Hallucinations in the psychotic state: Psychoanalysis and the neurosciences compared.

In this contribution, which takes account of important findings in neuroscientific as well as psychoanalytic research, the authors explore the meaning of the deep-going distortions of psychic functioning occurring in hallucinatory phenomena. Neuroscientific studies have established that hallucinations distort the sense of reality owing to a complex alteration in the balance between top-down and bottom-up brain circuits. The present authors postulate that hallucinatory phenomena represent the outcome of a psychotic's distorted use of the mind over an extended period of time. In the hallucinator

Sleep-related psychosis.

We report the case of a 19-year-old male student with a Kleine-Levin syndrome who was referred to our sleep laboratory during an episode of hypersomnia, hypersexuality, cognitive impairment and bizarre behaviour. Owing to similar clinical symptoms, he had been misdiagnosed with schizophrenia. This had led to a placement in a facility for persons with chronic schizophrenia, antipsychotic pharmacotherapy for 4 years and side effects, including substantial weight gain and hypertension. After cessation of the antipsychotic medication, the weight and blood pressure normalised. We started a prophyla

Optimal extent of dopamine D2 receptor occupancy by antipsychotics for treatment of dopamine supersensitivity psychosis and late-onset psychosis.

Several studies have proposed an optimal dopamine D2 receptor occupancy by antipsychotics (OOc) to establish optimal pharmacological treatment of schizophrenia. However, there are limitations to the use of the OOc, especially in application to patients with treatment-resistant schizophrenia, including dopamine supersensitivity psychosis (DSP) or late-onset psychosis (LOP). It has been suggested that D2 receptor density is up-regulated by chronic treatment of antipsychotics in DSP, whereas it may be low in LOP owing to age-related reduction. In estimation of the proposed OOc, these alterations

The explosion of the present and the encapsulation of time: transference phenomena in the analysis of a psychotic patient.

The paper deals with some basic problems concerning the experience of time and space in the psychoanalytic treatment of psychotic patients. Whereas borderline patients tend to distort the experience of time and space under emotional pressure, the concepts of time and space seem to dissolve in acute psychotic states of mind. Sometimes this manifests itself in an explosion of the present, where the past is ubiquitous and the future is perceived as the end of all times. The case of a 48 year-old patient with the external diagnosis of 'paranoid-hallucinatory schizophrenia' is presented to illustra

Psychoses as the presenting manifestation of Creutzfeldt-Jakob disease in an elderly male.

The differential diagnosis of acute psychosis includes delirium, dementia and primary psychiatric disorders. A comprehensive medical evaluation is crucial to make the proper diagnosis. We report the case of a healthy elderly man who presents with acute psychosis. We describe our diagnostic evaluation based on the patient's history and physical examination, which identified Creutzfeldt-Jakob disease. Avoiding diagnostic error in patients who face such a rapidly progressive and catastrophic illness is paramount.

Neurosyphilis presenting with psychotic symptoms and status epilepticus.

The widespread use of antibiotics in recent years has considerably modified the clinical features of neurosyphilis. Presently, atypical or masked forms of this disease often occur and obscure diagnosis, despite a thorough history and clinical work-up. Here, we report a patient with neurosyphilis presenting with psychotic symptoms who then developed status epilepticus and left limb weakness. Diffusion-weighted magnetic resonance imaging showed hyperintensity involving the right parietal, occipital and temporal lobes and the thalamus. Subsequent serological and cerebrospinal fluid tests confirme

Does dopamine mediate the psychosis-inducing effects of cannabis? A review and integration of findings across disciplines.

General population epidemiological studies have consistently found that cannabis use increases the risk of developing psychotic disorders in a dose-dependent manner. While the epidemiological signal between cannabis and psychosis has gained considerable attention, the biological mechanism whereby cannabis increases risk for psychosis remains poorly understood. Animal research suggests that delta-9-tetrahydrocannabinol (THC, the main psychoactive component of cannabis) increases dopamine levels in several regions of the brain, including striatal and prefrontal areas. Since dopamine is hypothesi…

Anorexia nervosa and Wernicke Korsakoff's syndrome: atypical presentation by acute psychosis.

A 16-year-old girl was admitted to the emergency department because of acute changes in mental state such as paranoid and nihilistic delusions, confabulations, and distortions of body schema perception. Her history was compatible with anorexia nervosa in that she had lost more than 17 kg in weight over one and a half years. Her body mass index was 14. She was diagnosed with Wernicke Korsakoff's syndrome and was given intravenous thiamine at 250 mg/day. Response was dramatic for nystagmus and gait incoordination but not for other symptoms. After dosage was increased to 750 mg/day all symptoms i

An incorrect diagnosis of psychosis?

This article examines a recent claim in which a patient alleged his general practitioner had incorrectly diagnosed him as suffering from psychosis. The GP had prescribed Melleril, which was complicated by the development of pancytopaenia.

[Delusional parasitosis: case report].

A case of a 75-year-old woman suffering from delusional parasitosis is described. The patient was referred to the consultant psychiatrist by a dermatologist with the diagnosis of psychosis. In course of the psychiatric treatment the patient revealed the delusional parasitosis and received antipsychotic treatment resulting in full remission. Delusional parasitosis (Ekbom syndrome) is a condition where a person has the unshakeable mistaken belief of being infested with parasites. Patients give detailed descriptions of the activity and offending parasites: crawling, biting, burrowing. According t

Distribution of symptom dimensions across Kraepelinian divisions.

BACKGROUND: Dimensional structures are established for many psychiatric diagnoses, but dimensions have not been compared between diagnostic groups. AIMS: To examine the structure of dimensions in psychosis, to analyse their correlations with disease characteristics and to assess the relative contribution of dimensions v. diagnosis in explaining these characteristics. METHOD: Factor analysis of the OPCRIT items of 191 Maudsley Family Study patients with schizophrenia, mood disorders with psychosis, schizoaffective disorder, and other psychotic illnesses, followed by regression of disease char

Impact of regulatory changes on first- and second-generation antipsychotic drug consumption and expenditure in Italy.

BACKGROUND: In 1994 a change in drug reimbursement status was implemented in Italy according to cost-effectiveness criteria. The aim of this study was to examine the impact of these changes on the use of antipsychotic (AP) drugs. METHODS: Data concerning actual quantities of antipsychotic agents dispensed in Italy from 1995 to June 2003 were obtained from the Italian Ministry of Health. For each antipsychotic agent, the number of defined daily doses (DDDs) per 1,000 inhabitants per day was calculated, as well as the annual expenditure in Euros. RESULTS: From 1995 to June 2003 prescriptions f
