Injury to the sciatic nerve is one of the more serious complications of acetabular fracture and traumatic dislocation of the hip, both in the short and long term. We have reviewed prospectively patients, treated in our unit, for acetabular fractures who had concomitant injury to the sciatic nerve, with the aim of predicting the functional outcome after these injuries. Of 136 patients who underwent stabilisation of acetabular fractures, there were 27 (19.9%) with neurological injury. At initial presentation, 13 patients had a complete foot-drop, ten had weakness of the foot and four had burning

Articular mass fracture-separation accounts for 9% of the fractures of the lower cervical spine. Neurologic complications are frequent and are usually radicular in nature. Unreduced, these fractures may cause persistent neck pain. The treatment is usually surgical, fusing two or three vertebrae. In this article we present a new treatment modality using reduction and stabilization of the dislocated fragment with a transpedicular lag screw. Previous anatomic studies have shown that the pedicles of the lower cervical spine are wide enough to accept 4.0-mm screws. An anatomic study was performed s

Choice of articulating materials, head size and the design of the articulation will become decisive for the long-term performance of a total hip arthroplasty (THA) and especially in terms of risk for dislocation and wear-related problems. Here we account for common alternatives based on available studies and the evidence that can be derived from them.Metal or ceramic femoral heads articulating against a liner or cup made of highly cross-linked polyethylene and ceramic-on-ceramic articulations have about similar risk for complications leading to revision, whereas the performance of metal-on-met

BACKGROUND: Atlanto-occipital dislocation is a rare and severe injury of the upper spine associated with a very poor prognosis.
CASE PRESENTATION: We report the case of a 59-year-old European man who suffered from out-of-hospital cardiac arrest following a motor vehicle accident. Cardiopulmonary resuscitation was initiated immediately by bystanders and continued by emergency medical services. After 30 minutes of cardiopulmonary resuscitation with a total of five shocks following initial ventricular fibrillation, return of spontaneous circulation was achieved. An electrocardiogram recorded aft

INTRODUCTION: Anterior shoulder dislocations are common injuries usually managed by closed reduction. However, irreducible cases, especially in elderly patients with Alzheimer's disease, complicate both diagnosis and management.
CASE PRESENTATION: A 78-year-old female with Alzheimer's disease presented with severe right shoulder pain and reduced function following physiotherapy. Cognitive decline complicated neurovascular assessment and communication. Radiographs identified a right anterior shoulder dislocation with a displaced greater tuberosity fracture. Multiple closed reductions under sed

BACKGROUND: Total hip arthroplasty (THA) is a common surgical intervention for patients who have seronegative spondyloarthropathies (SpA). However, there is a paucity of literature addressing the outcomes of THA specifically in SpA patients. This study aimed to investigate both the short-term and long-term systemic and orthopaedic outcomes of THA in SpA patients as a whole, as well as within the individual subtypes of SpA.
METHODS: This retrospective cohort study used a federated health research network, identifying 3,074 SpA patients who underwent THA between 2005 and 2022. Propensity score

BACKGROUND: Dual mobility (DM) total hip arthroplasty (THA) implants have been advocated for patients at risk for impingement due to abnormal spinopelvic mobility. Impingement against cobalt-chromium acetabular bearings, however, can result in notching of titanium femoral stems. This study investigated the incidence of femoral stem notching associated with DM implants and sought to identify risk factors.
METHODS: A multicenter retrospective study reviewed 256 modular and 32 monoblock DM components with minimum 1-year clinical and radiographic follow-up, including 112 revisions, 4 conversion T

BACKGROUND: The results of operative treatment of an unreduced elbow dislocation have been regarded with pessimism. Suggested procedures have included tendon-lengthening, tendon transfer, or reconstruction of ligament or bone.
METHODS: Three women and two men (average age, forty-nine years) with an unreduced dislocation of the elbow without associated fractures were treated with open relocation of the joint and hinged external fixation at an average of eleven weeks (range, six to thirty weeks) after the initial injury. The lateral soft tissues, including the origin of the lateral collateral l

BACKGROUND: Indications for surgical fixation of medial epicondyle fractures have been debated for over 4 decades, with no clear consensus supporting operative versus nonoperative management. Medial epicondyle fractures account for ∼11% to 20% of pediatric elbow fractures and are associated with posterior elbow dislocation in up to 60% of cases. The purpose of this study was to evaluate long-term outcomes of surgical versus nonsurgical management of medial epicondyle fractures with a minimum follow-up of 2 years.
METHODS: Pediatric patients evaluated for a medial epicondyle fracture at a tert

BACKGROUND: Improvements in spinal fusion devices and techniques have enabled stronger spinal fusion, resulting in excellent clinical outcomes. Nevertheless, complications associated with implants, such as screw misalignment, screw lubrication, cage dislocation, and skin issues, might occur. This study aimed to investigate the characteristics and symptoms of sacral fractures after spinal instrumented fusion.
METHODS: This case series retrospectively examined the medical records of eight patients (one man and seven women; mean age: 74 years) diagnosed with sacral fractures after undergoing pos

BACKGROUND: Unstable atlas fractures with concomitant transverse atlantal ligament (TAL) injury may be conservatively managed by halo-vest immobilization (HVI) or surgically treated by various fixation techniques. Many surgeons prefer surgical management due to complications, nonunion, and further dislocations with HVI. There are no comparative studies on surgical and nonsurgical management of unstable atlas fractures. We retrospectively assessed the radiological and clinical outcomes of surgical reduction with fixation vs. non-operative treatments for unstable atlas fractures with TAL rupture

BACKGROUND: Fractures of the acetabulum in younger patients are commonly treated by open reduction and internal fixation. For elderly patients, stable primary total hip arthroplasty with the advantage of immediate postoperative mobilization might be the adequate treatment. For this purpose, a sufficiently stable fixation of the acetabular component is required.
MATERIALS AND METHODS: Between August 2009 and 2014, 30 cases were reported in which all patients underwent total hip arthroplasty additionally to a customized implant designed as an antiprotrusion cage. Inclusion criteria were an acet

BACKGROUND: Traumatic lumbosacral instability is a rare but potentially devastating injury. These injuries are frequently associated with neurologic injury and often result in long-term disability. Despite their severity, radiographic findings can be subtle, and multiple reports exist in which these injuries were not recognized on initial imaging. Transverse process fractures, high-energy mechanisms, and other injury features have been suggested as indications for advanced imaging, which has a high degree of sensitivity in detecting unstable injuries.
CASE PRESENTATION: A 21-year-old man pres
