Pathogen-specific immunity debt in children after prolonged nonpharmaceutical interventions: a cross-sectional study in China.

BACKGROUND: During the COVID-19 pandemic, prolonged nonpharmaceutical interventions (NPIs) reduced the circulation of respiratory pathogens. The effects of NPIs on "immunity debt" in children are unclear, especially after almost 3 years of NPIs in China. METHODS: Between November 2021 and October 2023, a cross-sectional study of 235 children (age 9 months-5 years) was conducted at the China-Japan Friendship Hospital, Beijing. Serum IgG antibodies against 10 respiratory pathogens, including influenza A, influenza B, respiratory syncytial virus (RSV), parainfluenza virus, adenovirus (types 7 an

Exploring multidrug resistance patterns in community-acquired Escherichia coli urinary tract infections with machine learning.

While associations of antibiotic resistance traits are not random in multidrug-resistant (MDR) bacteria, clinically relevant resistance patterns remain underexplored. This study used association-set mining to explore resistance associations within E. coli isolates from community-acquired urinary tract infection isolates collected from 2018 to 2022 by France's national surveillance system. Association-set mining was applied separately to extended-spectrum beta-lactamase-producing E. coli (ESBL-EC) and non-ESBL-EC. MDR patterns with expected support (reflecting pattern frequency) and conditional

EXERT-BCN: An Exercise Regimen Designed to Improve Body Composition, Functional Capacity, and Strength After Treatment for Breast Cancer With Nutrition Optimization.

PURPOSE: Effective methods to improve body composition and metabolic/hormonal dysregulation are central to breast cancer care. We hypothesized that a nutrition regimen focused on food quality and an observed exercise regimen using high-load resistance training during or after cancer treatment would improve body composition and functional capacity. METHODS: Forty-four women with breast cancer, including survivors on therapy and in surveillance, excluding chemotherapy, underwent a continuously monitored dose-escalated exercise regimen utilizing heavy weights and linear progression in an exercis

The Environmental and Human Health Impact of Virtual General Surgery Residency Interviews With an In-Person Second Look.

PURPOSE: Health care contributes to climate change through several mechanisms, including potentially avoidable travel. A major change in academic medicine during the COVID-19 pandemic was the transition to virtual interviews and resultant reduced environmental impact. However, as travel restrictions have lifted, programs have explored interview styles incorporating both virtual and in-person aspects. This study estimated the net carbon emissions from a single general surgery residency program that offered virtual interviews paired with an optional in-person second-look event and quantified the

Validating Loon Lens 1.0 for Autonomous Abstract Screening and Confidence-Guided Human-in-the-Loop Workflows in Systematic Reviews.

OBJECTIVES: Title and abstract screening is a labor-intensive step in systematic literature reviews (SLRs). We examine the performance of Loon Lens 1.0, an agentic artificial intelligence platform for autonomous title and abstract screening and test whether its confidence scores can target minimal human oversight. METHODS: A total of 8 SLRs by Canada's Drug Agency were rescreened through dual human reviewers and adjudicated process (3796 citations, 287 includes, 7.6%) and separately by Loon Lens, based on predefined eligibility criteria. Accuracy, sensitivity, precision, and specificity were

Managing apnoea in trauma victims: the potential for dispatcher-assisted airway handling - a narrative review.

Trauma remains one of the leading causes of death worldwide, particularly among young adults. Despite improvements in structured trauma systems and hospital-based interventions, a substantial proportion of preventable deaths still occur in the prehospital phase, often within minutes and well before emergency medical services (EMS) intervention. Among the most critical and potentially reversible contributors to early mortality is apnoea, which can rapidly lead to hypoxic injury and death. This narrative review aims to explore the pathophysiology, epidemiology, and recognition of early post-trau

Perioperative Factors Associated with 2-Year Neurodevelopmental Outcome in Neonates Undergoing Congenital Cardiac Surgery with Cardiopulmonary Bypass: A Single-Center Cohort Study.

