Pediatric mutilating hand injuries.

Mutilating hand injuries in children are a devastating problem. With aggressive efforts at replantation and revascularization, methodic debridement, timely soft tissue coverage, and early mobilization, however, the results in these unfortunate children can be quite rewarding. The child often does well with the functional aspect of recovery and rehabilitation but will probably hide his or her deformed hand from friends and family. These children generally become more shy and reserved. The parents are the key to rehabilitation. A good relationship between the parent, the physician, and the hand

Subungual melanoma in situ: two independent streaks in one nail bed.

We report an unusual case of two nail-bed streaks under one nail, revealing melanoma in situ affecting the nail bed and plate, but not the nail fold. This unusual mode of presentation together with the absence of any nail-fold involvement has not been reported previously. We highlight the need for early histological assessment of nail-bed lesions and discuss subungual melanoma.

Fingertip reconstruction by nail bed grafting using thenar flap.

Ideally, treatment for fingertip injury should involve the least pain possible, using durable and sensate skin with due consideration to aesthetic aspects. This paper presents two cases of fingertip reconstruction through the use of thenar flaps and nail bed grafts. In either case, injury had been due to fingertip crushing and reconstruction was conducted immediately thereafter. A full-thickness nail bed with hyponychium and perionychium from the severed part was used for dorsal reconstruction and thenar flaps for finger pulp reconstruction. In both cases, the grafting was quite satisfactory a

Reconstruction of the labiomental region with local flaps.

BACKGROUND: Reconstruction of the labiomental area presents certain difficulties, especially in the assessment and selection of the incision lines to obtain the best cosmetic result. Among the factors influencing the selection of a reconstruction technique are the presence of natural folds and creases, skin laxity, and the size, shape, and orientation of the wound. Many operative techniques have been described for the reconstruction of this region. OBJECTIVE: To describe a combination of reconstruction techniques which restore the form, preserve sensation, and provide function of the labiomen

Recovery of sensitivity in the hand after reconstruction with arterialised venous flaps.

Loss of soft tissues of the fingers were repaired in 22 patients using 25 arterialised venous flaps harvested from the thenar, hypothenar, or forearm regions. Twenty-one of the flaps survived completely, 16 of which were raised from the thenar or hypothenar region, and the other five from the forearm region. We studied the sensory recovery and skin characteristics of the flaps harvested from the three regions. Good sensory recovery was obtained for the thenar or hypothenar venous flaps, which were characterised by durable skin and suitable texture for replacement of defects in the finger pulp.

Construction of female urethra using buccal mucosa graft.

PURPOSE: We reviewed our experience with buccal mucosa grafts for reconstructing difficult female urethral problems. METHODS AND METHODS: Since 1994, we have used a buccal mucosa graft to reconstruct the urethra in 7 girls 3 to 13 years old. The underlying pathological condition was a fibrotic urethra after previous operations for cloacal exstrophy, cloacal malformation, iatrogenic urethral stricture, and multiple false passages in a previously reconstructed urethra of vaginal mucosa that made clean intermittent catheterization difficult. A full-thickness buccal mucosa graft was tubularized i

[Experimental study on repair of bile duct defects with expanded polytetrafluoroethylene].

OBJECTIVE: To explore the possibility of repair of the extensive bile duct injuries with expanded polytetrafluoroethylene (ePTFE). METHODS: A total of 36 local healthy hybrid dogs were employed to establish bile duct injury models by means of partial removal of bile duct (group A, 14 dogs), excision of a segmental duct (group B, 10 dogs), and ligation of the lower part of common bile duct(group C, 5 dogs; group D, 7 dogs). Group A were patched with ePTFE mesh, group B replaced by ePTFE tube. Interposition of the grafts between gallbladder and duedenum or jejunum was performed on group C and g

Ear piercing for individuals with metal hypersensitivity.

OBJECTIVE: To describe and evaluate an ear piercing and earring retention method for individuals with metal hypersensitivity. SETTING: Private facial plastic surgery practice associated with a tertiary care medical center. METHODS: Thirty-one patients with a history of hypersensitivity to metallic jewelry (62 ears) underwent earlobe piercing with an intravenous catheter. RESULTS: None of the patients experienced an infection or hypersensitivity reaction. All patients were able to wear nickel-free earrings for short periods without using the shortened catheter. CONCLUSION: Using the distal

Vaginal reconstruction in adolescent females with Mayer-Rokitansky-Kuster-Hauser syndrome.

Throughout time, vaginal and uterine anomalies have had dramatic and disconcerting effects on individuals and families with regard to issues of femininity, reproduction, sexual options, body image, and self-esteem. Mayer-Rokitansky-Kuster-Hauser (MRKII) syndrome is characterized by complete or partial absence of the vagina, uterus and proximal fallopian tubes. The modified Abbe-McIndoe technique is commonly used to create a neovagina. This technique is considered by many to be the procedure of choice because of its simplicity, low morbidity, and success rate. Females with MRKH syndrome have op

Amputation or salvage of type 3B/3C tibial fractures: what the literature says about outcomes.

