Modified calcar ratio for predicting varus collapse in proximal humerus fractures.

INTRODUCTION: Proximal humerus fractures are common, especially in older adults, and often result in complications such as varus collapse after open reduction and internal fixation (ORIF). Despite successful surgical methods, complication rates remain significant, with varus collapse being a primary cause of failure. Ensuring proper screw placement, particularly the calcar screw, is vital to prevent this issue. This study introduces a modified calcar ratio (MCR) that combines two key measurements - tip distance and calcar distance - to predict varus collapse and improve surgical outcomes. MET

Risk Factor Analysis for Growth Arrest in Paediatric Physeal Fractures-A Prospective Study.

Background: Fractures through the physis account for 18-30% of all paediatric fractures, leading to growth arrest in up to 5.5% of cases. We have limited knowledge to predict which physeal fractures result in growth arrest and subsequent deformity or limb length discrepancy. The purpose of this study is to identify factors associated with physeal growth arrest to improve patient outcomes. Methods: This prospective cohort study was designed to develop a clinical prediction model for growth arrest after physeal injury. Patients ≤ 18 years old presenting within four weeks of injury were enrolled

Do traction radiographs of distal radial fractures influence fracture characterization and treatment?

BACKGROUND: Our center evaluates all distal radial fractures with traction radiographs before splinting. Although investigations of various imaging modalities to evaluate distal radial fractures have been presented in the literature, to our knowledge the use of traction radiographs has not been well described. We hypothesized that the addition of traction radiographs to standard radiographs increases interobserver and intraobserver reliability for injury descriptions, affects the choice of treatment plan, and decreases the perceived need for computed tomography. METHODS: Radiographs for fifty

Research on the Prevention and Control of Blast Impact Pressure in Deep Holes and the Mechanism of Coordinated Extraction of Harmful Gases.

Uncontrolled carbon monoxide (CO) release after deep-hole roof pressure-relief blasting remains a safety problem because conventional shallow drainage often fails to match the strata where blast gases are generated and retained. Here, we clarify the permeability-contrast-controlled retention mechanism and propose an integrated blasting-sealing-extraction framework that couples field diagnosis at the A503 working face, dynamic fracture modeling, confined explosion tests, adsorption-diffusion analysis, and industrial validation. The method uses the critical distance between the fracture front an

TiNbSn alloy spine rods resistant to fatigue fracture and flattening.

The critical issues of fatigue fracture and flattening in the Ti-6Al-4V and CoCrMo spine rods for spine deformity correction surgery remain unresolved. To address them we propose a hypothesis that fatigue fracture stems from crystal defects and residual stresses, and flattening originates from the Bauschinger effect. To test the hypothesis, TiNbSn alloy spine rods with an apex radius of 120 mm (R120) were prepared by heating an as-swaged TiNbSn rod at 400°C for 5 h ("warm-contoured") using a rod fixing device designed to shape the rod while minimizing plastic deformation to reduce crystal defe

Biodegradable (PLGA) Implants in Pediatric Trauma: A Brief Review.

Background/Objectives: Biodegradable implants have emerged as a promising alternative to traditional metallic fixation devices in pediatric orthopedic surgery. Avoiding implant removal is especially advantageous in children, who would otherwise require a second operation with additional anesthetic and surgical risks. This study reviews the current use of poly(lactic-co-glycolic acid) (PLGA) implants in pediatric fracture fixation and evaluates how they address limitations associated with traditional hardware. Methods: A narrative review was conducted summarizing current evidence, clinical expe

Interosseus Membrane Reconstruction Following a Forearm Injury: A Case Report.

Background  Essex-Lopresti injuries are complex, frequently missed forearm/elbow injuries consisting of a fracture of the radial head with disruption of both the distal radioulnar joint (DRUJ) and the interosseous membrane (IOM). Ideal management of these injuries remains controversial, especially in relation to the IOM, due in part to underdiagnosis of IOM injury and incomplete understanding of the IOM role. Methods  A 43-year-old man sustained an open radial shaft fracture with associated radial head subluxation, coronoid base fracture, and DRUJ injury with ulnar head subluxation. He was tak

A case report of surgical treatment of traumatic drop hand by quadruple tendons transfer.

INTRODUCTION: Drop hand is the case of inability to dorsiflexion of the hand and fingers according to radial nerve palsy (complete syndrome) or PIN (Posterior Interosseous Nerve) palsy (partial syndrome). Therefore, the patient loses part of the normal function of his/her hand causing life problems especially if it is the dominant hand. Tendon transfer is the main surgical treatment. CASE REPORT: We present a case of a young male who sustained multiple traumatic war injuries especially in the left upper limb; open humerus fracture, open ulna fracture, open radius fracture with a clear drop ha

Bone Health in Children with Rheumatic Disorders: Focus on Molecular Mechanisms, Diagnosis, and Management.

Bone is an extremely dynamic and adaptive tissue, whose metabolism and homeostasis is influenced by many different hormonal, mechanical, nutritional, immunological and pharmacological stimuli. Genetic factors significantly affect bone health, through their influence on bone cells function, cartilage quality, calcium and vitamin D homeostasis, sex hormone metabolism and pubertal timing. In addition, optimal nutrition and physical activity contribute to bone mass acquisition in the growing age. All these factors influence the attainment of peak bone mass, a critical determinant of bone health an

[Current trends in surgical treatment of radial nerve injuries associated with injuries of the humerus].

Radial nerve is the most common injured peripheral nerve after bone and wrist injuries in human body. Radial nerve can be injured by force causing fracture of the humeral shaft, due to compound fracture of the bone or by direct injury with small fragments of the bone. This happen in of cases and in remaining caseswas caused by manipulation during the treatment of the fracture of the humeral shaft. Because of the possibility of spontaneous recovery, indication and timing of surgical treatment of the radial nerve still remain controversial. There is contradictory approach in the treatment of the

Effects of risedronate or alfacalcidol on bone mineral density, bone turnover, back pain, and fractures in Japanese men with primary osteoporosis: results of a two-year strict observational study.

Although osteoporosis in men is already a major public health problem, there is still a dearth of data about the effects of risedronate in male osteoporosis, especially in Japanese with primary osteoporosis. Therefore, the objective of our study was to investigate the effects of risedronate on bone mineral density (BMD), bone turnover, back pain, and fractures in these patients prospectively for two years (at baseline, three months, six months, twelve months, and twenty-four months) both longitudinally and compared with those of alfacalcidol. The subjects enrolled for this study were 66 Japane

Determinants of bone mineral density in older men.

Although osteoporosis in men is increasingly recognized as an important health issue and bone mass appears to be a major determinant of fracture, there remain few data concerning the determinants of bone mass in men. To determine the correlates of bone density in men, we studied a large group of older subjects recruited from three rural communities in the northwestern United States. Three hundred and fifty-five men over the age of 60 years (mean 71.5 +/- 7.4 years) without known disorders of mineral metabolism were recruited by community advertising. Bone mineral density was measured at the lu

Recovery of strength after reduced pediatric fractures of the forearm, wrist or hand; A prospective study.

INTRODUCTION: The way strength recovers after reduction of pediatric fractures of the upper extremity has not previously been the specific scope of research. This is remarkable, since strength measurements are often used as an outcome measure in studies on trauma of the upper extremity. The aim of this study was to evaluate how strength recovers after sustainment of fractures of the forearm, wrist or hand treated by closed or open reduction in children and adolescents in the first 6 months after trauma. How much strength is lost at 6 weeks, 3 months and 6 months after trauma, and is this loss
