Chronic decubitus ulceration of the heels is a common condition encountered by podiatric physicians, especially in diabetic patients. Very often these ulcerations can progress to osteomyelitis of the calcaneus. Many times, this in turn leads to a below-the-knee amputation. A partial calcanectomy is a viable alternative to below-the-knee amputation. A more functional limb both mechanically and cosmetically is achieved, and the morbidity and mortality associated with the calcanectomy is less than with a below-the-knee amputation. A brief overview of the history and outcomes associated with this …

Fistula formation after restorative proctocolectomy poses a challenge to the surgeon and sometimes can lead to the excision of the pouch. A 21-year-old female patient developed an ileal J-pouch-sacral fistula with abscess and osteomyelitis of the sacrum, more than three years after the pouch construction for ulcerative colitis. Two months prior to this event, the patient had a single and transient episode of pouchitis. The role of pouchitis in the aetiopathogenesis of the fistula is unclear. To our knowledge, the late development of such a fistula has not been reported previously.

Deformities locomotor system in 24 children who have been operated on because of lipoma of conus medullaris are presented. At follow-up examination 6 patients were deformity free. The remaining group had feet deformities either present at birth or developed on average in 19th month of life. They mostly progressed and tended to recur after surgical correction. Trophic skin ulcer or chronic osteomyelitis of the foot was present in 30% of children; in 17% of cases they induced amputation. In 60% of cases scoliosis was found with mean angle of 10.2 degrees.

There is a high mortality of bacterial aortitis particularly as it is usually not recognized before the stage of rupture. Therefore the disease should early be considered in obscure febrile conditions. Problems of diagnosis and therapy are discussed on two own cases. A 63-year old man had a rupture of a small infrarenal aortic aneurysm in the course of salmonella sepsis, a 79-year old woman had a pyogenic osteomyelitis of a lumbar vertebra that spread to the aorta and caused its rupture. In both cases surgical treatment consisted of bleeding control followed by in-situ reconstruction.

A case of cryptococcal osteomyelitis of the skull is presented. The patient was an immunocompetent host with skull and skin involvement without central nervous system or pulmonary extension. The radiographic findings are reviewed to include skull films, bone scan, and computed tomographic and magnetic resonance imaging scans. The patient underwent surgical debridement of the lesion as well as systemic medical therapy with amphotericin B and flucytosine. The medical and surgical therapy for such lesions is reviewed. Surgical intervention is emphasized for the removal of bony sequestrum and nonv…

13 drill biopsies were microscopically examined and evaluated in respect of their histologic quality. 8 biopsies were obtained with a diamond hollow cutter, the other 5 specimens were sampled with a modified milling cutter according to Burkhardt. Regarding proper evaluation of bone biopsies e.g. in cases of osteomyelitis, factors as presence of bone marrow cavity or drill powder play an important role. Considering these factors the modified milling cutter according to Burkhardt yields, precise and careful handling provided, better results than the diamond hollow cutter.

Two patients with reconstruction of a massive mandibular defect with vascularized full-thickness calvarial bone flaps are reported. In Patient 1, the mandibular body developed osteomyelitis and once was replaced with a metallic prosthesis. The prosthesis later perforated the skin and was removed. Full-thickness calvarial bone flaps were elevated bilaterally to reconstruct the mandibular body. In Patient 2, the mandible was totally destroyed by invasion of squamous cell carcinoma. The lower one-half of the face was resected and replaced with a large island scalp flap with full-thickness calvari…

An unusual case of Mycobacterium marinum cutaneous infection is described. As a result of marked delay in the diagnosis, extensive local inflammation and destructive osteomyelitis occurred together with cutaneous dissemination in an immunocompetent host. Pathologic fractures in the infected bone necessitated amputation of the involved digit. The most striking feature of this case was the development of multiple widespread cutaneous lesions for several months following amputation of the infected digit and initiation of appropriate antimicrobial therapy. These new cutaneous lesions may reflect l…

The author evaluates the results of extension of 11 humeri in 10 children and adolescents with an average age of 12.5 years. The most frequent cause of the abbreviation was neonatal osteomyelitis. A Wagner apparatus was used with success and is considered highly suitable for this purpose. The maximum extension was 95 mm; one bone was extended twice with an interval of 2 years in between. The most serious complication was temporary paralysis of the nerves of the forearm and hand. Correction of the length of the upper extremities has a positive effect on the patient's psyche and on the aesthetic…

