Motor neurone disease.

Motor neurone disease (MND) patients exhibit poor gait, balance, and postural control, all of which significantly increases their risk of falling. Falls are frequent in the MND population, and are associated with an increased burden of disease. The complex interplay of both motor and extramotor manifestations in this disease contributes to the heterogeneous and multifactorial causes of such dysfunction. This review highlights the pathophysiologic influence of motor degeneration in gait disturbance, but also the additional influence on postural instability from other inputs such as cognitive im

A novel pathogenic variant c.975G>A (p.Trp325*) in the POU3F4 gene in Yakut family (Eastern Siberia, Russia) with the X-linked deafness-2 (DFNX2).

Here, we report a novel hemizygous transition c.975G>A (p.Trp325*) in POU3F4 gene (Xq21) found in two deaf half-brothers from one Yakut family (Eastern Siberia, Russia) with identical inner ear abnormalities ("corkscrew" cochlea with an absence of modiolus) specific to X-linked deafness-2 (DFNX2). Comprehensive clinical evaluation (CT and MR-imaging, audiological and stabilometric examinations) of available members of this family revealed both already known (mixed progressive hearing loss) and additional (enlargement of semicircular canals and postural disorders) clinical DFNX2 features in aff

The Tongue, Mandible, Hyoid System.

The craniocervical-mandibular system works in harmony and allows different functional tasks as the postural control of the cervical region. This system is formed by the temporomandibular joint, the masticatory muscles and ligaments connecting the temporomandibular joint and the cervical region. It has been seen to affect human posture, and many disorders of the temporomandibular joints may affect this functional arrangement. Notwithstanding this system considered as a functional arrangement, may offer various clinical explanations, it does not hold anatomical connections with the posterior reg…

Harmaline-induced tremor in mice: videotape documentation and open questions about the model.

BACKGROUND: Harmaline-induced tremor in rodents has been extensively used as an animal model for essential tremor (ET). However, there is no visual documentation in the published literature. METHODS: We injected mice subcutaneously with either 20 mg/kg of harmaline hydrochloride or saline and then videotaped the responses. RESULTS: Action and postural tremor in the mouse began 5 minutes after subcutaneous harmaline injection and peaked at approximately 30 minutes. The tremor involved the head, trunk, tail, and four limbs and lasted for approximately 2 hours. The forelimb tremor was postural

Optic neuropathy associated with spontaneous intracranial hypotension.

Spontaneous intracranial hypotension is characterized by postural headache that is generally associated with neck and/or back pain, radicular symptoms, nausea, and vomiting, and can sometimes be accompanied by cranial nerve symptoms. Although ocular manifestations are common, visual impairment due to optic nerve involvement is not commonly reported. Here, we report a case of a 34-year-old woman with postural headache and visual loss in the left eye associated with spontaneous intracranial hypotension. The orbital magnetic resonance imaging revealed increased intensity on T2-weighted images and

Peripheral trauma induced dystonia or post-traumatic syndrome?

The relationship between peripheral trauma and dystonia has been debated for more than a century but the issue still remains controversial. There are passionate supporters and detractors of the association and both the groups have their own arguments. This review aims to critically evaluate those arguments and presents current understanding of this association. In the process, the relevant case series and scientific papers exploring this subject have been discussed. Upon careful review of available literature coupled with their own experience, the authors believe that peripheral trauma can pre

Ergonomic applications to dental practice.

The term "work-related musculoskeletal disorders (WMSDs)," refers to musculoskeletal disorders to which the work environment contributes significantly, or to musculoskeletal disorders that are made worse or longer lasting by work conditions or workplace risk factors. In recent years, there has been an increase in reporting WMSDs for dental persons. Risk factors of WMSDs with specific reference to dentistry include - stress, poor flexibility, improper positioning, infrequent breaks, repetitive movements, weak postural muscles, prolonged awkward postures and improper adjustment of equipment. Erg…

Cardiac sympathetic denervation in bradykinesia-dominant Parkinson's disease.

Cardiac iodine-123-labeled-metaiodobenzylguanidine uptake is reduced in early-stage Parkinson's disease, suggesting sympathetic nerve degeneration. The scintigraphic findings in patients with Parkinson's disease with different clinical features have, however, not been established. Iodine-123-labeled-metaiodobenzylguanidine myocardial scintigraphy was performed in 143 patients with Parkinson's disease. The early and delayed heart to mediastinum ratios were analyzed according to the dominant motor deficit (tremor, bradykinesia, rigidity, and postural instability), age, sex, age at onset, disease

Subjective visual tilt and lateral instability after vestibular deafferentation.

CONCLUSION: Defective utricular function, reflected by deviation of the perceived visual horizontal or vertical, cannot by itself explain increased postural sway in the lateral direction for patients with unilateral vestibular deafferentation (uVD). OBJECTIVE: It is not known why some patients with chronic uVD report distressing unsteadiness while others do not. The objective of this study was to test the hypothesis that impaired utricular compensation, reflected by pathologic tilt in the subjective visual horizontal and vertical (SVH-V) test, would affect postural control in the lateral dire

Asterixis related to gabapentin as a cause of falls.

