Maternal and neonatal outcomes with the use of long acting, compared to intermediate acting basal insulin (NPH) for managing diabetes during pregnancy: a systematic review and meta-analysis.

BACKGROUND: To assess the impact of long-acting insulin analogues, compared to intermediate acting neutral protamine Hagedron (NPH), on maternal, perinatal and neonatal outcomes. METHODS: Studies for inclusion in the review were identified using a structured search strategy in PubMed, Scopus and Cochrane Central Register of Controlled Trials (CENTRAL) database. Studies that were randomized controlled trials or observational in design were considered for inclusion. Eligible studies should have compared the maternal, perinatal and neonatal outcomes between pregnant women with gestational diabet

Prenatal exposure to nitrate from drinking water and the risk of preterm birth: A Danish nationwide cohort study.

UNLABELLED: Evidence is emerging that preterm birth (PTB, birth before 37 completed weeks of gestation), a risk factor for neonatal mortality and future morbidity, may be induced by maternal nitrate ( N O 3 - ) exposure from drinking water. The objective of this study is to assess the association between maternal exposure to nitrate and the risk of PTB in a nationwide study of liveborn singletons. METHODS: We estimated maternal nitrate exposure from household tap water for 1,055,584 births in Denmark to Danish-born parents during 1991-2015 by linkage of individual home address(es) with n

NAFTNet retrospective report on the treatment of anti-Ro/SSA mediated fetal heart block with dexamethasone.

BACKGROUND: Complete atrioventricular block (CAVB) is a complication of maternal antibody positivity and treatment of fetal disease is controversial in terms of efficacy and safety. We hypothesized that dexamethasone treatment for fetal anti-Ro/SSA antibody-mediated cardiac disease leads to better pregnancy outcomes than expectant management. METHODS: A retrospective multi-center cohort study of anti-Ro/SSA antibody positive pregnancies with fetal conduction disease reported by participating North American Fetal Therapy Network (NAFTNet) centers between January 2010 and December 2018. The pri

Wildfire exposure during pregnancy and the risk of adverse birth outcomes: A systematic review.

BACKGROUND: Maternal wildfire exposure (e.g., smoke, stress) has been associated with poor birth outcomes with effects potentially mediated through air pollution and psychosocial stress. Despite the recent hike in the intensity and frequency of wildfires in some regions of the world, a critical appraisal of the evidence on the association between maternal wildfire exposure and adverse birth outcomes has not yet been undertaken. We conducted a systematic review that evaluated the scientific evidence on the association between wildfire exposure during pregnancy and the risk of adverse birth outc

Neonatal and Maternal Adverse Outcomes Among Low-Risk Parous Women at 39-41 Weeks of Gestation.

OBJECTIVE: To compare the composite neonatal or maternal adverse outcome among low-risk, parous women at 39-41 weeks of gestation. METHODS: This was a retrospective cohort study using the U.S. vital statistics data sets (2012-2016). We evaluated low-risk parous women with nonanomalous singleton gestations who delivered at 39, 40, or 41 weeks of gestation (as reported in completed weeks, eg, 39 weeks includes 39 0/7-39 6/7 weeks of gestation). The primary outcome, the composite neonatal adverse outcome, included any of the following: Apgar score less than 5 at 5 minutes, assisted ventilation f

Sleep duration, vital exhaustion, and odds of spontaneous preterm birth: a case-control study.

BACKGROUND: Preterm birth is a leading cause of perinatal morbidity and mortality worldwide, resulting in a pressing need to identify risk factors leading to effective interventions. Limited evidence suggests potential relationships between maternal sleep or vital exhaustion and preterm birth, yet the literature is generally inconclusive. METHODS: We examined the relationship between maternal sleep duration and vital exhaustion in the first six months of pregnancy and spontaneous (non-medically indicated) preterm birth among 479 Peruvian women who delivered a preterm singleton infant (<37 wee

Effect of mass deworming with antihelminthics for soil-transmitted helminths during pregnancy.

BACKGROUND: Helminthiasis is an infestation of the human body with parasitic worms. It is estimated to affect 44 million pregnancies, globally, each year. Intestinal helminthiasis (hookworm infestation) is associated with blood loss and decreased supply of nutrients for erythropoiesis, resulting in iron-deficiency anaemia. Over 50% of the pregnant women in low- and middle-income countries (LMIC) suffer from iron-deficiency anaemia. Though iron-deficiency anaemia is multifactorial, hookworm infestation is a major contributory cause in women of reproductive age in endemic areas. Antihelminthics

Nutritional management of the low birth weight/preterm infant in community settings: a perspective from the developing world.

Globally, about 20 million infants are born with low birth weight (LBW; <2500 g). Of all LBW infants, approximately 95% are born in developing countries. The greatest incidence of LBW occurs in South-Central Asia; the second greatest is in Africa. The two main reasons for LBW are preterm birth (<37 weeks) and intrauterine growth restriction (IUGR), which are risk factors for increased morbidity and mortality in newborn infants. Maternal nutrition status is one of the most important risk factors for LBW/IUGR. Providing balanced protein energy and multiple micronutrient supplements to pregnant w

[Meningococcal disease and VA-MENGOC BC in minors less than 1 year of age. Cuba, 1983 to 1991].

