Application of palatal plate for nonextraction treatment in an adolescent boy with severe overjet.

A modified C-palatal plate (MCPP) is introduced as a treatment option for adolescent patients with Class I malocclusion and severe overjet. A boy, 10 years 11 months old, was successfully treated without extractions in 22 months. Indications for clinical application of the MCPP as well as procedures and biomechanical analysis of the treatment effects are described in detail. The MCPP was used to distalize the maxillary dentition. The results were stable 1 year after retention. Clinicians should consider the application of MCPP as a nonextraction treatment option for adolescents with Class I ma

Forced eruption of a palatally impacted and transposed canine with a temporary skeletal anchorage device.

Treatment of palatally impacted and transposed canines is challenging for both orthodontists and oral surgeons. To treat a maxillary canine impaction without risking damage to adjacent teeth, it is necessary to use cone-beam computed tomography for proper localization and treatment planning. In this case report, a palatally impacted canine was initially retracted distally with a microimplant inserted in the palatal slope and then was moved buccally into its ideal position. The patient's occlusion and smile esthetics were significantly improved after orthodontic treatment.

Class I malocclusion with severe bimaxillary protrusion: a successful result.

This case report describes the successful treatment of a 29-year-old young woman who had severe bimaxillary dento-alveolar protrusion, with the four premolars already extracted. Her face was convex, with a closed naso-labial angle, protruding and everted lower lip, associated with mentalis strain and a class I canine and molar relationships. After retraction of anterior teeth by using maximum anchorage mechanics through directional forces, facial changes were impressive, with a significant improvement of the dento-alveolar protrusion. Retraction of the incisors had corrected proquelia, reduced

Congenitally missing maxillary lateral incisor treated with atypical extraction pattern.

The congenital absence of one or more maxillary lateral incisors poses a challenge to effective treatment planning for general dentists and dental specialists. An evaluation of anterior smile esthetics must include both static and dynamic evaluations of frontal and profile views to optimize both dental and facial appearance. This article presents a case with canine substitution treatment to replace a missing maxillary lateral incisor combined with the extraction of two mandibular lateral incisors and a small maxillary lateral incisor Both the patient's occlusion and facial appearance were sign

The dilemma of the open gingival embrasure between maxillary central incisors.

AIM: The aim of this report is to present the etiology, diagnosis, and treatment planning strategy in the presence of an open gingival embrasure between the maxillary central incisors. BACKGROUND: The open gingival embrasure or "black triangle" is a visible triangular space in the cervical region of the maxillary incisors. It appears when the gingival papilla does not completely fill in the interdental space. The space may occur due to: (1) disease or surgery with periodontal attachment loss resulting in gingival recession; (2) severely malaligned maxillary incisors; (3) divergent roots; or (

Treatment of unilateral buccal crossbites in the primary, early mixed, and permanent dentitions: case reports.

It is important to recognize the development of an arch-width problem in pediatric patients and to determine the best time to treat it. One of these conditions is the posterior (buccal) crossbite where one may also find an exaggerated overjet, caused by maxillary excess, and/or mandibular width deficiency. One may also find a mandibular midline deviation on the side of the crossbite, creating a long-term orthopedic problem with a mild facial asymmetry. When correction of this condition is attempted in adulthood a poor results can be expected making this the most important reason for early trea

A modified monobloc for the treatment of obstructive sleep apnoea in paediatric patients.

The aims of this study were to determine the differences in craniofacial morphology between children with obstructive sleep apnoea (OSA) and control subjects, and to investigate the effects of modified monobloc (MM) appliance treatment in OSA children. The treatment group consisted of 20 OSA Caucasian subjects (10 boys and 10 girls) with ages ranging from 4 to 8 years (mean 5.91 years). The control group comprised 20 healthy Caucasian subjects without OSA (10 boys and 10 girls) with ages ranging from 5 to 7 years (mean 6 years). Polysomnography was used to establish the diagnosis of OSA and to

Intraoral distraction osteogenesis to lengthen the ascending ramus. Experience with seven patients.

