Mandibular Condylar Fractures and Occlusal Management.

Mandibular condyle fractures are among the most common mandibular injuries, and management is a subject of debate. The most common classification systems are highlighted. Algorithm-based clinical decision making may help practitioners in their management. This review focuses on the history and management strategies, including non-surgical, closed treatment, and open reduction and internal fixation (ORIF). ORIF has been increasing in favor over the past decade owing to reduced complications compared with nonsurgical treatment. The surgical approaches and plating techniques have continued to evo

Management of fractures of the talus: body and head regions.

Talar head and body injuries are not recognized easily and can create significant long-term disability when missed. Careful investigation of any injury about the ankle requires clinical and radiographic examination. A CT scan is extremely helpful in diagnosing and treating these injuries. Displaced fractures require open reduction of the major joint surfaces and internal fixation. Prolonged nonweight bearing and immobilization is the norm. Despite aggressive management, complications that involve avascular necrosis and posttraumatic arthritis to the subtalar and ankle joints occur frequently.

[Intramedullary nailing of distal mid-hand fractures--technique, instruments, case reports].

Dislocation and instability of distal metacarpal fractures necessitate open reduction and stable fixation. Several methods of internal fixation have been described, but all of them are technically difficult. Intramedullary pinning from proximal to distal, first described by Foucher in 1976, is a dynamically stable operation method and easy to perform. New instruments and the newly developed pin have been constructed and adapted to the operation method. All is kept as technically straightforward as possible. Demonstration of the handling and clinical cases are described, and advantages of the p

Pathologic fracture of the distal radius in a 25-year-old patient with a large unicameral bone cyst.

BACKGROUND: Distal radius fractures (DRF) are often referred to as osteoporosis indicator fractures as their incidence increases from age 45. In the group of young adults, distal radius fractures normally result from high-energy trauma. Wrist fractures in young patients without adequate trauma thus raise suspicion of a pathologic fracture. In this report we present the case of a fractured unicameral bone cyst (UBC) at the distal radius in a young adult. To the author's best knowledge, this is the first detailed report in an UBC at the distal radius causing a pathologic DRF in an adult patient.

Surgically treated acetabular fractures in adult patients.

In this retrospective study, 27 acetabular fractures treated by open reduction and internal fixation with an average follow-up of 3.7 years (1.5-4.0) are reported. Reduction leaving a displacement of < or = to 2 mm was achieved in 9 hips (30%). Good or excellent results were obtained in 22 hips (82%), and in 2 patients who were referred to us after 3 weeks, we performed supra-acetabular osteotomy to obtain congruent joints with fairly good results. The most common complication was heterotopic ossification in 59% of the patients, which led to poor results in 2 patients with grade IV. Avascular

Surgical Management of a Periprosthetic Femoral Fracture Above a Total Knee Arthroplasty: A Case Report.

Periprosthetic distal femur fractures above total knee arthroplasty (TKA) are challenging due to compromised bone quality and the need to preserve the prosthesis. With increasing TKA volumes, the incidence of these injuries is rising. A 78-year-old female patient presented with left thigh pain and inability to bear weight following a fall. Imaging revealed a comminuted distal femur fracture proximal to a well-fixed TKA. Open reduction and internal fixation was performed using the AxSOS 3 Ti Distal Lateral Femur Plating System (Stryker, Portage, MI) via a lateral approach. Fixation was achieved

Fatal Duodenal Ulcer Hemorrhage in a Post-Operative Patient of Proximal Femur Fracture Undergoing Carbon Ion Radiotherapy for Locally Advanced Pancreatic Carcinoma: A Case Report.

The incidence and mortality rates of pancreatic cancer have been increasing. Due to the high likelihood of metastasis and vascular invasion, most pancreatic cancer cases are inoperable at the time of diagnosis. Carbon-ion radiotherapy (CIRT) has recently been introduced as a treatment option for locally advanced pancreatic cancer (LAPC). However, potential complications, including peptic ulcer disease (PUD), have been noted. This report presents a case of death due to PUD following a proximal femur fracture, with a history of CIRT for unresectable LAPC. A 59-year-old female undergoing CIRT for

Traumatic bilateral lumbosacral facet dislocation without fracture: illustrative case.

