Diagnostic Pitfalls of CT in Malignant Superior Cerebellar Artery Infarction: Implications for Treatment Decisions and Future Management Strategies.

Background/Objectives: Superior cerebellar artery (SCA) infarction is a rare but clinically significant subtype of posterior circulation stroke. Extensive swelling in the SCA territory may cause downward brainstem compression and appear as brainstem hypodensity on computed tomography, potentially leading to premature treatment withdrawal. Methods: We report the case of a 50-year-old woman with acute SCA-territory infarction (NIHSS = 7) presenting with vertigo, dysphagia, dysarthria, and diplopia. Initial computed tomography suggested extensive brainstem infarction, prompting withdrawal of trea

Conservative Management of a Large Periapical Lesion Using Decompression Technique: A Case Report.

The management of extensive periapical lesions poses a significant challenge in endodontic therapy due to their potential cystic nature, anatomical proximity to critical structures, risk of cortical perforation, and complex infection dynamics. This report presents a successful case of a large through-and-through periapical lesion managed conservatively via decompression. Initial periapical radiographs revealed poor-quality root canal therapy and a large radiolucent lesion associated with a maxillary lateral incisor. Cone-beam computed tomography (CBCT) imaging confirmed a through-and-through l

Multicenter retrospective cohort of EUS-guided anterograde pancreatic duct access.

Background and study aims  Pancreatic duct (PD) cannulation may be difficult during conventional endoscopic retrograde cholangiopancreatography (ERCP) due to underlying pathology, anatomical variants or surgically altered anatomy. Pancreatic access in these cases previously necessitated percutaneous or surgical approaches. Endoscopic ultrasound (EUS) allows for an alternative and can be combined with ERCP for rendezvous during the same procedure, or for other salvage options. Patients and methods  Patients with attempted EUS access of the PD from tertiary referral centers between 2009 and 2022

Conservative Approach in the Management of Large Periapical Cyst-Like Lesions. A Report of Two Cases.

Background and Objectives: Periapical cystic lesions are a pathology frequently addressed to endodontic specialists. Although their therapy is still not standardized, the treatment should be as conservative as possible and by endodontic means, as they are lesions of endodontic origin. The present case report describes two cases of upper central incisors with large cyst-like periapical lesions, and their one-year follow up. Materials and Methods: Endodontic orthograde treatment was performed under copious irrigation with sodium hypochlorite, in association with calcium hydroxide as an intra-can

Headache in Patients with Cervical Spondylotic Myelopathy.

PURPOSE: The anatomical mechanisms of cervicogenic headache caused by upper cervical lesions have been reported. However, the pathomechanisms of headache caused by lower cervical spine disorders remain unknown. The purpose of the current study was to clarify the prevalence and pathogenesis of headaches in patients with cervical spondylotic myelopathy (CSM). METHODS: In this retrospective study, a questionnaire regarding preoperative and postoperative symptoms was sent to 147 patients with CSM who were surgically treated in our hospital during the previous 10 years. All of the surgical procedu

A case of idiopathic hypertrophic pachymeningitis presenting with chronic headache and multiple cranial nerve palsies: A case report.

RATIONALE: Idiopathic hypertrophic pachymeningitis (IHP) is a rare condition, characterized by a chronic fibrosing inflammatory process usually involving either the intracranial or spinal dura mater, but rarely both. Here, we report a rare case of IHP affecting both the intracranial and spinal dura mater. We also discussed the diagnosis, management, and outcome of IHP. PATIENT CONCERNS: We reviewed the case of a 60-year-old woman presenting with chronic headache, multiple cranial nerve palsies and gait disturbance. Magnetic resonance imaging (MRI) of her head revealed thickened and contrast-e

A New Less Invasive Technique for Multiple-Level Spontaneous Spinal Epidural Hematomas: Wash-and-Go Technique.

Aim Spinal epidural hematomas are rare entity in neurosurgery practice. Most of them are spontaneous due to anticoagulant therapy and called spontaneous spinal epidural hematomas (SSEHs). Laminectomy or hemilaminectomy for affected levels is still the first choice in the operative treatment of an SSEH. We describe a new less invasive surgical technique, performing single-level laminectomy and washing with 0.9% sodium chloride through a thin soft catheter for a 12-level thoracic-cervical SSEH in a patient under anticoagulant therapy. Patient and Operative Technique A 55-year-old woman was broug

Idiopathic Spinal Epidural Lipomatosis in the Lumbar Spine.