Reconstructive microsurgical techniques, improvements in skeletal fixation, and aggressive bone grafting have been successful in salvaging high-energy open tibial fractures that would have been amputated in the past. Although limb salvage is often feasible, it is not always advisable-functional outcomes and patient satisfaction are not merely a matter of achieving union and restoring a soft-tissue envelope. This article examines current literature that addresses the complex interplay of salvage and restoration of a functional limb.

Surgical treatment of the rheumatoid thumb.

Rheumatoid arthritis commonly affects the thumb. Deformity does not require surgical intervention unless pain is present or a functional deficit exists, but if bony erosion develops and surgical treatment may be compromised in the future, earlier intervention may be indicated. When the pathogenesis and pathoanatomy of the impaired rheumatoid thumb are appreciated, and appropriate treatment is selected, surgical intervention is likely to provide a favorable outcome for the patient.

Understanding the perionychium.

The fingernail and toenail are unique to primates. The anatomy and physiology of the nail must be understood for nail care to be effective. The most common cause of deformity of the nail bed is trauma, which requires careful suturing and postoperative care if the desired results are to be achieved. Congenital anomalies, tumors, and infection can also cause nail deformities and can frequently be corrected or improved surgically.

New tumor formation on split-thickness skin grafted areas in xeroderma pigmentosum.

Xeroderma pigmentosum is a relatively rare systemic disease transmitted through an incomplete sex-linked recessive gene. It is characterized by malignant skin degeneration. One of the most effective treatment choices for the malignant changes is full-face resurfacing with skin grafts. Grafts harvested from areas that have some freckles may show malignant degeneration by ultraviolet exposure. The authors present a patient whose face was resurfaced with a split-thickness skin graft and was admitted due to new tumor formation on her resurfaced face.

[Combined retropubo-perineal radical prostatectomy and a new method of reconstructing the posterior urethra].

OBJECTIVE: To evaluate the efficacy of a new method of reconstructing the posterior urethra for the combined retropubo-perineal radical prostatectomy. METHODS: Six cases of prostate cancer were treated by operative method mentioned previously. For the operation, the routine retropubic radical prostatectomy was carried out first and then through the perineal incision, 4-6 cm of the residual urethra was seperated, held upward and anastomosed with the bladder neck. RESULTS: The urinary catheter was maintained for 14-28 days postoperatively on an average of 21.6 days. After 2-17 (average, 7.3) m

Upper extremity salvage using the tensor fascia lata flap: report of two cases.

The tensor fascia lata pedicled flap was successfully used to salvage 3 severely injured upper extremities in 2 patients. Both patients had undergone 3 prior free tissue transfers without complete closure of their wounds. All 3 tensor fascia lata flaps (2 myocutaneous, 1 myofascial) survived entirely. We believe this flap offers a distinct advantage compared with the groin flap when pedicled flap coverage of the upper extremity is required.

Microneurovascular transfer of contralateral latissimus dorsi in Poland's syndrome.

A 2-year-old girl with Poland's syndrome presented with syndactyly of the first, second and third webs of the left hand and ipsilateral absence of the sternocostal portion of pectoralis major. Latissimus dorsi on the same side was also absent. A microvascular free contralateral latissimus dorsi muscle transfer was undertaken to reconstruct the chest wall and anterior axillary fold. The transfer was innervated by intercostal nerve transfer and produced a neuromuscular unit of good aesthetic appearance and function.

The blood supply of the reverse temporalis muscle flap: anatomic study and clinical implications.

Although the reverse temporalis muscle flap has been used clinically, the exact vascular connection between the superficial and deep temporal vessels has not been clearly defined. The purpose of this study was to investigate the vascular territory of the reverse temporalis muscle supplied by the superficial temporal vessels. Six cadaver heads were studied using a colored lead oxide injection through the superficial temporal artery. The specimens were examined macroscopically and radiographically. The reverse temporalis muscle flap was then applied to a clinical case presenting with traumatic a

[Reconstruction of hip joint function: old fracture-dislocation of hip joint complicated with deformed healing of upper 1/3 of fractured femur--a case report].

OBJECTIVE: To explore a method of reconstruction of hip joint function after deformed healing of the upper 1/3 of fractured femur as a complication of old fracture dislocation of hip joint. METHODS: A patient with loss of function in hip joint and fusion of knee joint was treated with lock for femur intra-medullary fixation in April 1997. RESULTS: Before operation, the diseased hip joint lost its most functions and the entire lower extremity was disabled because the knee joint had been fused. One year after operation, the follow-up examination revealed that the patient could walk by crutches
