emergency department pain protocol for sickle cell disease on hospitalization rates.

Sickle cell disease (SCD) is an inherited red blood cell disorder with significant complications, including vaso-occlusive crisis (VOC), the most common cause of emergency department (ED) visits and hospitalizations. Pain in children with SCD is often underestimated. We evaluated the impact of a standardized institutional nurse-initiated ED SCD pain protocol, using intranasal (IN) fentanyl (INF) as the initial agent, on hospital admission rates. We conducted a preintervention/postintervention study of patients with SCD (aged 0-21 years) presenting to a quaternary pediatric ED for uncomplicated

An Analysis of Trauma Patient Deaths in the Emergency Department - an Opportunity for Performance Improvement Initiatives.

Introduction: Trauma is a leading cause of presentation to the emergency department (ED). Improving outcomes requires identification of areas for potential improvement, as well as identifying factors of patients with non-survivable injuries that will unnecessarily consume resources. We sought to analyze the presentations of trauma patients who expired while still in the ED. Methods: We analyzed data from patients in the Trauma Quality Improvement Program (TQIP) registry whose death time was less than their ED length of stay to a maximum of 3 hours. We used descriptive statistics. Results: From

Patterns of Trauma Presentation in Ilorin, Nigeria: A 15-Year Review.

BACKGROUND: Injuries make up a significant portion of the emergency surgical patient load in most hospitals, representing a major public health threat The burden of injury as a public health issue is especially pronounced in low- and middle-income countries, where injuries are responsible for up to 90% of mortality. Identifying common mechanisms of trauma and patterns of presentation can aid in identifying at-risk populations and in the development of targeted preventive protocols. METHODS: From February 2000 to January 2015, a prospective observational study was conducted to examine the patt

Intermittent Asthma and Risk of Severe Exacerbation in Children.

BACKGROUND: Because of risk of severe asthma exacerbations, current Global Initiative for Asthma recommendations advise against use of short-acting beta-agonists (SABAs) alone as the first step in treating mild asthma. It is unclear if everyone with mild asthma carries equal risk for severe asthma exacerbations. RESEARCH QUESTION: Is there a subgroup of patients with mild asthma with very low risk of severe asthma exacerbations? STUDY DESIGN AND METHODS: This study cohort used administrative claims data for patients ages 2 to 18 years with intermittent asthma enrolled in Ohio Medicaid Manage
