Polymorphism of the catechol-O-methyltransferase gene in Han Chinese patients with psoriasis vulgaris.

PSORIASIS VULGARIS IS DEFINED BY A SERIES OF LINKED CELLULAR CHANGES IN THE SKIN: hyperplasia of epidermal keratinocytes, vascular hyperplasia and ectasia, and infiltration of T lymphocytes, neutrophils and other types of leukocytes in the affected skin. Catechol-O-methyltransferase (COMT) 158 polymorphism can reduce the activity of the COMT enzyme that may trigger defective differentiation of keratinocytes in psoriasis. Immunocytes can degrade and inactivate catecholamines via monamine oxidase (MAO) and COMT in the cells. We hypothesized that the COMT-158G > A polymorphism was associated with…

Incidence and clinical predictors of psoriatic arthritis in patients with psoriasis: a population-based study.

OBJECTIVE: To determine the incidence and disease-specific predictors of clinically recognized psoriatic arthritis (PsA) in patients with psoriasis. METHODS: We identified an incidence cohort of psoriasis subjects age >/=18 years diagnosed between January 1, 1970 and December 31, 1999 in a population-based setting. Psoriasis diagnoses were validated by confirmatory diagnosis in the medical record. Incident and clinically recognized PsA subjects were classified according to the Classification of Psoriatic Arthritis (CASPAR) criteria. Cox proportional hazards models were used to identify predic…

Deletion of the late cornified envelope LCE3B and LCE3C genes as a susceptibility factor for psoriasis.

Psoriasis is a common inflammatory skin disease with a prevalence of 2-3% in individuals of European ancestry. In a genome-wide search for copy number variants (CNV) using a sample pooling approach, we have identified a deletion comprising LCE3B and LCE3C, members of the late cornified envelope (LCE) gene cluster. The absence of LCE3B and LCE3C (LCE3C_LCE3B-del) is significantly associated (P = 1.38E-08) with risk of psoriasis in 2,831 samples from Spain, The Netherlands, Italy and the United States, and in a family-based study (P = 5.4E-04). LCE3C_LCE3B-del is tagged by rs4112788 (r(2) = 0.93…

Targeted treatment of psoriasis with adalimumab: a critical appraisal based on a systematic review of the literature.

Psoriasis is a chronic inflammatory disease affecting 2% to 3% of the population in Western countries. Psoriasis is associated with limited quality of life, cardiovascular disease, and depression. The approval of injectable biological agents has revolutionized the management of moderate to severe psoriasis. Adalimumab is a human monoclonal antibody against tumor necrosis factor (TNF) alpha approved for moderate-to-severe plaque-type psoriasis and psoriatic arthritis (PsA). This systematic review summarizes the evidence concerning the efficacy, clinical effectiveness, safety, and cost-effective…

The burden of autoimmune disease: a comparison of prevalence ratios in patients with psoriatic arthritis and psoriasis.

BACKGROUND: Current research has confirmed that many inflammatory autoimmune (AI) diseases are due to derangements in multiple cytokine pathways. Some of these cytokines appear to play comparable key roles across diseases, suggesting that a similar underlying systemic inflammatory cascade could be responsible for various disease states. A recent study supports the hypothesis of a common cytokine-based pathology by showing that having one AI disease significantly increased the risk of having another AI disease. Psoriasis is an AI, manifesting as a chronic inflammatory skin condition and is clea…

Mesangial proliferative glomerulonephritis with or without IgA deposits: the morphological characters in psoriasis vulgaris.

BACKGROUND/AIM: Chronic glomerulonephritides associated with psoriasis vulgaris have been reported in the literature. However, because of the limited number of cases and the lack of specific histological findings, the existence of psoriatic nephropathy remains controversial. This study was performed to investigate the clinical and morphological characteristics of renal involvement in psoriasis vulgaris. METHODS: We studied retrospectively 11 patients with psoriasis vulgaris. All patients had nephritic urinary sediments and underwent percutaneous renal biopsy between January 1997 and December …

Psoriasis disappearance after the first phase of an oncologic treatment: a serendipity case report.

Psoriasis is a multifaceted disorder with psychosocial and physical aspects that negatively impact the quality of life. Strategies of treatment must address both rapid control of the disease and maintenance of benefits. For short and long-term control of localized psoriasis, recent data support the combined use of topical corticosteroids and either calcipotriene or tazarotene which seem to be the most effective approach. For generalized disease, UVB treatment provides the safest means of achieving long-term control of the disease. Acitretin is a very helpful adjunct for improving the efficacy …

Development of new-onset psoriasis in a patient receiving infliximab for treatment of rheumatoid arthritis.

Infliximab is a chimeric monoclonal antibody that selectively blocks tumor necrosis factor-alpha (TNF-alpha). It is indicated for the treatment of numerous inflammatory diseases, including rheumatoid arthritis, Crohn disease, ulcerative colitis, ankylosing spondyilitis, and psoriatic arthritis. Infliximab has also been shown to be a well tolerated and highly effective treatment for psoriatic skin lesions. We report an interesting case of unexpected, new-onset psoriasis in a patient on infliximab for rheumatoid arthritis.

Relationship between anger and pruritus perception in patients with chronic idiopathic urticaria and psoriasis.

BACKGROUND: There is evidence that chronic idiopathic urticaria (CIU) and psoriasis are associated with personality based difficulties in emotional regulation particularly with regard to the feeling of anger. This deficit in emotional awareness could lead to the phenomenon that emotions are rather experienced in bodily symptoms such as pruritus. AIM: We investigated whether there is a relationship between pruritus as major symptoms in CIU and psoriasis and the experience of negative emotions. SETTING: Forty-one CIU patients and 44 psoriasis patients treated at Bonn University Hospital and 49…

Long-term results of radiotherapy in patients with chronic palmo-plantar eczema or psoriasis.

BACKGROUND AND PURPOSE: Radiotherapy (RT) is well accepted for therapy-refractory palmo-plantar eczema or psoriasis, despite of lacking evidence regarding beneficial long term effects. Furthermore, the optimal irradiation dose is unknown. We evaluated the outcome of RT with two different RT single/total dose (SD/TD) treatment policies. PATIENTS AND METHODS: 28 consecutive patients with therapy-refractory eczema (n = 22) or psoriasis (n = 6) of palms and/or soles were irradiated twice a week either with a D(max) SD of 1 Gy (6/98-5/03; median TD: 12 Gy) or 0.5 Gy (6/03-7/04; median TD: 5 Gy). Clinical review of 202 patients with vulval lichen sclerosus: A possible association with psoriasis.

Two hundred and two patients with clinically typical or biopsy-confirmed vulval lichen sclerosus were reviewed either at consultation (75%) or by retrospectively examining their chart. At diagnosis, 79% were 50 years or older. Ninety-six per cent complained of itching, pain and/or dyspareunia. Lichen sclerosus most often affected the labia minora and perineum but 50% had perianal and 13% had extragenital disease. Thirty-five patients gave a history of psoriasis (17%), which affected the vulval area in 10. Thyroid disease was reported in 39 patients (19%), and 33 gave a family history of thyroi…
