Long-term results after surgical treatment of renal hyperparathyroidism when fewer than four glands are identified at operation.

BACKGROUND: When fewer than four parathyroid glands are discovered during cervical exploration in cases of renal hyperparathyroidism, some have suggested that the glands could be resected and that no parathyroid tissue should be grafted. The aim of the present study was to evaluate this approach. Indeed, no detailed follow-up of such patients has been reported so far. STUDY DESIGN: Between September 1979 and July 1995, 157 patients underwent a cervical exploration for renal hyperparathyroidism in our department. In 23 cases, fewer than four parathyroid glands were found. The present study rep…

Resetting of parathyroid hormone secretion after vitamin D3 treatment in hypoparathyroidism and after parathyroid adenectomy in primary hyperparathyroidism.

The relationship between parathyroid hormone (PTH) secretion and extracellular calcium (Ca) level is reciprocal causality. The equilibrium operating point determines basal PTH secretion rate and basal extracellular Ca level. We studied how this equilibrium was achieved in the subjects with decreased PTH secretion or decreased parathyroid glands number. Basal/maximum ratio of serum PTH, which reflects the basal secretory state of parathyroid glands, was increased in 9 hypoparathyroid patients treated with vitamin D3 (VD3) [7 patients with idiopathic hypoparathyroidism] and in seven of nine para

Localization of abnormal parathyroid gland(s) using thallium-201/iodine-123 subtraction scintigraphy in patients with primary hyperparathyroidism.

Tl-201/I-123 subtraction scintigraphy was performed in 17 patients with clinical symptoms and biochemical measurements suggestive of primary hyperparathyroidism. Nineteen abnormal sites were identified. These results were correlated with PTH measurements and surgical findings. Three sites were considered unrelated to the parathyroid glands, two corresponding to palpable thyroid nodules and one to muscle uptake of unknown origin. One scintigram did not reveal either of two abnormal glands while two others were considered falsely positive in view of surgical failure. Fourteen sites corresponded …

Clinical usefulness of the (chloride-90)/phosphate ratio for distinguishing primary hyperparathyroidism from hypercalcemia due to other causes.

In order to obtain a good separation line between patients with primary hyperparathyroidism (1 degree HPT) and those with non-parathyroidal hypercalcemia (NPHC), serum chloride (Cl) and phosphate (P) concentrations were analyzed. Ninety-nine per cent of the patients with 1 degree HPT had a Cl/P ratio greater than or equal to 33, but 29% of patients with NPHC were also included in this range. When the (Cl-90)/P ratio was used to separate into two groups, 98% of the patients with 1 degree HPT had a ratio greater than or equal to 5.0, and 94% of the patients with NPHC had a ratio less than 5.0. F…

[Hyperparathyroidism secondary to cirrhosis. Arguments supplied by ionized calcium, urinary cyclic AMP and blood N-terminal parathyroid hormone].

A laboratory study including estimation of 25 OH vitamin D, terminal parathormone (PTH) C and N fractions, urinary cyclic AMP (AMP cU) and ionised calcium, was carried out in 25 patients, 10 cases of alcoholic cirrhosis without decompensation (group 1) and 15 cases of decompensated cirrhosis (group 2) in order to seek evidence in favour of hyperparathyroidism secondary to cirrhosis. The results show: 1) The existence of hypovitaminosis D which seems to be independent of the liver failure. 2) A very definite increase in terminal PTH N in group 2 compared with group 1 (p < 0.01), without any inc…

Challenges and unmet needs of [18F]fluorocholine PET in hyperparathyroidism: A framework for refining current practice and advancing parathyroid imaging.

The advent of [18F]fluorocholine (FCH) positron emission tomography (PET)/computed tomography (CT) or magnetic resonance (MR) imaging has enhanced the diagnostic performance of preoperative imaging in patients with primary and renal hyperparathyroidism (HPT) during the past decade [1, 2]. Numerous studies and meta-analyses have confirmed the excellent performance of FCH PET, including in complex cases (i.e., inconclusive or negative results on parathyroid scintigraphy, recurrence, renal hyperparathyroidism (HPT), multiple endocrine neoplasia type 1 (MEN1)-related pHPT), with diagnostic accurac…

Serum Biomarker-Based Diagnostic Tools for Primary Hyperparathyroidism: A Systematic Review and Meta-Analysis with Implications for Primary Care.

Background: Hyperparathyroidism is a common endocrine disorder, and its diagnosis can be complex. Various indices based on blood biomarkers have been proposed to improve diagnostic accuracy. The objective of this systematic review was to analyze the diagnostic utility of different indices in primary hyperparathyroidism. Methods: A systematic review was performed with searches up to January 2026. Risk of bias was assessed, and a meta-analysis was conducted for indices with two or more studies, calculating sensitivity, specificity, and other accuracy measures. The certainty of the evidence was e…

Parathyroidectomy timing for persistent hyperparathyroidism in kidney transplant candidates and recipients: from secondary to tertiary disease: a literature review.

