A call for inclusion of arthroplasty as essential surgery.

In 2015, the World Health Assembly and the Lancet Commission on Global Surgery affirmed the need for equitable access to essential surgical care. Additionally, the World Bank's Disease Control Priorities project identified 44 essential surgical procedures for health-care systems in low- and middle-income countries. These procedures address the global burden of disease, are cost-effective and are feasible to implement in resource-constrained settings. Notably, arthroplasty, specifically, hip and knee replacements, was excluded. A decade later, this omission warrants reconsideration. With improv

Dibutyl phthalate induces sarcopenia via TNFα/TNFR1-mediated proteolytic and pyroptotic axes: evidence from NHANES and experimental models.

Environmental exposure to plasticizer dibutyl phthalate (DBP) is increasingly implicated in skeletal muscle decline, yet the effects and underlying mechanisms remain elusive. This study investigates the impact of DBP on skeletal muscle using a cross-scale integration of epidemiological modeling, computational toxicology, and experimental validations. Mixture modeling of 3,514 NHANES adults (2011-2018) demonstrated that combined phthalate exposure negatively correlated with skeletal muscle mass not only in aged but also in young populations. DBP metabolite monobutyl phthalate (MBP) emerged as t

Determinants of Quality of Life and Mental Health in Kenyan Psoriasis Patients: A Cross-Sectional Analysis from the Kenyan Psoriasis Registry.

PURPOSE: Psoriasis is a chronic immune-mediated disease with global prevalence of 2-3% that is associated with significantly reduced quality of life (QoL) and worsened mental health. Despite this, there is a lack of research in African psoriasis populations, with no modern epidemiological studies conducted in Kenya to examine these factors. This study aims to identify demographic and clinical features associated with dermatology-related QoL and mental health among psoriasis patients enrolled in the newly established Kenyan Psoriasis Registry (KPR), based at Moi Teaching and Referral Hospital i

The Waist-to-Height Ratio and Incidence of High LDL Cholesterol in Japanese: A Retrospective Cohort Study.

AIM: Low-density lipoprotein cholesterol (LDL-C) is a major contributor to atherosclerotic cardiovascular disease (CVD). The waist-to-height ratio (WHtR) has been proposed as a practical marker of central obesity; however, evidence linking WHtR with incident high LDL-C levels in Japanese populations remains limited. This study investigated the association between WHtR and future LDL-C elevation in a community-based cohort in Japan. METHODS: This retrospective cohort study included 3,934 adults aged 30-74 years without dyslipidemia at baseline from the Iki Epidemiological Study of Atherosclero

Metabolic Syndrome and Pancreatic Cancer: Linking Insulin Resistance, Inflammation, and Carcinogenesis.

Metabolic syndrome (MetS) is a composite classification defined by heterogeneous combinations of cardiometabolic abnormalities. Several components have been associated with pancreatic cancer (PC), but studies often conflate long-term metabolic exposure, tumor-related prediagnostic metabolic change, and prognosis after diagnosis. This review critically evaluates these three settings. This narrative review searched PubMed, Scopus, and Web of Science through April 2026, prioritizing meta-analyses, prospective cohorts, Mendelian randomization studies, clinical studies, preclinical mechanistic evid

From Infection Control to Healthcare System Resilience: Lessons Learned from SARS-CoV-2 Research in Healthcare Workers.

The COVID-19 pandemic placed unprecedented pressure on healthcare systems and exposed healthcare workers (HCWs) to biological hazards, organizational pressures, and psychological strain. Evidence generated during the emergency shows that HCW protection cannot rely on isolated measures, but requires an integrated framework combining epidemiological surveillance, contact tracing, infection prevention and control, vaccination, occupational health, and workforce support. Contact tracing helped identify occupational exposures and clarify how duration, proximity, and inadequate use of personal prote

Connecting the Airways: Current Trends in United Airway Diseases.

The concept of united airway disease (UAD) highlights the bidirectional relationship between inflammatory disorders of the upper airways-such as allergic rhinitis and chronic rhinosinusitis with or without nasal polyps (CRSwNP/CRSsNP)-and lower airway diseases, most notably asthma. This paradigm is supported by epidemiological, embryological, and immunological evidence demonstrating that airway inflammation represents a single, interconnected process rather than isolated compartmental pathology. Central to many UAD phenotypes is type 2 (T2) inflammation, driven by cytokines including IL-4, IL-

Hypertensive heart disease and HFpEF:atrial functional valve regurgitation as the trigger of acute heart failure in a syndrome of exertional dyspnoea.

Hypertensive heart disease (HHD) is widely regarded as the principal structural substrate underlying heart failure with preserved ejection fraction (HFpEF). Accordingly, contemporary models have largely focused on ventricular remodeling, myocardial fibrosis, microvascular abnormalities as the mechanisms linking chronic hypertension to heart failure (HF). However, current concepts increasingly recognize HFpEF as a syndrome predominantly characterized by impaired exercise tolerance and exertional dyspnea without tendency to congestion at rest, whereas the mechanisms responsible for progression t

Canine vector-borne diseases in Lebanon: Unveiling prevalence trends and risk factors for public health and disease control.

Canine vector-borne diseases (CVBDs) pose a significant threat to dogs and public health. Although Lebanon's environmental conditions favor transmission, data on the prevalence and risk factors of CVBDs such as Ehrlichia spp., Anaplasma spp., Borrelia burgdorferi, and Dirofilaria immitis remain scarce. This study investigates the prevalence of these pathogens in dogs across key urban and peri-urban regions (North Lebanon, Mount Lebanon, and Beirut) while identifying associated risk factors contributing to their spread. A cross-sectional study was conducted with 425 dogs. Blood samples were col

Exogenous Hormone Use and Breslow Thickness in Women With Cutaneous Melanoma: A Retrospective Cohort Study.

BACKGROUND: Cutaneous melanoma remains a major public health issue with increasing incidence among fair-skinned populations. Beyond well-established risk factors such as ultraviolet exposure, there is evidence suggesting female sex hormone influence in melanoma biology and prognosis. Although several epidemiological studies have explored the relationship between exogenous hormones and melanoma risk, findings remain inconsistent. This study aims to investigate the association between exogenous hormonal use and Breslow thickness assessing differences across female life stages. METHODS: A retros
