OBJETIVE: We followed our previously reported algorithm based on intra and postoperative parathyroid hormone (PTH) levels to predict postthyroidectomy hypoparathyroid hypocalcemia. Familial hypocalciuric hypercalcemia (FHH) causes lifelong hypercalcemia with features that overlap with typical primary hyperparathyroidism (PHPT). The incompleteness of this overlap has led to divergent nomenclatures for FHH. I compare two nomenclatures. One sets FHH as an entity distinct from PHPT. We present a 46-year-old Caucasian lady with symptomatic hypocalcaemia. Investigations revealed markedly raised parathyroid hormone (PTH) levels with vitamin D deficiency. A number of conditions causing secondary hyperparathyroidism were ruled out from her medical history and initial investigations. In uremic plasma, it was proven that not only 1-84 PTH but a large C fragment PTH (probably 7-84 PTH) existed considerably and Intact PTH assay measured both PTH molecules. Whole PTH was developed to measure 1-84 PTH exclusively. From our study of the iliac bone biopsy, Whole PTH and Whole PTH/7-84PTH ratio were superior indicator to Intact PTH in diagnosis of adynamic bone disease. Although there is possibility of induction of adynamic bone disease in vitamin D treatment, Whole PTH will serve as a useful monitoring method in order to prevent such risk.

It has been shown that intact-PTH assay measures not only full-length biologically active PTH with 84 amino acids, but also some PTH fragments such as (7-84) PTH. Whole-PTH assay is a new method believed to react only with (1-84) PTH. Because circulatory concentrations of various fragments of PTH are very high especially in patients with secondary hyperparathyroidism associated with renal failure, evaluation of PTH levels by whole-PTH assay seems to be useful for accurate assessment of parathyroid function in these patients.