Negative myoclonus, commonly known as asterixis, is often observed in patients with toxic-metabolic encephalopathies or focal brain lesions. It is a movement disorder characterized by postural lapses resulting from brief cessation of tonic muscular contraction. Negative myoclonus has a characteristic appearance on needle electromyography. Lapses in continuous postural muscle activity can lead to falls. This increased risk of falls makes it particularly important to recognize and treat negative myoclonus, especially in patients with multiple medical problems, deconditioning, and gait disturbanc

[Biomechanical risk in two dairy industries with different levels of mechanization].

Risks of biomechanical overload of the upper limbs and of altered posture are not well investigated in dairy industry. This study carries out an evaluation of ergonomic and postural risk in two cheese factories in North Sardinia, characterized by a different level of automation, in order to point out at the more dangerous activities for biomechanical overload and to check possible improvements determined by mechanization. Ergonomic analysis has been performed by using NIOSH and OSHA check-lists, and then using the Strain Index method for repetitive movements. Results show that mechanization ha…

Clinical differentiation of parkinsonian syndromes: prognostic and therapeutic relevance.

Parkinson disease is the most common cause of parkinsonism, but other causes should always be excluded because they have a different prognosis, respond differently to medical treatment, and should not be managed by surgical means. However, diagnosis, even by experts, is challenging; one autopsy series showed an error rate of 24%. Distinction between various diagnostic possibilities depends on the history and examination findings. The use of certain medications, the rapid rate of disease progression, early onset of falling, the presence of certain dysautonomic symptoms, cognitive or behavioral

The clinical expression of primary dystonia.

Dystonia (from the Greek, "altered muscle tone") refers to sustained and vigorous contractions forcing a body region into an abnormal position that is consistently present. Dystonic postures and movements can variably combine to produce a wide spectrum of clinical presentations. The movement can affect one, two or more body regions, as in focal, segmental or generalised dystonia. Dystonia arising in childhood tends to progress to a generalised form; adult-onset dystonia, instead, is often focal or segmental. Dystonic movements have specific features that can be recognised by clinical observati…

Carpal tunnel syndrome. It's not just with keyboards.

CTS is often preventable. Take care of your hands when at work or at play when performing repetitive tasks, especially if there is also force, vibration, or awkward postures. Try to keep your wrists in a neutral posture, use tools and techniques that minimize required force, have a proper working surface height, design and arrange your work space to minimize the CTS risk factors, and make sure to have enough breaks to have sufficient recovery time. Avoid being overweight. If you experience pain similar to that described with CTS, see your medical doctor. He or she can help differentiate CTS fr…

[A case of autosomal recessive juvenile parkinsonism (AR-JP) with onset at the age of 6 years].

We report a 26-year-old woman with autosomal recessive juvenile parkinsonism (AR-JP). Her parents made consanguineous marriage. The disease started with dystonic posture of legs 20 years prior to the admission (6 years old). The equinovarus foot appeared at 9 years old and parkinsonism gradually developed. These symptoms were remarkably improved after sleep. Levodopa was effective on her parkinsonian symptoms. However, the response to levodopa gradually reduced. On admission, she presented with a masked face, marked rigidity, moderate retropulsion, postural tremor in the upper limbs and mild h…

[Animal nutrition in veterinary medicine--actual case reports. Nutritionally related disturbences in bone development in emus and rheas].

Two cases of disturbed health and growth of the skeleton in emus and rheas caused by faults in feeding and mineral supply are reported. 13 of 37 emus developed (beginning in the 3rd week) more and more signs of perosis. The diet fed in this case was based mainly on pelleted diets for piglets and rabbits and unpelleted supplements. The ingesting behaviour (selective intake and refuse of fines and supplements) resulted in an imbalanced mineral intake. In the second case 3 of 15 young rheas showed unphysiological postures of the necks vertebral column. Disposed by an absolutely insufficient calci…

Reflex behavior and programming in Parkinson's disease.

Several aspects of characteristically impaired postural adjustments indicate a defective central programming of the respective EMG patterns in PD. This may be due to deficits in the supraspinal control of spinal interneuronal circuits. This impairment obviously contributes to the difficulty of these patients in performing two motor acts simultaneously. In addition, parkinsonian patients exhibit a reduced sensitivity of polysynaptic reflexes in the leg extensor muscles which correlates with their postural instability. The activity of monosynaptic reflexes is negligible, as in healthy subjects.

Circulatory autonomic failure 50 years after acute poliomyelitis.

A 59-year old woman who presented with postural dizziness 50 years after an acute episode of poliomyelitis is described. There were no new neurological signs and no evidence of motor neuron disease. She had postural hypotension with an abnormal Valsalva. Investigations led to a diagnosis of hypo-adrenergic orthostatic hypotension, with a predominantly preganglionic sympathetic lesion and intact vagal baroreflex pathways. Although pure autonomic failure and multiple system atrophy are possible causes of circulatory autonomic failure, no other new neurological or autonomic features have develope
