[Nonunion after distal radius fracture: A review].

Nonunion after distal radius fracture is rare, serious and unpredictable. The preferred treatment is resection of the nonunion zone followed by open reduction and internal fixation with autologous iliac crest bone graft. When the distal fragment is less than 5mm high, a dorsal plate that bridges the radiocarpal joint is recommended. In patients with low functional demands, radiocarpal fusion is a logical choice. The bone healing rate is 58% and could be improved by using a vascular graft. Bone healing is not synonymous with good clinical outcomes. Nonunion after distal radius fracture is a ser

Sialocele: A rare sequlae of transparotid approach in subcondylar fracture management.

Iatrogenic injury and/or damage to the parotid salivary gland during transparotid approach for open reduction and internal fixation of mandibular condyle fracture is a rare event. Accumulation of saliva in the gland leads to formation of a sialocele. Huge sialocele often seeks drain through the most dependent area through an extraoral wound, whereas in the absence of extraoral fistula, saliva can be redirected intraorally using a stent. A case of mangement of sialocele caused by damage to glandular elements during a transparotid approach for a subcondylar fracture reduction is reported. The va

[Intramedullary fixation of displaced middle phalangeal neck fractures].

Middle phalanx neck fracture is an unusual injury. In order to prevent malunion and stiffness, a surgical treatment is usually recommended. Owing to the distal localization of the fracture, internal fixation is demanding. Open reduction can further damage the soft tissues and the bone itself, resulting in stiffness or condylar necrosis. The bulk of plate and screws can interfere with the extensor apparatus and transarticular or cross K-wires may damage the joint surfaces or ligaments. An intramedullary pinning technique is reported minimizing these pitfalls. The technique is described and illu

Primary arthroplasty for management of osteoporotic fractures about the knee.

Primary cemented arthroplasty of the knee is a viable alternative to open reduction and internal fixation (ORIF) for treatment of osteoporotic fractures about the knee. This permits early return of knee function and weight bearing activity. Stemmed revision total knee arthroplasty implants and techniques are needed, which can be associated with complications of late loosening and periprosthetic fracture. However, for elderly sedentary patients who would not be expected to outlive the durability of the arthroplasty and with fracture patterns in which ORIF may be associated with poor outcomes, p

Simultaneous bilateral patellar tendon avulsion in an adolescent.

A 13-year-old boy sustained an injury to both knees upon landing after a forceful jump in a soccer game. Plain radiography and magnetic resonance imaging demonstrated bilateral distal patellar tendon avulsions without fracture of the tibial tuberosities and the physes. To our knowledge, this particular injury has not been previously described in the literature. Open surgeries and internal fixation were performed with excellent functional outcome. This type of injury was similar to the well-recognised acute tibial tuberosity avulsion fracture in terms of the pathogenesis and treatment. We propo

Coronal shear fracture of the distal humerus in an 11-year-old patient.

Coronal shear fractures of the distal humerus are uncommon in the pediatric population. We present the case of an 11-year-old girl who sustained a coronal shear fracture of the distal humerus, requiring open reduction and internal fixation with the use of headless screws. At the time of the latest follow-up, she had fully recovered the range of motion on the affected limb, had returned to full physical activities without limitations, radiographs showed a completely healed fracture with restoration of the articular line, and there was no signs of avascular necrosis of the capitellum.

Late lower extremity fractures in patients with paraplegia.

We report the result of the treatment of 26 lower limb fractures in patients with established paraplegia. We treated one fracture of the femoral neck, one intertrochanteric, two subtrochanteric, two of the femoral shaft, seven of the supracondylar region, six of the proximal tibia, one of the tibial shaft, and six pilon fractures. Nine fractures were treated non-operatively, and the remaining fractures were treated operatively. Union was achieved in all the patients, but we think that open reduction and internal fixation may improve the level of independence and mobility of these patients duri

The unstable shoulder girdle: early functional treatment utilizing open reduction and internal fixation.

The unstable shoulder girdle--a combination of a scapular neck fracture with a clavicular fracture, acromioclavicular dislocation, or sternoclavicular dislocation--represents a rare and often underestimated injury. Sufficient treatment to permit early function is necessary to avoid a frozen shoulder as well as to treat concomitant thoracic injuries. This is possible only by open reduction and internal fixation. In most cases this can be achieved by a sole osteosynthesis of the clavicular injury. We retrospectively analyzed 12 cases and classified the results according to the Constant Score. Th

[Dislocation fracture of the head of the tibia. II. Therapeutic procedure].