Overgrowth of epidural fat, known as spinal epidural lipomatosis (SEL), can cause symptomatic compression of the spinal cord, conus medullaris, or cauda equina. Suggested predisposing factors such as obesity, steroid use, and diabetes mellitus have been based on a few reported cases, many of which were not surgically confirmed. There is a paucity of epidemiological data in surgically confirmed cases for this disorder. The purpose of this independently reviewed, retrospective, matched cohort analysis was to compare the demographics and incidence of comorbidities of patients who underwent lumbar

Early decompressive craniectomy for traumatic brain injury in resource poor centres:a tertiary institution experience.

UNLABELLED: Decompressive craniectomy (DC) is the surgical management of increasing intracranial pressure (ICP) following a severe traumatic brain injury (TBI) as well as severe cerebrovascular insult. Established protocols for the use of DC include monitoring ICP and going through non-surgical methods to reduce ICP before DC which is the last resort. ICP monitors and facilities to render patients hypothermic and for inducing barbiturate coma are not readily available in resource depleted facilities, hence the need for a timely DC. DC when timely done saves lives and improves chances of surviv

Surgical treatment of patients with unilateral cerebellar infarcts: clinical outcome and prognostic factors.

BACKGROUND: There are limited data on the long-term outcome and on factors influencing the prognosis in patients with cerebellar infarcts treated with surgical decompression. METHODS: Thirty-two patients (age 64.3 ± 9.9 years) with expansive unilateral cerebellar infarcts were retrospectively evaluated. All patients were treated with ventriculostomy, suboccipital decompressive craniectomy and removal of the necrotic tissue. The Glasgow Coma Scale (GCS) and the Reaction Level Scale (RLS) scores evaluated the level of consciousness during hospitalization, while the modified Rankin Scale (mRS) w

Anterior cervical discectomy with or without fusion with ray titanium cage: a prospective randomized clinical study.

STUDY DESIGN: A prospective randomized clinical study. OBJECTIVE: To compare 2 surgical methods in the treatment of cervical radiculopathy caused by hard or soft disc herniation; namely, simple discectomy versus discectomy with an additional interbody fusion with a Ray titanium cage. SUMMARY OF BACKGROUND DATA: Although an interbody fusion after anterior decompressive surgery for hard or soft disc herniation is widely accepted, there is no scientific evidence that convincingly demonstrates that insertion of graft material for interbody fusion is necessary after discectomy and decompression o

[Current management of hand leprosy].

Leprosy, a chronic infectious disease caused by mycobacterium leprae affects an estimated 700,000 persons each year. Clinically, leprosy can be categorized as paucibacillary or multibacillary disease. Leprosy is important largely because of the deformities, disabilities and handicap it causes in a proportion of those affected by the disease. There are surgical procedures and techniques to correct or limit the deterioration of these conditions. In the past these surgical procedures were only performed in special institutions for treating leprosy and their complications. However, with the widesp

Use of a multiple logistic regression model to determine prognosis of dairy cows with right displacement of the abomasum or abomasal volvulus.

Data at admission and at surgery were collected on 458 cows with right displacement of the abomasum or abomasal volvulus, to derive multiple logistic regression models for predicting postsurgical outcome (productive, salvaged, or terminal). The derived models contained few and easily obtained variables. The weight associated with each variable was determined objectively. Three admission variables (heart rate, base excess, and plasma chloride concentration), and 5 surgical variables (heart rate, base excess, diagnosis, method of decompression used, and appearance of abomasal serosa) were used i

Stent or surgery for malignant low bile duct obstruction?

The development of non-surgical techniques for the relief of malignant low bileduct obstruction has cast doubt on the best way of relieving jaundice, particularly in patients fit for surgery whose life expectancy is more than a few weeks. We did a randomised prospective controlled trial comparing endoscopic stent insertion and surgical biliary bypass in patients with malignant low bileduct obstruction. 204 patients were randomised (surgery 103, stent 101); 3 subsequently proved to have benign disease and were excluded, leaving 101 surgical and 100 stented patients for assessment.
