Fatal early onset infection in an extremely low birth weight infant due to Morganella morganii.

OBJECTIVE: This paper reports a case of chorioamnionitis due to Morganella morganii in a mother who presented with ruptured membranes at 24 weeks' gestation and was treated with dexamethasone and prophylactic ampicillin. Her premature infant developed severe early onset infection due to the same organism and expired. STUDY DESIGN: A clinical case report of M. morganii infection complicating preterm rupture of membranes is presented. Possible risk factors for maternal and neonatal infection with this organism as well as the therapy of neonatal M. morganii infection are discussed. RESULTS: Ris

[Para-infectious aseptic meningo-encephalitis following SARS-CoV-2 infection: two case reports].

Neurological manifestations following COVID-19 have been increasingly reported, yet the pathophysiology of para-infectious processes remains unclear. We present two patients who developed headaches, gait instability, cognitive impairments, and inflammatory cerebrospinal fluid (CSF) changes after SARS-CoV-2 infection. Imaging revealed meningitis and encephalitis, respectively, but there was no direct pathogen detection. After excluding other causes and initiating corticosteroid therapy, both patients showed significant improvement. These cases highlight the relevance of autoimmune mechanisms in

Bilateral Optic Neuritis Following Acute Glyphosate Inhalation: A Case Report.

BACKGROUND: Bilateral optic neuritis is a rare condition generally associated with inflammatory, demyelinating, or toxic causes. Its association with glyphosate exposure has rarely been documented. CASE PRESENTATION: A 66-year-old man with hypertension, hypercholesterolemia, and hypothyroidism developed rapid bilateral vision loss within hours after acute inhalational exposure to glyphosate during agricultural work. MRI showed bilateral optic nerve hyperintensity consistent with optic neuritis. Cerebrospinal fluid and serum anti-NMO antibody tests were negative, while visual evoked potentials

Pembrolizumab-Induced Anti-Yo-Antibody-Positive Cerebellitis: A Case Report.

We report a rare case of pembrolizumab-induced anti-Yo-antibody-positive cerebellitis in a patient with stage IV lung adenocarcinoma. Following two cycles of combination therapy, the patient developed nystagmus and ataxia. Despite tumour regression and normal magnetic resonance imaging (MRI) findings, cerebrospinal fluid analysis revealed lymphocyte-dominant hypercellularity, increased protein, and oligoclonal bands. Immune checkpoint inhibitor (ICI)-related cerebellitis was diagnosed after excluding infectious causes, metastases, and cancerous meningitis coupled with a positive anti-Yo antibo

Melioidosis, the great masquerader: From lung to joints: A case report.

Melioidosis, caused by Burkholderia pseudomallei, is an under-diagnosed tropical infection that often mimics tuberculosis and other chest diseases. We report the case of a 42-year old diabetic female who presented with fever and pneumonia and who developed septic arthritis. Blood, joint fluid, and abscess cultures grew B. pseudomallei. Initial treatment was with meropenem but owing to a suboptimal response, the patient was changed to a combination therapy with ceftazidime which led to clinical and biochemical improvement. She was then discharged with a three month course of co-trimoxazole for

Persistent Undocumented Meningoencephalitis due to Histoplasmosis in an HIV-Infected Patient in Brazil.

The diagnosis of central nervous system histoplasmosis is challenging because histoplasmosis can mimic tuberculosis and the performance of mycological diagnostic methods in the cerebrospinal fluid is questionable. A 50-year-old woman with HIV infection, living in Amapa (northern Brazil), on suboptimal antiretroviral therapy for 8 years, presented to our unit with a 3-year history of recurrent undocumented meningoencephalitis, with several unsuccessful empirical antituberculosis and partially successful anticryptococcal regimens, and presented progressive motor weakness in the lower limbs, diff

[Rheumatoid meningitis without arthritis presenting as widespread symmetrical high-‍intensity lesions along cerebral sulci on diffusion-weighted MRI: a case report].

