Simultaneous closure of a perilymphatic fistula and placement of cochlear implant in a case of complex inner ear malformation.

KEY CLINICAL MESSAGE: In young infants, under the age of one-year, cochlear malformation with profound hearing loss complicated by a perilymphatic fistula (PLF), presents a serious clinical challenge, warranting immediate audiological and surgical intervention. Timely PLF detection and closure, along with an early CI can significantly improve the prognosis of such patients and helps them in achieving their maximum hearing and developmental potential, in the long term. ABSTRACT: Inner ear malformation (IEM) with incomplete partition and cystic cochlea is mostly accompanied by profound hearing …

Variability in Inner Ear Morphology Among a Family With Pendred Syndrome Due to a SLC26A4 Gene Variant.

OBJECTIVES: Pendred syndrome, an autosomal recessive disorder, is often associated with pathogenic variants of the SLC26A4 gene that encodes the pendrin protein. Given its autosomal recessive inheritance, tracing the family history and screening siblings become crucial once a diagnosis of Pendred syndrome is confirmed. This case report aims to underscore the variability in inner ear morphology within a family diagnosed with Pendred syndrome, all carrying the same SLC26A4 gene mutation. METHODS: A chart review and a review of the literature. RESULTS: We present a family of 4, all of whom poss…

Speech intelligibility prediction based on a physiological model of the human ear and a hierarchical spiking neural network.

A speech intelligibility (SI) prediction model is proposed that includes an auditory preprocessing component based on the physiological anatomy and activity of the human ear, a hierarchical spiking neural network, and a decision back-end processing based on correlation analysis. The auditory preprocessing component effectively captures advanced physiological details of the auditory system, such as retrograde traveling waves, longitudinal coupling, and cochlear nonlinearity. The ability of the model to predict data from normal-hearing listeners under various additive noise conditions was consid…

Macro- and microscopic anatomy of the digestive tract in the red-eared slider (Emydidae: Trachemys scripta elegans).

The red-eared sliders (Emydidae: Trachemys scripta) is characterised by a high adaptability to a variety of environment and threatens the habitat of Japanese native species. The ability to digest a variety of diets may attribute to the high adaptive capacity of this species to various environments, however, the digestive morphology remains scarcely described in red-eared sliders. In this study, we investigated the macro- and microscopic anatomy of the esophagus, stomach, small intestine, and large intestine in red-eared sliders. All segments of the digestive tract had longitudinal mucosal fold…

Into the large ears: otitis externa associated with nematodes, mites, and bacteria in Asian elephants (Elephas maximus).

BACKGROUND: The Asian elephant (Elephas maximus), which is an endangered species, harbors several parasites. Among the ectoparasites that it harbors, ear mites of the genus Loxanoetus have the potential to cause external otitis, an inflammation that may also be associated with the presence of other microorganisms. We assessed the relationships between ear mites, nematodes, yeast, bacterial rods, and cocci sampled from the ears of captive Asian elephants in Thailand. In addition, we discuss the possibility that dust-bathing behavior may be triggered by ear mite infestation, and that this in tur…

The spontaneous mouse mutant low set ears (Lse) is caused by tandem duplication of Fgf3 and Fgf4.

The external ear develops from an organized convergence of ventrally migrating neural crest cells into the first and second branchial arches. Defects in external ear position are often symptomatic of complex syndromes such as Apert, Treacher-Collins, and Crouzon Syndrome. The low set ears (Lse) spontaneous mouse mutant is characterized by the dominant inheritance of a ventrally shifted external ear position and an abnormal external auditory meatus (EAM). We identified the causative mutation as a 148 Kb tandem duplication on Chromosome 7, which includes the entire coding sequences of Fgf3 and F…

Engineered triamcinolone acetonide loaded glycerosomes as a novel ear delivery system for the treatment of otitis media.

Ear-oriented therapeutics vehiculation strategies are requisites for effective treatment of various otic ailments including otitis media (OM). Conquering minimal permeability of the intrinsic barrier of middle ear; intact tympanic membrane (TM) is still a defiance. In this study, the fabrication of glycerosomes was explored to boost triamcinolone acetonide (TA) delivery to the middle ear via the otic application to improve treatment of OM. Opting a d-optimal design, TA glycerosomes were formulated and optimized using ethanol injection method. The optimized formula was assessed for morphology, …

Disorders of the eye, ear, skin, and nervous system in women with Turner syndrome -a nationwide cohort study.

The literature about eye, ear, nose, skin, and nervous system disorders in women with Turner syndrome is equivocal. Impaired vision and hearing in women with Turner syndrome have been described, and case reports of Turner syndrome girls suffering from epilepsy have been published, but no large population-based-studies have explored the occurrence of any of these disorders. We aimed to investigate the risk of admission with disorders related to the eye, ear, nose, skin, and nervous system, compared with background females, and the impact of hormone replacement therapy on these conditions. 1,156…

Reconstruction of constricted ears by combing bilateral cartilage flaps bridging with V-Y advancement flap.

BACKGROUND: Despite a number of surgical procedures for the reconstruction of moderate to severe constricted ears described in the literature, a most cost-effective method remains to be explored. It is still a challenge to maximize the full use of the ear cartilage and surrounding skin while achieving the best results. METHODS: From 2011 to 2016, seven constricted ear patients were enrolled in this study. Five of them were moderate (type IIB Tanzer classification) deformities, and two were severe (type III Tanzer classification). All constricted ear patients were treated with bilateral cartil…

Anthropometric assessment of microtia patients' normal ears and discussion on expander selection in auricular reconstruction surgery.

The tissue expansion technique is the most suitable procedure for Chinese patients with microtia. However, it is difficult to determine whether the expanded flap is sufficient, and there are no clear or objective guidelines for determining the volume of the expander for different sizes of auricles. One hundred patients with unilateral microtia who visited our department in 2021 were randomly selected for auricular data collection using 3D scanning. The auricle length, width, projection, perimeter, and surface area were measured. Eight different volumes of expanders underwent CT and the surface…

Parameter Identification From Normal and Pathological Middle Ears Using a Tailored Parameter Identification Algorithm.

Current clinical practice is often unable to identify the causes of conductive hearing loss in the middle ear with sufficient certainty without exploratory surgery. Besides the large uncertainties due to interindividual variances, only partially understood cause-effect principles are a major reason for the hesitant use of objective methods such as wideband tympanometry in diagnosis, despite their high sensitivity to pathological changes. For a better understanding of objective metrics of the middle ear, this study presents a model that can be used to reproduce characteristic changes in metrics…