Advances in neonatal cardiac surgery have significantly reduced mortality rates for complex congenital heart diseases (CHD). However, long-term morbidities, particularly neurodevelopmental (NDV) outcomes, remain a concern. Perioperative factors implicated in NDV delays are less described. This study aimed to assess NDV outcomes at 2 years in a cohort of neonates undergoing cardiopulmonary bypass (CPB) and identify perioperative variables associated with non-optimal NDV. This analysis included neonates from the Loire Infant Follow-up Team (LIFT) cohort who underwent CPB between 2014 and 2021. N

An Innovative Approach to Chin Ptosis Correction with Surgical Net.

OBJECTIVE: Chin ptosis, characterized by the descent of soft tissues below the inferior mandibular border, significantly impacts lower facial aesthetics. Traditional facial rejuvenation procedures often fail to comprehensively address this condition. The surgical net technique offers a novel approach for precise repositioning and stabilization of the chin. This study evaluates the clinical outcomes of this technique in correcting chin ptosis within comprehensive facial rejuvenation procedures. METHODS: A retrospective review was conducted on 20 patients who underwent correction of chin ptosis

Outcome measures of instrumented gait analysis in hereditary spastic paraplegia: a systematic review.

BACKGROUND: Hereditary spastic paraplegias (HSPs) comprise a group of genetic movement disorders characterized by progressive spasticity and weakness of the lower limbs leading to gait deficits. Instrumented gait measures are applied to quantify gait patterns in HSP objectively. However, there is no consensus on the most relevant HSP-specific digital outcome measures for future clinical studies. AIM: This systematic review aims to summarize outcome measures of instrumented gait analysis in HSP patients, focusing on both traditional motion capture (MOCAP) and inertial sensor systems. METHODS:

Precise bonebridge implantation in challenging cases: a novel approach using virtual planning and electromagnetic navigation.

BACKGROUND: The second generation of the Bonebridge (BB) bone conduction implant was shown to be a viable option even for younger children with conductive or mixed hearing loss. However, preoperative imaging often shows only small areas where the FMT (4.5 mm) or screws (4.0 mm) can be fully and safely inserted without the need to use lifts. Navigation systems allow precise placement of the device and prevent potential complications such as dural or vascular injuries. The latest version of the preoperative planning software Otoplan® allows to assess bone thickness and perform virtual implantati

Patients' perspectives of ophthalmic management of facial nerve paralysis.

OBJECTIVE: Facial nerve paralysis (FNP) affects eye muscle function causing lagophthalmos. This leaves the cornea vulnerable to irritation and injury which, if untreated, may cause permanent vision impairment. Decreased eye muscle control impacts facial expressions and quality of life. Although non-surgical interventions such as lubrication are routinely used in standard eyecare, patient perspectives regarding these interventions have not been extensively investigated. This study aims to investigate patients' perspectives of conservative ophthalmic management for FNP. METHODS AND ANALYSIS: Pa

Is routine axillary lymph node dissection needed to tailor systemic treatments for breast cancer patients in the era of molecular oncology? A position paper of the Italian National Association of Breast Surgeons (ANISC).

BACKGROUND: De-escalation of axillary surgery in breast cancer (BC) management began when sentinel lymph node biopsy (SLNB) replaced axillary lymph node dissection (ALND) as standard of care in patients with node-negative BC. The second step consolidated ALND omission in selected subgroups of BC patients with up to two macrometastases and recognized BC molecular and genomic implication in predicting prognosis and planning adjuvant treatment. Outcomes from the recent RxPONDER and monarchE trials have come to challenge the previous cut-off of two SLN in order to inform decisions on systemic ther

Bioprinting of anisotropic functional corneal stroma using mechanically robust multi-material bioink based on decellularized cornea matrix.

Corneal scarring is a common cause of blindness, affecting millions globally each year. A huge gap between the demand and supply of donor tissue currently limits corneal transplantation, the only definitive therapy for patients with corneal scarring. To overcome this challenge, researchers have harnessed the efficacy of 3D bioprinting to fabricate artificial corneal stromal constructs. With all the different bioinks available, the decellularized corneal matrix-based bioprinted construct can fulfill the required biological functionality but is limited by the lack of mechanical stiffness.
