Effects on cognition and quality of life with unilateral magnetic resonance-guided focused ultrasound thalamotomy for essential tremor.

OBJECTIVE Although neurosurgical procedures are effective treatments for controlling involuntary tremor in patients with essential tremor (ET), they can cause cognitive decline, which can affect quality of life (QOL). The purpose of this study is to assess the changes in the neuropsychological profile and QOL of patients following MR-guided focused ultrasound (MRgFUS) thalamotomy for ET. METHODS The authors prospectively analyzed 20 patients with ET who underwent unilateral MRgFUS thalamotomy at their institute in the period from March 2012 to September 2014. Patients were regularly evaluated

An examination of the association between prenatal cocaine exposure and brain activation measures of arousal and attention in young adults: An fMRI study using the Attention Network Task.

Prenatal drug exposure, including cocaine, alcohol, marijuana, and tobacco, is associated with deficits in behavioral regulation and attention. Using fMRI, the objective of this study was to characterize the association between prenatal cocaine exposure (PCE) and the underlying neural substrates associated with behavioral outcomes of attention. Forty-seven young adults were recruited for this study from the ongoing Maternal Health Practices and Child Development (MHPCD) Project, a longitudinal study of the effects of PCE on growth, behavior, and cognitive function. Three groups were compared:

Early diagnosis, monitoring, and treatment of optic neuritis.

BACKGROUND: About half of multiple sclerosis patients present with optic neuritis (ON) as a clinically isolated syndrome (CIS). In the Optic Neuritis Treatment Trial study, 28% of patients with ON and an abnormal brain magnetic resonance imaging (MRI) did not have a relapse at the end of 15 years. It is still difficult to predict which CIS patients will go on to develop clinically definite multiple sclerosis and which will have a benign course. REVIEW SUMMARY: This review focuses on more advanced methods of detecting and quantifying ON in multiple sclerosis that have been developed in the pas

Cognitive functioning in Behçet's disease.

The impact of Behçet's disease on higher cognitive functions is still poorly understood. We proposed (1) to characterize the neuropsychological profile of Behçet's disease patients with (Neuro-BD) and without (BD) neurological manifestations; (2) to identify which clinical, psychopathological, and genetic variables are related to neuropsychological performance; and (3) to explore the association between cognitive functioning and neuroimaging findings in BD patients. Fifteen Neuro-BD and 35 BD patients in the nonactive phase of their illness underwent a neurological examination, performed a com

The neuropsychology of obsessive compulsive disorder: the importance of failures in cognitive and behavioural inhibition as candidate endophenotypic markers.

Obsessive compulsive disorder (OCD) is a highly debilitating neuropsychiatric condition with estimated lifetime prevalence of 2-3%, more than twice that of schizophrenia. However, in contrast to other neuropsychiatric conditions of a comparable or lesser prevalence, relatively little is understood about the aetiology, neural substrates and cognitive profile of OCD. Despite strong evidence for OCD being familial, with risk to first-degree relatives much greater than for the background population, its genetic underpinnings have not yet been adequately delineated. Although cognitive dysfunction i

[How to examine a patient with higher cortical dysfunction at bedside].

In practice, a neuropsychological evaluation may serve one or more purpose. The first systematic applications of neuropsychological assessment dealt with diagnosis. Before the days of sophisticated neuroimaging, the localization of a cerebral lesion was neuropsychology's most important function. Recent developments in neuroradiological techniques have greatly reduced the contributions of neuropsychological assessment to diagnosis and lesion localization. Today, neuropsychological assessment is most usually called upon for the detailed behavioral description necessary for intelligent patient ca

Association of the Discordance Between Apolipoprotein B and Low-Density Lipoprotein Cholesterol With Cognition.

BACKGROUND: This study aimed to investigate and validate the association of the discordance between apoB (apolipoprotein B) and low-density lipoprotein cholesterol (LDL-C) or non-high-density lipoprotein cholesterol with cognitive function and explore the mediating role of cerebral large- and small-vessel diseases on this association, for which previous evidence was limited. METHODS: Community-dwelling adults aged >50 years from the Polyvascular Evaluation for Cognitive Impairment and Vascular Events study were included in this cross-sectional study. Global cognitive function was evaluated us

Body mass index decline and cognitive trajectories in mild cognitive impairment: the role of maternal history of Alzheimer's disease.

BACKGROUND: Maternal history of Alzheimer's disease (MH) has been associated with increased risk of Alzheimer's disease (AD), earlier onset, and distinct neurobiological features. In parallel, an anthropometric-metabolic phenotype characterized by body mass index (BMI) decline and unintentional weight loss has been described as an early feature preceding cognitive decline in the prodromal stages of AD. OBJECTIVE: To investigate whether MH is associated with a distinctive anthropometric-metabolic and cognitive phenotype in individuals with mild cognitive impairment (MCI), focusing on longitudi

APOE4-specific glymphatic effects on clinical progression in the Alzheimer's Disease Neuroimaging Initiative with pathological correlates in the A4 study.

