BACKGROUND: The global annual estimate for cryptococcal disease-related deaths exceeds 180,000, with three fourth occurring in sub-Saharan Africa. The World Health Organization (WHO) recommends cryptococcal antigen (CrAg) screening in all HIV patients with CD4 count < 100/μl. As there is no previous published study on the burden and impact of cryptococcal disease in Sierra Leone, research is needed to inform public health policies. We aimed to establish the seroprevalence and mortality of cryptococcal disease in adults with advanced HIV attending an urban tertiary hospital in Sierra Leone.

Disease caused by Cryptococcus gattii typically manifests as meningoencephalitis or pulmonary nodules. Endobronchial lesions are rare, and most cases are caused by Cryptococcus neoformans. We describe here a case of endobronchial disease in a child caused by C gattii. The disease spectrum in this patient was notable for the discovery of anti-granulocyte macrophage colony-stimulating factor autoantibodies.

A total of 515 yeast strains were isolated from the nasal smears of Queensland koalas and their breeding environments in Japanese zoological parks between 2005 and 2012. Pulmonary complications occur in over half of patients with AIDS at some point in their illness. A chest X-ray is a valuable first-line investigation. The plain film and computed tomographic features of these complications are described and several examples illustrated. The role of computed tomography, including high resolution scanning techniques in further investigation and management, is discussed.

An increasing number of patients are presenting with central nervous system complications of human immunodeficiency virus infection. New imaging technologies such as magnetic resonance imaging, magnetic resonance proton spectroscopy, single-photon emission computed tomography, and positron emission tomography are playing an ever-increasing role in the diagnosis of these complications. As therapeutic modes improve, imaging may assume a growing role in monitoring the responses to therapy among these patients.

BRL 49594A (BRL) is a water soluble analogue of amphotericin B which has been developed as a polyene with efficacy similar to amphotericin B but with much reduced toxicity.