A 52-year-old woman presented with the subacute onset of pantalgia, fever and consciousness disturbance. MRI revealed widespread symmetrical high-intensity areas along the sulci in both cerebral hemispheres on diffusion-weighted imaging (DWI). Rheumatoid meningitis was suspected based on elevated levels of anti-cyclic citrullinated peptide antibodies (ACPA) in both serum and cerebrospinal fluid (CSF), as well as an elevated ACPA index. Brain biopsy revealed marked infiltration of inflammatory cells in the subarachnoid space, confirming the diagnosis of rheumatoid meningitis. The patient fully

Atezolizumab-induced encephalitis with subdural hemorrhage and subarachnoid hemorrhage in a patient with hepatocellular carcinoma.

We report the case of a 76-year-old man who was diagnosed with advanced stage hepatocellular carcinoma and was treated with atezolizumab plus bevacizumab therapy. Two weeks after 1st dose, he presented with acute changes in consciousness followed by hypothermia. A cerebrospinal fluid test showed an elevated cell count, total protein, and albumin. Infectious, anatomical, endocrinal, and neoplastic etiologies were ruled out. Based on the findings, atezolizumab-induced encephalitis was suspected, and high dose steroid therapy was administered. The patient's conscious level and hypothermia recover

Autoantibody Encephalitis: Presentation, Diagnosis, and Management.

Autoantibody encephalitis causes distinct clinical syndromes involving alterations in mentation, abnormal movements, seizures, psychiatric symptoms, sleep disruption, spasms, and neuromyotonia. The diagnoses can be confirmed by specific antibody tests, although some antibodies may be better detected in spinal fluid and others in serum. Each disorder conveys a risk of certain tumors which may inform diagnosis and be important for treatment. Autoantibodies to receptors and other neuronal membrane proteins are generally thought to be pathogenic and result in loss of function of the targets, so un

Abdominal Distension and Vascular Collapse.

We present the case of a 43-year-old gentleman who presented to the emergency room with acute abdominal distension, confusion and vascular collapse. The emergent radiologic imaging obtained showed massive bilateral adrenal enlargement, but despite the initial clinical suspicion of possible overwhelming sepsis and/or massive abdominal/intralesional hemorrhage, lab tests based obtained rapidly confirmed the diagnosis of acute Addisonian crisis which responded dramatically to adrenocorticoid hormone replacement therapy and aggressive fluid resuscitation. The patient's established history of metas

Primary Pulmonary Amebiasis Complicated with Multicystic Empyema.

Amebiasis is a parasitic infection caused by the protozoan Entamoeba histolytica. While most infections are asymptomatic, the disease could manifest clinically as amebic dysentery and/or extraintestinal invasion in the form of amebic liver abscess or other more rare manifestations such as pulmonary, cardiac, or brain involvement. Herein we are reporting a case of a 24-year-old male with history of Down syndrome who presented with severe right side pneumonia complicated with multicystic empyema resistant to regular medical therapy. Further investigation revealed a positive pleural fluid for E.

Deterioration of the immune response induced by sulfamethoxazole-trimethoprim in a rheumatoid arthritis patient with Pneumocystis jirovecii pneumonia.

A 73-year-old woman with rheumatoid arthritis treated with methotrexate and prednisolone was admitted with dyspnea and ground-glass opacity on chest CT. We diagnosed her with Pneumocystis jirovecii pneumonia (PCP) based on a positive PCR analysis of Pneumocystis jirovecii and the presence of cysts in bronchoalveolar lavage fluid. The PaO2 was 74.7 Torr on room air, and treatment with sulfamethoxazole-trimethoprim only was initiated. The hypoxemia and ground-glass opacity increased on hospital day 3, and the administration of adjunctive steroid therapy resulted in an improvement in the patient'

Bacterial meningitis: new therapeutic approaches.

Bacterial meningitis remains a disease with high mortality and long-term morbidity. Outcome critically depends on the rapid initiation of effective antibiotic therapy. Since a further increase of the incidence of pathogens resistant to antibacterials can be expected both in community-acquired and nosocomial bacterial meningitis, the choice of an optimum initial empirical antibiotic regimen will gain significance. In this context, the use of antibiotics which are bactericidal but do not lyse bacteria, may emerge as a therapeutic option. Conversely, the role of corticosteroids, which decrease th
