Exploring Tai Chi in rheumatoid arthritis: a quantitative and qualitative study.

BACKGROUND: Rheumatoid arthritis (RA) is a chronic, inflammatory and systemic disease which affects the musculoskeletal system. Exercise programmes are reported to improve physical functioning in patients with RA. Tai Chi is a traditional Chinese martial art which combines slow and gentle movements with mental focus. The purpose of this study was to study in which way Tai Chi group exercise impacted on disease activity, physical function, health status and experience in RA patients, applying quantitative and qualitative methods. METHODS: Fifteen patients with RA (13 females, age 33-70 years) …

Perioperative Anesthetic Considerations for Temporal Bone Tumor Resection in Fibroblast Growth Factor 23 (FGF23)-Related Tumor-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumors (PMTs) are uncommon, predominantly benign neoplasms frequently associated with dysregulated phosphate metabolism through the overproduction of fibroblast growth factor 23 (FGF23). Elevated levels of FGF23 lead to phosphate depletion, which may impair tissue oxygen delivery, central respiratory drive, and respiratory muscle function. Systemic manifestations may include osteomalacia and neuromuscular dysfunction, with an increased risk of respiratory and cardiovascular complications. PMTs often present with nonspecific clinical findings and may remain unrecognized

Green waters, red alert: ecologically relevant eutrophic nitrate concentrations cause sublethal effects on crayfish food-searching and antipredator behaviour.

Eutrophication, often caused by excess nitrogen, currently affects over 40% of water bodies globally. However, little is known about nitrate's sublethal toxicity on species considered hardy against this stressor, like crayfish. Pollutants can alter behaviour through long-term physiological changes or after short exposures by interfering with stimuli detection. To investigate whether nitrates affect crayfish behaviour, two experiments were conducted whereby crayfish were exposed to ecologically relevant moderate (50 mg NO3-/L) and high (150 mg NO3-/L) nitrate concentrations for 2 weeks or 30 mi

Clinical perspectives on the virtual human twin: an overview of engagement and adoption challenges.

Digital twins (DTs) in healthcare have the potential to transform patient care, but their adoption depends critically on the perspectives of clinical professionals, pa-tients, and the public. As part of the Ecosystem for Digital Twins in Healthcare (EDITH) initiative, we undertook a programme of targeted engagement to under-stand, explore and integrate these perspectives into the development of the Euro-pean Virtual Human Twin (VHT). Methods included surveys, breakout sessions at public ecosystem meetings, and three dedicated workshops with clinicians and pa-tient representatives covering card

Therapeutic potential of adipose-derived stem cells for noncommunicable diseases.

PURPOSE: The progressive increase in life expectancy has been accompanied by a growing burden of chronic degenerative noncommunicable diseases (NCDs), with major consequences for quality of life and healthcare systems. This scenario underscores the need for innovative therapeutic strategies aimed at preserving organ function and metabolic homeostasis. Adipose-derived stem/stromal cells (ASCs) have emerged as a promising tool in regenerative medicine due to their multipotency, immunomodulatory properties, and paracrine activity. METHODS: We reviewed available preclinical and clinical evidence s

Curcumin Between Pleiotropic Potential and Translational Constraints.

Curcumin is widely recognized for its anti-inflammatory and antioxidant properties; however, this conventional framing obscures a broader, complex, and mechanistically diverse pharmacology. Here, we advance a refined perspective that situates curcumin within a hierarchical and multilayered architecture shaped by the dynamic interplay of intrinsic chemical reactivity, metabolic transformation and exposure, and microbial modulation. From this standpoint, curcumin functions as a network-level modulator, producing context-dependent outcomes rather than uniform or linear responses. Consequently, it

Extracellular traps from myeloid cells as therapeutic targets in traumatic heterotopic ossification.

After musculoskeletal injury, a considerable proportion of patients develop heterotopic ossification (HO), the formation of bone at ectopic sites. Traumatic HO is a disabling condition, potentially resulting in loss of joint function and compression of neurovascular structures. Available treatment options are often unsuccessful, frequently necessitating surgical resection of HO lesions with a high risk of recurrence. Given that myeloid cells, including neutrophils and macrophages, are among the first cell types to infiltrate injured tissue, the present study explored the relationship of extrac

Epigenetic Mechanisms of Vitamin D in the Aging Process: A Narrative Review.

