Automated multi-class classification of skin lesions through deep convolutional neural network with dermoscopic images.

As an analytic tool in medicine, deep learning has gained great attention and opened new ways for disease diagnosis. Recent studies validate the effectiveness of deep learning algorithms for binary classification of skin lesions (i.e., melanomas and nevi classes) with dermoscopic images. Nonetheless, those binary classification methods cannot be applied to the general clinical situation of skin cancer screening in which multi-class classification must be taken into account. The main objective of this research is to develop, implement, and calibrate an advanced deep learning model in the contex

Eclipta alba (Bhringraj) in ayurveda has been widely used as a traditional medicine for its multi-therapeutic properties for ages. Luteolin (LTL), wedelolactone (WDL) and apigenin (APG) are the three main bioactive phytochemicals present in Eclipta alba extract. However there was a lack of sensitive bioanalytical method for the pharmacokinetics of these free compounds in plasma which majorly contributes for their activities after oral administration of Eclipta alba. The present study aims to develop a sensitive, rapid and reliable liquid chromatography tandem mass spectrometry (LC-MS/MS) metho

Integrating ctDNA Dynamics and Sequential Organoid Drug Screening Enhances Breast Cancer Treatment Efficacy.

UNLABELLED: Dynamic changes in ctDNA variant allele frequency reflect real-time therapeutic efficacy. Timely treatment modification guided by ctDNA variant allele frequency may enable precision selection of optimized regimens to counteract evolving resistance in breast cancer. In this study, we developed a therapeutic approach integrating ctDNA dynamics with sequential organoid drug screening to optimize breast cancer treatment. Targeted deep sequencing on plasma samples from 71 patients with breast cancer revealed that ctDNA clearance correlates with improved disease-free survival. Additional

An Eight-Century Evolution of Asthma Therapeutics in Western Countries: From Medieval Regimens to Anti-inflammatory Reliever Therapy and Precision Biologics.

The changes in our understanding of asthma etiopathogenesis has led to new therapies and a significant reduction in asthma exacerbations as well as mortality. The management of asthma has evolved over several centuries from constitutional/environmental palliation, to the recognition of recurrent reversible airway obstruction and to the recognition of inflammatory changes in the lung compartment. The therapeutic armamentarium has also shifted from using alternative therapies at first, to bronchodilator rescue alone and then to inflammation-directed controller therapy with corticosteroids. In th

Modeling donor age and type trade-offs for personalized selection in allogeneic hematopoietic cell transplantation with post-transplant cyclophosphamide prophylaxis.

The prevailing fixed-hierarchy approach to donor selection for allogeneic hematopoietic cell transplantation (HCT) fails to capture critical interactions between donor age and type. We addressed this by analyzing 1713 adult recipients of matched related (MRD), matched unrelated (MUD), or haploidentical donors, receiving post-transplantation cyclophosphamide, using both regularized-Cox and XGBoost machine learning models. Our analysis revealed that the overall, independent association of donor age with survival was modest; however, its importance became pivotal within specific, context-dependen

High-Dose Intravenous Vitamin C in Critical Illness: A Translational Exposure-Response Framework for Biomarker-Guided Precision Therapy.

High-dose intravenous vitamin C (HDIVC) has been investigated as a potential adjunctive therapy in critical illness, including sepsis, acute respiratory distress syndrome (ARDS), and COVID-19. Despite a strong mechanistic rationale, clinical trials have yielded inconsistent results. From a clinical pharmacology perspective, this variability may reflect, at least in part, differences in pharmacokinetic exposure, timing of administration, and patient selection rather than a lack of biological activity. Intravenous administration enables plasma concentrations in the millimolar range (≈1-5 mM), fa

A Machine Learning-Based Strategy Predicts Selective and Synergistic Drug Combinations for Relapsed Acute Myeloid Leukemia.

UNLABELLED: Combination therapies are one potential approach to improve the outcomes of patients with relapsed/refractory (R/R) disease. However, comprehensive testing in scarce primary patient material is hampered by the many drug combination possibilities. Furthermore, inter- and intrapatient heterogeneity necessitates personalized treatment optimization approaches that effectively exploit patient-specific vulnerabilities to selectively target both the disease- and resistance-driving cell populations. In this study, we developed a systematic combinatorial design strategy that uses machine le

Prostate cancer (PCa) is a major cause of cancer-related mortality in men worldwide. PCa mortality is expected to almost double by 2040 which is probably associated with the increase in obesity rates, a global health problem linked to an increased risk of developing multiple cancer types. Therefore, the identification of improved, personalized, therapeutic and diagnostic/prognostic tools is urgently needed. MicroRNAs have emerged as promising diagnostic/therapeutic tools for various metabolic/tumoral pathologies but the clinical utility and pathophysiological role of different miRNAs in the in
