Introduction: The conditions associated with pregnancy, such as preeclampsia, gestational diabetes, and disruptions of Placental lipid transport, are increasingly becoming known to be the cause of intrauterine growth restriction. Recent data indicate that compensatory efforts can initiate neuroinflammatory and systemic reactions in the mother, which can influence foetal development. Methods: This study aims to learn how maternal brain inflammation and neuroimmune activation during complicated pregnancies can be triggered by abnormal placental lipid carriage. Results: PBIC-SM and NLIP-IM models

Preeclampsia significantly contributes to maternal and perinatal morbidity and mortality, with twin pregnancies increasing this risk. Severe preeclampsia often requires pregnancy termination to protect maternal health. We report two cases of IVF-conceived twin pregnancies complicated by early preeclampsia. The first case involved a 34 year-old woman, gravida-2, abortion-1 at 21 weeks gestation with monochorionic twins, blood pressure of 150/80 mmHg, and proteinuria of 1600 mg. She underwent radiofrequency ablation of one twin at 22 weeks and delivered a healthy infant at 37 weeks. The second c…

BACKGROUND: Pregnancy in individuals with pulmonary hypertension associated with adult congenital heart disease carries substantial maternal and fetal risk and is classified as modified World Health Organization 2.0 class IV when pulmonary arterial hypertension (PAH) is present. When pregnancy is continued, management requires individualized shared decision-making and expert multidisciplinary planning.
CASE SUMMARY: We describe a case of a pregnant 24-year-old woman with complex congenital heart disease who presented with untreated PAH at initial cardio-obstetrics evaluation. The patient ulti

Cardiovascular (CV) disease is the leading cause of maternal mortality worldwide and has been partially linked to the cardiometabolic remodeling required to support fetal growth. In healthy pregnancies, these adaptations reverse postpartum without long-term consequences; however, impaired reverse remodeling (RR) increases future CV risk. Pregnant women living with HIV face additional risk, though it remains unclear whether this is driven by chronic low-level viremia or continuous antiretroviral therapy (ART). ART used during pregnancy often includes nucleoside reverse transcriptase inhibitors

Prenatal maternal stress (PNMS) determines lifetime mental and physical health. Here, we show in rats that PNMS has consequences for placental function and fetal brain development across four generations (F0-F3). Using a systems biology approach, comprehensive DNA methylation (DNAm), miRNA, and mRNA profiling revealed a moderate impact of PNMS in the F1 generation, but drastic changes in F2 and F3 generations, suggesting compounding effects of PNMS with each successive generation. Both maternal and placental miRNA gene targets included de novo DNA methyltransferases, indicating robust PNMS-ind

Vaccination during pregnancy is crucial due to increased maternal vulnerability to infectious diseases. However, uptake of recommended vaccines (influenza, pertussis, COVID-19) remains suboptimal, particularly among disadvantaged groups. This qualitative study explored healthcare professionals' (HCPs) perspectives, selected purposively, on factors influencing maternal vaccination in London. Data from a workshop transcript and an online quality improvement survey involving 15 GPs, two midwives, two obstetricians and one pharmacist were thematically analysed. HCPs highlighted that pregnant women

Breastfeeding is crucial for infant survival, growth, and health, and it enhances maternal-infant bonding and well-being. However, breastfeeding rates typically fall below international targets, partly due to a high prevalence of latching difficulties, intermittent sucking, refusing the breast, or poor milk supply. Here, we propose that such uniquely human difficulties might be ameliorated by recognising, understanding, and facilitating the olfactory mechanisms that, in other mammals, regulate breastfeeding initiation and maternal-infant relationships in the first weeks of life. We briefly rev

Bone mineral density (BMD) is crucial for bone strength, with even a modest decrease significantly elevating fracture risk. Calcium imbalance during pregnancy contributes to BMD loss, as maternal stores are mobilized to support fetal skeletal development. Vitamin D deficiency exacerbates this issue, with prevalence rates alarming in various populations. Supplementation with vitamin D and calcium aims to mitigate BMD decline; however, optimal dosing and efficacy remain debated. Studies utilizing innovative diagnostic tools like radiofrequency echographic multi spectrometry (REMS) and quantitati

Widespread SARS-CoV-2 infection among pregnant individuals has led to a generation of fetuses exposed in utero, but the long-term impact of such exposure remains unknown. Although fetal infection is rare, children born to mothers with SARS-CoV-2 infection may be at increased risk for adverse neurodevelopmental and cardiometabolic outcomes. Fetal programming effects are likely to be mediated at least in part by maternal immune activation. In this review, we discuss recent evidence regarding the effects of prenatal SARS-CoV-2 infection on the maternal, placental, and fetal immune response, as we

The conserved CD94/NKG2A inhibitory receptor is expressed by nearly all human and ∼50% of mouse uterine natural killer (uNK) cells. Binding human HLA-E and mouse Qa-1, NKG2A drives NK cell education, a process of unknown physiological importance influenced by HLA-B alleles. Here, we show that NKG2A genetic ablation in dams mated with wild-type males caused suboptimal maternal vascular responses in pregnancy, accompanied by perturbed placental gene expression, reduced fetal weight, greater rates of smaller fetuses with asymmetric growth, and abnormal brain development. These are features of the

Sickle cell disease (SCD) is the most common inherited hemoglobinopathy and is associated with increased risk of complications and early mortality. Nowadays, with improved health care facilities, antibiotic prophylaxis, vaccination, and availability of drugs like hydroxyurea, the life expectancy of SCD patients has improved. More women are reaching reproductive age group and are expressing their desire to reproduce. Though SCD adversely affects pregnancy, leading to increased incidence of maternal and perinatal complications like pre-eclampsia, preterm labor, IUGR, abortions etc., adequate car

Folate is vital for fetal development. Periconceptional folic acid supplementation and food fortification are recommended to prevent neural tube defects. Mechanisms whereby periconceptional folate influences normal development and disease are poorly understood: epigenetics may be involved. We examine the association between maternal plasma folate during pregnancy and epigenome-wide DNA methylation using Illumina's HumanMethyl450 Beadchip in 1,988 newborns from two European cohorts. Here we report the combined covariate-adjusted results using meta-analysis and employ pathway and gene expression

2-methoxyestradiol (2ME2) is a potent antiangiogenic molecule that inhibits the expression of hypoxia-inducible factor (HIF)-1alpha and, consequently, of VEGF and other HIF-1alpha target genes. Although 2ME2 is elevated during pregnancy in maternal serum, its presence in fetal fluids and its impact in neonatal health are unknown. In this study, we 1) described normal levels of 2ME2 in maternal blood, cord blood, breast milk, and amniotic fluid, and 2) compared a composite measure of perinatal outcome between infants born with high and low levels of 2ME2. We found that 2ME2 was significantly de