A study of the chronological series of mortality due to meningococcal disease in children under one year old, the group of highest incidence during the last epidemy in Cuba, was carried out. Data were collected by doing a survey in an uniform way since 1983 throughout the country. More than 90% of the population between 3 months and 5 years old were vaccinated with VAMENGOC BC since the end of 1988 until April, 1990. The behaviour of this disease was studied in order to identify the influence of this vaccine. It is stressed that the mortality incidence reached its epidemic achme in 1986 and be

Causes of perinatal death at a Saudi maternity hospital.

The time, cause and avoidability of perinatal deaths were analyzed in infants who were delivered in the main Maternity hospital in Riyadh and who represented a large proportion of all births in the city. The crude perinatal mortality rate was 39.8 per 1000 births. Of all deaths 53 percent occurred either intrapartum or within 24 hours of birth. The mortality rate in this time period was 9 times higher, and the intrapartum mortality rate 16 times higher than the corresponding Swedish rates. The causes of death were classified according to Wigglesworth. Of all deaths, 37 percent were due to asph

Iron Deficiency and Iron Deficiency Anemia in Pregnancy: A Narrative Review for Canadian Obstetrical Practice.

Iron deficiency is the most common medical condition encountered during pregnancy and remains substantially underdiagnosed when screening relies on hemoglobin alone. Even in high-income countries such as Canada, iron deficiency affects most pregnant women. Iron deficiency and iron deficiency anemia are associated with increased maternal, perinatal, and neonatal morbidity and mortality, including adverse neurodevelopmental outcomes. This narrative review summarizes the epidemiology, physiology, clinical consequences, screening strategies, and management of iron deficiency in pregnancy, with a f

Dengue and scrub typhus coinfection in pregnancy: A rare case with multiorgan dysfunction.

Tropical infections continue to cause significant morbidity and mortality in India. While mixed tropical infections are relatively common in the general population, they are rare during pregnancy. We report a 24-year-old pregnant woman with a history of previous lower segment caesarean section and preeclampsia, who developed dengue and scrub typhus infection near-term. The illness was complicated by multiple organ dysfunction syndrome, yet with timely multidisciplinary management, both maternal and neonatal outcomes were favourable. Literature on such mixed tropical infections in pregnancy rem

Obstetric sepsis: Clinical pearls for diagnosis and management.

Sepsis is a leading cause of maternal morbidity and mortality worldwide. Early recognition and treatment improve outcomes. Multiple sepsis diagnostic screening tools are available and may be used in clinical practice; however, early thorough bedside evaluation of the patient is fundamental. Obstetricians should be able to recognize sepsis and promptly initiate potentially life-saving treatments, such as fluid resuscitation, vasopressors, broad-spectrum antibiotics, and early source control. It is recommended that obstetrical care, including delivery timing, not be altered solely due to the dia

Hypercalcaemia due to parathyroid carcinoma presenting in the third trimester of pregnancy.

Primary hyperparathyroidism (pHPT) in pregnancy may be associated with significant maternal and fetal morbidity and mortality. Medical management of pHPT in pregnancy is limited, and surgery is the only definitive therapeutic option. The ideal timing for surgery is mid-second trimester, but surgery may also be safely performed in the third trimester. Delayed parathyroid surgery may result in a hypercalcaemic crisis postpartum owing to loss of active placental calcium transfer. We present a case of parathyroid carcinoma in pregnancy presenting with pre-eclampsia at 32 weeks' gestation.

Abruption-associated prematurity.

Chronic, subacute decidual hemorrhage (ie, abruptio placenta and retrochorionic hematoma formation) is an important contributor to preterm parturition. Such hemorrhage induces thrombin from decidual tissue factor, which plays a pivotal role in the development of preterm premature rupture of membranes and preterm delivery by acting through protease-activated receptors to promote the production of pro-inflammatory cytokines, and matrix-degrading metalloproteinases. Severe, acute abruption can lead to maternal and fetal mortality. Current management of abruption is individualized based on severit

Renal hematoma: an unusual cause of acute abdomen and fetal demise in severe pre-eclampsia.

Retroperitoneal hematoma occurs rarely in an obstetric patient. Renal hematoma may present with signs and symptoms, which may mimic the clinical presentation of abruptio placentae or rupture uterus. Although renal hematoma is sometimes a surgical emergency due to hypovolemic shock, conservative management by angiographic embolization may be done. Timely diagnosis and management is required to decrease the maternal mortality and morbidity. We hereby report a case of spontaneous renal hematoma in a patient with severe pre-eclampsia who presented with acute abdomen and intrauterine fetal death.

[Second-degree burn in a pregnant woman].

The incidence of burn injuries in pregnancy is low. Nevertheless, burns can cause maternal and foetal mortality and should be treated aggressively if substantial. Minor burns may frequently be managed without hospitalization. A 27-year-old primigravid woman with a gestational age of 38+6 presented with an abdominal partial-thickness burn covering 3% of her total body surface area. Conservative treatment was conducted in the emergency department. Medical induction of labour was performed. A healthy boy was delivered vaginally within 36 hours after the injury.