A case of rectourethral fistula is described, occurring after radical prostatectomy and adjuvant radiotherapy for pathologic Stage C carcinoma of the prostate. Urethral instrumentation for stricture disease immediately preceded development of the stricture. Conservative management for five years has resulted in the development of osteomyelitis of the pubis, but with antibiotic suppression the patient remains active and asymptomatic. Predisposing factors for the development of rectourethral fistula following radical prostatectomy may include adjuvant radiotherapy and recurrent urethral instrume…

Infections due to Pseudomonas aeruginosa are not confined to the hospital intensive care unit. This paper examines the association of P. aeruginosa and several community-acquired infections. Hot tub folliculitis is a recently described disorder occurring in outbreaks among persons who unknowingly immerse themselves in contaminated whirlpools, spas, or swimming pools. The green nail syndrome and other dermatoses are also reviewed. Infective endocarditis, invasive external otitis, and puncture would osteomyelitis are serious infections that carry high risks for the patient and challenge the phys…

Fibrous dysplasia may affect the jaws as a monostotic lesion which most often ceases to be active at the end of normal bone growth. In some cases there is no stabilization on completion of normal skeletal growth. This may lead to severe malformation and functional disturbance to the face. In these cases, the patient must be followed regularly and resection of the deformed tissue must be performed repeatedly as the recurrences appear. Attention is also brought to the fact that fibrous dysplasia may be difficult to separate from chronic sclerosing osteomyelitis, a condition that should be treate…

Two children with osteomyelitis, staphylococcal septicaemia and toxic shock syndrome (TSS) are reported. Both patients presented with fever, shock, scarlatiniform rash and diffuse mucositis. The multi-organ dysfunction which characterizes this syndrome was apparent in both patients, who had jaundice, functional renal impairment, diffuse myalgia and non-localizing neurological signs. One child developed toxic myocarditis and the other thrombocytopenia. Both showed a significant rise in antibody titre to enterotoxin B produced by the offending staphylococci, suggesting that this toxin was respon…

Five babies, aged between one to three months presented with clinical and serological evidence of syphilis. In addition to the usual periosteal and metaphyseal bone lesions of lues, areas of focal lucencies and sequestra were present. The association of sequestra in focal diaphyseal erosions may represent a non-dystrophic manifestation of the disease. The lesions were attributed to the syphilitic process. Trivial trauma to disorganised metaphyses, dystrophic changes and possibly focal osteomyelitis account for the spectrum of bone lesions in congenital syphilis. No tubular bone appears immune …

Report on a follow-up of 38 children treated with surgery for bronchiectasis during the last 10 years at the Department of Paediatric Surgery at the Dresden Medical Academy. 25 patients could be followed-up. In 17 of them a thoracotomy was performed in an age below 10 years. Besides some minor changes like narrowing of the intercostal spaces several cases with scoliosis of the vertebral column could be found. The importance of early controls and intensive physiotherapy for these cases is stressed. A case with rib osteomyelitis after surgery, which made thoracoplasty necessary is discussed.

Lincomycin, a chemically new antibiotic effective against Gram-positive organisms, was evaluated in vitro and tested clinically. In vitro testing indicated that lincomycin is especially effective against Staphylococcus aureus. Clinical testing showed that lincomycin was free of toxicity in a series of 18 cases of staphylococcal infection. Of particular interest was its pronounced effectiveness in nine cases of chronic osteomyelitis, one of which was of 15 years' duration and unresponsive to all other forms of antibiotic and surgical treatment. The only side effect noted was loose stools in the…

OBJECTIVES: Cemento-osseous dysplasia (COD) is the most common benign fibro-osseous lesion of the jaws, typically asymptomatic and identified through characteristic radiographic features. However, its rare association with simple bone cysts (SBCs) can significantly alter its presentation, mimicking more aggressive jaw lesions. This study aims to report additional cases of COD-associated SBCs and to review the literature to evaluate whether such lesions may represent a specific manifestation of the COD spectrum.
MATERIALS AND METHODS: Two cases of florid COD with atypical radiolucencies were d

Acute musculoskeletal infection is a common pediatric emergency that requires early diagnosis and management, without which adverse sequelae can include functional impairment, limb deformities, persistent infection, progression to sepsis, and mortality. Magnetic resonance imaging (MRI) is the gold standard for diagnosis. In recent years, there has been growing interest in the use of rapid MRI protocols for acute pediatric musculoskeletal infection, with benefits including quicker scan times with decreased need for sedation and contrast. In the present study, we discuss key considerations, prac