BackgroundThe APOE ε4 allele is the strongest genetic risk factor for late-onset Alzheimer's disease (AD). However, some carriers show "clinical resilience", maintaining their cognition despite high risk.ObjectiveWe hypothesized that the glymphatic system, the brain's waste clearance pathway, may attenuate APOE ε4-mediated neurodegeneration, and tested this in two independent cohorts.MethodsWe analyzed 625 cognitively normal older adults from the Alzheimer's Disease Neuroimaging Initiative (ADNI) discovery cohort and 447 amyloid-positive participants from the preclinical Anti-Amyloid Treatment

BDNF Val66Met moderates memory impairment, hippocampal function and tau in preclinical autosomal dominant Alzheimer's disease.

SEE ROGAEVA AND SCHMITT-ULMS DOI101093/AWW201 FOR A SCIENTIFIC COMMENTARY ON THIS ARTICLE: The brain-derived neurotrophic factor (BDNF) Val66Met polymorphism is implicated in synaptic excitation and neuronal integrity, and has previously been shown to moderate amyloid-β-related memory decline and hippocampal atrophy in preclinical sporadic Alzheimer's disease. However, the effect of BDNF in autosomal dominant Alzheimer's disease is unknown. We aimed to determine the effect of BDNF Val66Met on cognitive function, hippocampal function, tau and amyloid-β in preclinical autosomal dominant Alzheime

Neuroanatomical correlates of cognitive functioning in prodromal Huntington disease.

INTRODUCTION: The brain mechanisms of cognitive impairment in prodromal Huntington disease (prHD) are not well understood. Although striatal atrophy correlates with some cognitive abilities, few studies of prHD have investigated whether cortical gray matter morphometry correlates in a regionally specific manner with functioning in different cognitive domains. This knowledge would inform the selection of cognitive measures for clinical trials that would be most sensitive to the target of a treatment intervention. METHOD: In this study, random forest analysis was used to identify neuroanatomica

Cerebral blood flow and metabolic abnormalities in Alzheimer's disease.

In this review I summarize observations of PET and SPECT studies about cerebral blood flow and metabolic abnormalities in Alzheimer's disease. In very early AD flow or metabolism reduces first in the posterior cingulate gyrus and precuneus. This reduction may arise from functional deafferentation caused by primary neural degeneration in the remote area of the entorhinal cortex that is the first to be pathologically affected in AD. Then medial temporal structures and parietotemporal association cortex show flow or metabolic reduction as disease processes. The reason why flow or metabolism in me

Clinical and Pathological Progression of Awareness Trajectories in Preclinical Alzheimer's Disease.

Subtle alterations in awareness may emerge in the preclinical stage of Alzheimer's disease (AD), yet their clinical significance and translational relevance remain unclear. This study aimed to evaluate associations of distinct awareness trajectories with clinical and multimodal AD biomarker measurements in cognitively unimpaired (CU) older adults. This prospective study analyzed data from the Anti-Amyloid Treatment in Asymptomatic Alzheimer's (A4) and Longitudinal Evaluation of Amyloid Risk and Neurodegeneration (LEARN) cohorts (∼4.5-year follow-up). Awareness trajectories were defined using a

[Anorexia nervosa in the light of neurocognitive functioning: New theoretical and therapeutic perspectives].

INTRODUCTION: Anorexia nervosa is a serious psychiatric disorder, for which very few validated therapeutic strategies exist. The specific sociocognitive style of anorexic patients has already been described in the 1960s: it involves a concrete style with abstraction difficulties. Current neuropsychological tests have contributed to a more precise definition of these difficulties. NEUROPSYCHOLOGICAL DATA: IS THERE A SPECIFIC COGNITIVE PROFILE?: Contrary to common beliefs, these patients' intellectual performances are not superior to those of the general population. However, detailed comparison

Association of neighborhood disadvantage with cognitive function and cortical disorganization in an unimpaired cohort: An exploratory study.

INTRODUCTION: Neighborhood disadvantage has been shown to impact health and cognitive outcomes, while morphological similarity network (MSN) can elucidate structural morphological patterns underlying cognitive functions. We hypothesized MSNs could provide cortical patterns linked with neighborhood disadvantage and cognitive function, explaining the potential risk of cognitive impairment in disadvantaged neighborhoods. METHODS: For cognitively unimpaired participants from the Wisconsin Alzheimer's Disease Research Center or Wisconsin Registry for Alzheimer's Prevention (n = 524), and the Alzhe

Similarity-guided multi-view functional brain network fusion.

Understanding the intricate patterns and interactions within functional brain networks (FBNs) is crucial for the accurate diagnosis and analysis of mental disorders. Brain function can be represented through different brain networks, each providing complementary insights into underlying neural processes. Integrating data from these different sources enables a more comprehensive and precise understanding of brain function. However, effectively combining these heterogeneous data while maintaining the structural integrity of each modality remains a critical challenge. To address this challenge, w

Goal-Oriented Attentional Self-Regulation Training in Chronic Mild Traumatic Brain Injury is Linked to Microstructural Plasticity in Prefrontal White Matter.

Impaired attention is one of the most common, debilitating, and persistent consequences of traumatic brain injury (TBI), which impacts overall cognitive and executive functions in these patients. Previous neuroimaging studies, trying to understand the neural mechanism underlying attention impairment post TBI, have highlighted the role of prefrontal white matter tracts in attentional functioning in mild TBI (mTBI). Goal-Oriented Attentional Self-Regulation (GOALS) is a cognitive rehabilitation training program that targets executive control functions in participants by applying mindfulness-base
