Methods of skeletal reconstruction following resection of lung carcinoma invading the chest wall.

The 5 per cent of patients with carcinoma of the lung directly invading the chest wall present a challenge for treatment. Complete resection of all tumor, including the chest wall, should be done. Immediate reconstruction to prevent a flail chest ensures a smooth postoperative course. Excellent palliation and a 46 per cent 5-year survival rate can be achieved if there is no spread to mediastinal lymph nodes.

[Diminution of radical surgical cavities with hydroxyapatite].

Canal wall surgery is the safest way to eradicate cholesteatoma and to prevent recurrence of the disease. On the other hand, resulting cavity problems such as a permanently discharging ear and poor functional results made the development of alternative procedures necessary. A new technique to obliterate the mastoid volume or to reduce an old cavity by means of hydroxyapatite granulate is presented. Characteristics of the material and how to manage it are demonstrated. First experiences in 30 ears are reported. Hydroxyapatite granulate enables us to establish a primarily epithelized small cavit

Postoperative osteomyelitis following implant arthroplasty of the foot: diagnosis with indium-111 white blood cell scintigraphy.

Many complications can occur following insertion of silicone elastomer implants into the foot. Postoperative infection may be difficult to distinguish from other conditions such as dislodgment, fracture, ectopic and heterotopic new bone formation, synovitis, and bursitis. White blood cell scintigraphy, in conjunction with the clinical scenario, may prove to be an invaluable tool in the diagnosis of postoperative osteomyelitis, subsequent to implant arthroplasties.

Toxic shock syndrome following breast and nasal surgery.

Toxic shock syndrome is a rapidly developing disease which may be lethal if not recognized and treated early. While this disease is usually associated with menstruating females, it is being seen in both male and female patients following clean, elective surgery. Toxic shock syndrome has been reported following surgical procedures which normally have a very low incidence of postoperative wound infections. Toxic shock syndrome is associated with Staphylococcus aureus bacteria which produce the toxic shock syndrome toxin 1. Three patients with postoperative toxic shock syndrome, of varying degree

Silicone synovitis following silicone implant arthroplasty.

Silicone synovitis is a late complication of silicone implant arthroplasty. The problem is usually heralded by pain and limitation of motion caused by synovitis and bone destruction. Once silicone synovitis develops, secondary procedures include removal of the implant, synovectomy, and reconstructive surgery individualized to the patient's pathology.

Absent latissimus dorsi muscle and anhidrotic axilla in Poland's syndrome. Case report.

The case of an 18-year-old female with absence of the right breast, sternocostal part of the major pectoral muscle and total absence of the minor pectoral muscle and the latissimus dorsi muscle, is presented. She also had decreased sweating capacity and absence of terminal hairs in the central part of her right axilla. Histological examination of skin specimens from this area of the axilla demonstrated hypoplasia of the apocrine sweat glands, but normal eccrine sweat glands, indicating an apocrine dependent axillary sweating function. The breast was constructed using a direct subcutaneous pros

The scrotal myocutaneous flap.

The scrotum is a thermoregulatory, well-vascularized structure formed by skin and nonstriated muscle with unique elastic properties. This makes it an ideal source of tissue coverage for problem wounds in its vicinity. Two patients in which scrotal musculocutaneous flaps were used are reported: one, a paraplegic, with a recurrent ischioperineal decubitus ulcer, and another with an ulcer of the penis with exposed Dacron graft previously placed to treat Peyronie's disease. After reviewing the anatomy of the scrotum and the existent literature, we studied scrotal vascularity in a fresh specimen by

Proplast augmentation for posttraumatic zygomatic deficiency.

Facial deformities following fractures of the zygomatic complex are common. Included in this article are the indications, work-up, and surgical technique for placement of Proplast implant material to correct the deformity of the malunited zygomatic complex. Three cases are presented to illustrate this approach.

A pertrochanteric fracture in an adolescent treated for an osteogenic sarcoma of the shaft of the femur with chemotherapy, tumour resection and endoprosthesis.

Osteosarcomas of the femur are treated nowadays, if possible, by chemotherapy, resection of the tumor and reconstruction with an endoprosthesis. There is not only a risk of epiphyseal fractures in the long bones due to chemotherapy but also of a fracture in the remnant of the treated femur. These fractures should be treated conservatively because of the risk of infection and non-union if treated operatively.

Vaginal aplasia: therapeutic options.

Müllerian dysgenesis, a common developmental anomaly presenting as primary amenorrhea, is characterized by absence of the vagina and absence or rudimentary development of the uterus. Anatomic anomalies of the axial skeleton, inguinal canal, and kidney often accompany this psychologically devastating condition. Twenty-two cases are reviewed in regard to associated clinical features, principles of treatment, and adequacy of outcome. A good functional result can be achieved either by the use of a progressive nonsurgical self-dilatation technique, or by surgical cavitation with skin grafting, alth

Orbital reconstruction.

Twenty patients with severe orbital and orbitomaxillary deformities secondary to oncologic surgery and radiation were studied. Sixteen patients underwent complete reconstruction of the orbit and the periorbital area. The orbital reconstruction was accomplished with autogenous tissue (skin and dermis grafts) that was anchored to the bony orbit. The periorbital area was augmented with customized implants. Most patients now wear a satisfactory prosthesis with or without lightly tinted glasses, giving them an acceptable orbital symmetry.

Results of endolymphatic sac surgery in advanced Meniere's disease.

A rational surgical approach to Meniere's disease eludes us because of the uncertainty of the pathophysiology and the variability in the natural history of the disease. The results of different surgical techniques are difficult to compare because of differences in patient selection, methods of analysis, and length of follow-up reported. This study reviews long-term results in twenty-four ELS procedures in which the test group and parameters of evaluation are carefully described. The overall rate of vertigo control is 75 percent and incidence of significant hearing improvement is 25 percent. Th

Rupture of digital extensor tendons following distal ulnar resection.

We have treated four patients with rupture of one or more digital extensor tendons following a distal ulnar resection for a traumatic problem. The rupture was caused by the abrasive effect of the distal part of the ulna at the site of resection during pronation and supination. Two patients were treated with a silicone-rubber prosthesis ad tendon grafts and two, with resection of an ulnar bone spike and local tendon transfers. Tendon rupture is best avoided by strict adherence to the surgical technique described by Darrach. If rupture of the tendon occurs, reconstructive procedures must be devi