Aging is driven by progressive epigenetic alterations-DNA methylation drift, aberrant histone modifications, chromatin remodeling, and non-coding RNA dysregulation. Vitamin D, acting through its nuclear receptor vitamin D receptor (VDR), modulates the epigenetic landscape to potentially counteract these age-related changes. This review first describes age-related epigenetic alterations, then outlines vitamin D signaling and its interface with the epigenetic machinery. Next, tissue-specific epigenetic actions of vitamin D in the immune, musculoskeletal, and nervous systems are discussed. Finall

Clinical Outcomes and Adverse Events of Continuous Local Antibiotic Perfusion for Orthopedic Infections: A Multi-Category Retrospective Study.

INTRODUCTION: Bone and soft-tissue infections, including fracture-related infection (FRI), periprosthetic joint infection (PJI), and soft-tissue infection (STI), remain a significant challenge in orthopedic surgery. Continuous local antibiotic perfusion (CLAP) offers a promising local treatment strategy to maintain high antibiotic concentrations; however, robust real-world data obtained across different infection categories are limited. MATERIALS AND METHODS: This retrospective, multicenter study included 155 patients (FRI, n = 72; PJI, n = 22; STI, n = 61) treated with CLAP at tertiary emerg

Clinical manifestations of chromosome 19p13.11 duplication.

BACKGROUND: Chromosome 19 is the most gene-dense chromosome in the human genome, with a high frequency of segmental duplications that predispose it to genomic rearrangements. While deletions of chromosome 19 have been associated with various clinical conditions, duplications remain poorly characterised. Here, we report three cases involving 19p13.11 duplication and describe the associated clinical phenotype. METHODS: We describe three unrelated individuals with microduplications at 19p13.11 identified either via clinical whole-exome sequencing or chromosomal microarray. The probands underwent

Hyperglycemia enhances Chikungunya virus-induced tissue damage: Histopathological evidence from a murine model.

Chikungunya fever (CHIKF) is a re-emerging viral disease characterized by acute systemic manifestations and debilitating musculoskeletal symptoms that can persist after viral clearance. Although typically self-limiting in healthy individuals, clinical outcomes are significantly worsened in patients with pre-existing comorbidities, particularly diabetes mellitus (DM). Epidemiological data links DM to heightened CHIKF severity and a greater risk of developing chronic arthropathy, yet the mechanism underpinning this association remains poorly understood. In this study, we established an in vivo s

Wearable Sensors and Artificial Intelligence for Ecological Knee Osteoarthritis Assessment: Development and Feasibility of a Hybrid Digital Phenotyping Framework.

Osteoarthritis (OA) is a highly prevalent musculoskeletal disorder and a major cause of disability, posing growing challenges for healthcare systems worldwide. Conventional supervised clinical assessments provide valuable insights but are largely limited to cross-sectional snapshots and often fail to reflect the variability of real-world functioning, physical activity patterns, and symptom fluctuations experienced by individuals with OA, especially those with knee OA. This perspective introduces a multisensor digital phenotyping framework for smart knee OA assessment, integrating supervised la

Mental health of caregivers for people with dementia and cerebral palsy as a key determinant of caregiver burden: a multivariable analysis.

BACKGROUND: Informal caregiving plays a vital role in supporting dependent individuals; however, prolonged caregiving is associated with significant physical and psychological strain. Understanding factors associated with caregiver burden is essential for designing effective interventions to protect caregiver health and sustain long-term care systems. OBJECTIVE: To examine the associations between psychological, physical, and contextual factors on caregiver burden and to identify variables independently associated with caregiver burden. METHODS: A cross-sectional study was conducted with 73

Gut dysbiosis and vitamin-dependent immune regulation in degenerative musculoskeletal and bone diseases.

Degenerative musculoskeletal and metabolic bone diseases are increasingly recognized as conditions sustained not only by endocrine and mechanical factors, but also by chronic low-grade immune activation and osteo-immune imbalance. This Perspective proposes a mechanistic framework in which gut dysbiosis may contribute to skeletal degeneration through alterations in vitamin-dependent immune regulation, with particular attention to the interaction between vitamin D signaling and microbiota-derived menaquinones. Dysbiosis may impair intestinal barrier integrity and increase exposure to microbial-a

Patient awareness and perception of cardiovascular risk in rheumatic and musculoskeletal diseases: results from an international survey.

OBJECTIVES: Inflammatory rheumatic and musculoskeletal diseases (RMDs) are hallmarked by an increased risk of cardiovascular (CV) disease compared with the general population. Evidence suggests that CV risk remains under-recognised and underdiscussed in routine care, although data on patient experience is lacking. We aimed to assess awareness, perceptions, and experiences regarding CV risk and prevention in patients with RMDs. METHODS: An anonymous, online survey was codesigned by rheumatologists, researchers, and patient research partners in English language and translated into 9 languages.