BACKGROUND AND OBJECTIVE: Secondary hyperparathyroidism (SHPT) in patients with chronic kidney disease (CKD) is a major challenge in the context of patients awaiting renal transplantation. Optimal timing of parathyroidectomy (PTX) for these patients remains controversial, particularly to prevent tertiary hyperparathyroidism (THPT) and minimize postoperative complications such as hungry bone syndrome (HBS). The aim of this study is to evaluate current evidence on optimal timing of PTX in renal transplant candidates. METHODS: To review the current evidence regarding the optimal timing of PTX in…

Cost-effectiveness analysis comparison of etelcalcetide, paricalcitol, and cinacalcet for the treatment of secondary hyperparathyroidism in patients with chronic kidney disease.

BACKGROUND: Secondary hyperparathyroidism is a common complication in patients with chronic kidney disease (CKD), which can lead to cardiovascular diseases and the risk of fractures. Calcimimetics and active vitamin D analogs are the most frequently prescribed therapeutic drugs for treating this illness. RESEARCH DESIGN AND METHODS: A decision-tree model was constructed from the perspective of the Chinese healthcare system, with a 1-year time horizon. The primary outcome was the incremental cost-effectiveness ratio (ICER). One-way and probabilistic sensitivity analyses were performed. RESULT…

Severe Hyperparathyroidism in a 3-year-old girl Due to Homozygous Inactivating Variant of the Calcium Sensing Receptor Gene.

Homozygous inactivating pathogenic variants of calcium-sensing receptors cause severe hyperparathyroidism (HPT), typically presenting in early infancy. In the pediatric age group, HPT is uncommon and can be due to parathyroid adenoma or hyperplasia. We describe the case of a 3-year, 10-month-old girl who presented with severe HPT with symptomatic hypercalcemia, skeletal demineralization, and bone pains resulting from a homozygous missense variant p.Gly670Arg in the CaSR gene that was previously reported in association with familial hypocalciuric hypercalcemia in its heterozygous state. Parathy…

Effect of Parathyroidectomy Timing on the Successful Resolution of Tertiary Hyperparathyroidism in Kidney Transplant Recipients: A Systematic Review and Meta-Analysis.

Introduction: Tertiary hyperparathyroidism following kidney transplantation is a well-recognized complication in patients with pre-existing mineral imbalances due to chronic renal failure. Parathyroidectomy remains the only definitively curative option for tertiary hyperparathyroidism. The optimal timing of parathyroidectomy, before or after transplantation, is debated in the literature. This study aims to assess whether parathyroidectomy timing affects the successful resolution of tertiary hyperparathyroidism in patients with a functional kidney transplant. Methods: We conducted a systematic …

Lithium-Induced Hyperparathyroidism in a Patient With Bipolar Disorder: The Importance of Monitoring Complications in Patients Undergoing Chronic Lithium Therapy.

Lithium remains a cornerstone in the pharmacological management of bipolar disorder due to its well-established mood-stabilizing properties. However, chronic lithium therapy is associated with a spectrum of potential complications, including hyperparathyroidism and progression to chronic kidney disease (CKD). We present the case of a patient with bipolar disorder who developed lithium-induced hyperparathyroidism and CKD following long-term lithium therapy. During a routine follow-up, the patient was found to have hypercalcemia, elevated parathyroid hormone levels, and increased serum creatinin…

A Challenging Case of Secondary Hyperparathyroidism From Hypovitaminosis D in a Young Man With Hypertensive Crisis and Target Organ Damage.

Hypertension is a common global condition, with most cases being primary and a minority arising from secondary causes, often endocrine in or … cular hypertrophy, consistent with target organ damage. Further investigations identified secondary hyperparathyroidism due to profound hypovitaminosis D as the likely cause. Targeted treatment corrected the vitamin D deficiency and hypocalcaemia while gradually controlling blood pressure, leading to a favourable outcome. This exceptionally rare presentation undersco …

A case report of primary hyperparathyroidism due to a giant substernal parathyroid adenoma managed using a neck incision approach.

RATIONALE: Primary hyperparathyroidism is an endocrine disease primarily caused by a single benign parathyroid adenoma. It is characterized by symptoms such as hypercalcemia, hypophosphatemia, kidney stones, and bone destruction. Parathyroid adenomas are generally located on the posterior aspect of the thyroid gland, while giant parathyroid adenomas in the anterior superior mediastinum are relatively rare. Typically, benign anterior superior mediastinal tumors are managed through sternotomy or thoracoscopic surgery, which can be associated with significant trauma or complications. PATIENT CON…