Fracture dislocations of the tibial plateau have associated pathology including ligament- and meniscus tears. They therefore require different treatment than split- or impression fractures. The operative and nonoperative management depends on the soft tissue injuries including ligament tears and depends also on the functional requirements of the patient. Surgical approach, operative time and protection of vascular supply of soft tissue and bony fragments are the main factors influencing postoperative complications and outcome. The different fracture dislocation types according to the Moore cla

[Hamulus fracture: a frequently unrecognized injury. Presentation of diagnosis and therapy based on two case reports].

Fractures of the hook of the hamate are rare injuries of the hand. They are caused by direct trauma to the palm, however there are some cases without history of trauma. While physical examination is often suspicious of fracture, standard X-ray films can fail to demonstrate the fracture. Bone scans show an increased uptake in the ulnar region of the wrist. The carpal tunnel view or computed tomography confirm the diagnosis. Operative treatment is recommended. Simple excision of the hook, bone grafting or internal fixation are possible procedures. The diagnostic problems are demonstrated in two

Proximal ulnar fractures in adults: a review of 163 cases.

The aim of this study was to report the epidemiological characteristics and the experience of 5 departments of trauma, in France, in the management of fractures of the proximal ulna. 163 patients with fractures of the proximal ulna with a mean age of 49.9 years (range 16-97) were managed. The most common mode of injury was a motor vehicle collision (48%). 18% sustained associated injuries to the ipsilateral limb. Open fractures were present in 42 patients (25%). A total of 109 patients had a fracture of the olecranon, with the Mayo 2A and B types most frequently seen (66%). The patients were i

Simultaneous Surgical Correction of Skeletal Class III Dentofacial Deformity During Acute Management of Facial Fractures: Patient Selection and Surgical Technique.

BACKGROUND: Mandibular prognathism is a common dentofacial deformity in Asia. Treatment of such condition may vary from orthodontic camouflage to surgical orthodontics with orthognathic surgery depending on the severity of the condition. Because of the prominent position of the mandible, fractures involving different locations of the mandible commonly occur in maxillofacial trauma. Anatomical reduction of maxillofacial fractures and restoration of the pretraumatic occlusion are the primary goals of acute management of facial fractures. In patients with dentofacial deformity, simultaneous surgi

Unilateral lag-screw technique for an isolated anterior 1/4 atlas fracture.

STUDY DESIGN: Fractures of the atlas are classified based on the fracture location and associated ligamentous injury. Among patients with atlas fractures treated using external immobilization, nonunion of the fracture could be seen. OBJECTIVE: Ideally, treatment strategy for an unstable atlas fracture would involve limited fixation to maintain the fracture fragments in a reduced position without restricting the range of motion (ROM) of the atlantoaxial and atlantooccipital joints. SUMMARY OF BACKGROUND DATA: Such a result can be established using either transoral limited internal fixation or

The Community Orthopaedic Surgeon Taking Trauma Call: Lateral Humeral Condyle Fracture Pearls and Pitfalls.

Lateral condyle fractures are second only to supracondylar humeral fractures when it comes to pediatric elbow fractures. Whether minimally displaced or substantially displaced, these growth plate fractures may present significant treatment challenges. These include nonunion, malunion, avascular necrosis, and growth disturbance. It must be remembered that lateral condyle fractures often represent significantly displaced intra-articular fractures involving the epiphysis that demand anatomic reduction and stable internal fixation. This article focuses on some of the pertinent pearls and pitfalls

Management of acute fractures and nonunions of the proximal pole of the scaphoid.

It is my belief that all acute proximal pole fractures should be treated by open reduction and internal fixation, via a dorsal approach. There is no longer any place for conservative treatment of these fractures, because a lengthy period of plaster immobilization is required and there is an unacceptably high risk of nonunion with conservative management. Unfortunately, nonunion of the proximal pole remains a common and disabling problem which demands careful evaluation and treatment. Internal fixation combined with limited cancellous bone grafting produces very satisfactory results in terms of

Interfragmentary Compression Forces Vary Based on Scaphoid Bone Screw Type and Fracture Location.

BACKGROUND: The objective of this study was to determine the interfragmentary compression forces generated in a foam model as a function of headless compression screw type (fully threaded and central threadless) and fracture location. METHODS: Eighty-eight polyurethane foam models were fixed across a simulated transverse fracture with either a fully threaded screw or a central threadless screw. The location of the transverse fracture varied along the length of the foam model in 2 mm increments for 11 fracture locations. The force generated at the fracture site upon fixation was utilized to de