Seven children with facial asymmetry, mean age 12 years (range 11-14.5) were treated by intraoral distraction osteogenesis to lengthen the hypoplastic ramus. We achieved a mean increase in length of the ramus of 13mm (range 10-16). In only one patient did we achieve a posterior open bite on the distraction side. All patients ended with a symmetrical chin. It was helpful to place an orthodontic bite block on the opposite side either preoperatively or postoperatively to cant the plane of occlusion. The duration of follow-up was too short to allow conclusions to be drawn about the future requirem

Friction in perspective.

In the past, most frictional resistance studies have been conducted in a steady state condition that does not simulate the dynamics of the oral environment. Various oral functions as chewing, swallowing, speaking, etc, as well as the oral tissues contacting any orthodontic appliances, result in periodic, repetitive, minute relative motion at the bracket/arch wire interfaces several thousand times each day. This in turn affects the normal forces at the interfaces, and because frictional resistance is directly proportional to the normal force, a pilot study was undertaken to emulate the dynamic

Increase of vertical dimension--consequences for the maxillomandibular relationship. A clinical approach.

The purpose of the present study was to compare the vertical change in the incisor and molar areas caused by insertion of a bite opening appliance and to analyse the occurrence of rotation and eruption during the following treatment period (mean 4.3 months). Eight patients between 15 and 24 years of age with vertical overlaps > or = 4 mm were treated by means of a maxillary fixed lingual arch appliance with an anterior bite plane. The retruded mandibular position was recorded with cold cure acrylic before treatment, immediately after insertion of the fixed appliance, and after removal of it. V

[The bonding strength of silane-coated metal brackets with different adhesives].

The aim of this study was to test under thermo stress the effect of various adhesives on the bonding strength of brackets with a new retention mechanism in comparison to brackets with a conventional retentive basis. In the test 17 different adhesives and metal brackets with two different retention mechanisms were used. One group of metal brackets was coated by means of the Kulzer silane coating procedure and the other had a mesh basis. The latter served as the control group. Both were held in place by adhesives and their bonding strength tested. The following conclusions were drawn: 1. Both of

External Root Resorption: Unique Cone Beam Computed Tomography Findings of a Rare Radiographic Entity.

External root resorption occurs as a physiological or pathological process following pulpal necrosis. It ensues due to a traumatic insult to the pulp from underlying mechanical and chemical stimuli. The mechanical stimuli that favour external resorption include excessive orthodontic forces generated during orthodontic tooth movement, abnormal occlusal forces of mastication caused by traumatic occlusion, pressure effects caused by underlying impacted teeth, and odontogenic cysts and tumours. External resorption can also result from chemical stimuli such as bleaching, extruded filling materials,

Correction of Supernumerary Tooth-Associated Anterior Crossbite in Mixed Dentition Using a 2 × 4 Appliance.

Anterior crossbite is a common tooth alignment discrepancy seen among the paediatric population especially during the mixed dentition stage. There are many causes of anterior crossbite, and the presence of supernumerary tooth/teeth is one of them. Early interceptive intervention is essential to avoid the need for more complex orthodontic treatment later. This case report describes the management of a 10-year-old child who presented with anterior crossbite involving Teeth 21 and 31, leading to gingival recession of Tooth 31. The crossbite malocclusion was associated with a supernumerary tooth i

The application of retromolar mini-screws for distalization in preparation for orthognathic surgery: A case series.

Combined orthodontic-surgical treatment is often chosen by clinicians and patients to enhance both skeletal and occlusal relations in cases of moderate to severe jaw discrepancies. Presurgical orthodontic preparation is crucial for achieving arch coordination and eliminating dentoalveolar compensation. Three cases with varying dental and skeletal characteristics are presented, one with protruded mandible and Class III relationship, the second with maxillary vertical excess and retruded mandible, and the third with retruded mandible. Distalization using retromolar skeletal anchorage for molar d

Management of Impacted Premolar With Root Dilaceration Caused by Radicular Cyst in Deciduous Molar: A Case Report.

Radicular cyst is the most common inflammatory odontogenic cyst in the oral cavity, but it rarely occurs in deciduous teeth. Radicular cysts in deciduous teeth can lead to root dilaceration in permanent successor teeth, resulting in impacted permanent teeth. We presented an 11-year-old boy who underwent decompression and drainage of a radicular cyst in deciduous tooth, followed by early orthodontic traction of permanent successor tooth with root dilaceration. The cyst healed successfully, and the impacted premolar was gradually aligned into the dental arch with favorable root development This