BACKGROUND: Traumatic bilateral lumbosacral facet dislocations without fractures are extremely rare. Only 7 cases have been documented since the first description by Watson-Jones in 1974. Although various treatment strategies have been reported, no consensus has been reached regarding the best surgical approach. OBSERVATIONS: A 35-year-old female presented for medical attention following a high-speed motor vehicle collision. She sustained multiple injuries, including an abdominal aortic injury requiring emergent thoracic endovascular aortic repair. She was found to have bilateral lumbosacral

Recurrent patellar dislocation after medial patellofemoral ligament reconstruction.

We report on three cases of recurrent lateral patellar dislocation following a medial patellofemoral ligament (MPFL) reconstruction for patellar instability. In all three cases, an isolated MPFL reconstruction was performed with a double autogenous gracilis graft. The patellar fixation was done through bone tunnels. All three patients presented with a definite moderate to severe traumatic episode resulting in a recurrent patella dislocation and a transverse avulsion fracture at the medial rim of the patella. All three were treated by an open reduction and internal fixation with good results. N

Intraoperative 3D imaging of the facial skeleton using the SIREMOBIL Iso-C3D.

OBJECTIVES: With respect to high-contrast structures, cone beam computed tomography (CBCT) offers an alternative imaging modality to computed tomography (CT), requiring less radiation exposure. The C-arm system SIREMOBIL Iso-C3D for three-dimensional (3D) reconstruction has made this modality available for intraoperative use. This paper presents the first intraoperative images of the facial skeleton using the SIREMOBIL Iso-C3D. METHODS: Cases of a zygomaticomaxillary complex fracture, a mandibular angle fracture and a bimaxillary repositioning osteotomy are described to demonstrate the applic

Soft tissue coverage options for ankle wounds.

Soft tissue deficiencies of the ankle are caused by several mechanisms, such as trauma, tumor, and infection. Compounding the reconstructive problems is that soft tissue problems often present in patients who have underlying diseases such as peripheral vascular disease, diabetes or both. For example, a 65-year-old person with diabetes who smokes two packs of cigarettes per day sustains an ankle fracture. After undergoing open reduction and internal fixation of the fracture, there is subsequent wound behiscence over the patient's fibular plate. The wound edges cannot be reapproximated, and ther

[The osteosynthesis implant and early postoperative infection: healing with or without removal of the material?].

This prospective study served as a quality control of a revision concept for case of post-traumatic infection following open reduction and internal fixation in fracture treatment. It is based on clinical and microbiological criteria and has two aims: (1) eradication of the infection and avoidance of development of chronic osteitis; (2) maintenance of internal fixation, if possible. Thirty-four patients were recruited in this study. Surgical revisions were performed according to a consistent concept (debridement, irrigation, local chemotherapy, drainage) in defined time intervals (2 days). The

Fracture separation of the olecranon ossification center in adults.

Traumatic disruption of an incompletely fused olecranon physis in adults has been reported on only one occasion by O'Donoghue in 1942, while stress fractures of the olecranon physis have been well-recognized and reported. A series of three adult males with traumatic disruption of an incompletely fused olecranon physis was studied. Two were competing in football at the time of the injury, while a third patient sustained the injury during a fall. All of our patients were former throwing athletes and all sustained a direct blow to the dominant elbow at the time of the fracture. In our study, as w

Retrospective analysis of external pin fixation of mandibular fractures: A 25-year single institution experience.

External fixation of mandible fractures is an infrequently utilized fixation treatment modality in comparison to open reduction internal fixation (ORIF) or closed reduction techniques. However, external fixation still provides a necessary modality in the management of select mandibular fractures. Therefore, we aim to draw trends, outcomes, and treatment timelines through a retrospective analysis to better guide, advise and predict realistic treatment outcomes in the future for our patients. This abstract shows trends in indications, complications, airway management and timeline to definitive r
