Deep-Learning based Sleep Apnea Detection using SpO2 and Pulse Rate.

This work presents automated apnea event de-tection using blood oxygen saturation (SpO2) and pulse rate (PR), conveniently recorded with a pulse oximeter. A large, diverse cohort of patients (n=8068, age≥40 years) from the sleep heart health study dataset with annotated sleep events have been employed in this study. A deep-learning model is trained to detect apnea in successive 30 s epochs and performances are assessed on two independent sub-cohorts of test data. The proposed algorithm showcases the highest test performance of 90.4 % area under the receiver operating characteristic curve and 5

Positive outcome of average volume-assured pressure support mode of a Respironics V60 Ventilator in acute exacerbation of chronic obstructive pulmonary disease: a case report.

INTRODUCTION: We were able to treat a patient with acute exacerbation of chronic obstructive pulmonary disease who also suffered from sleep-disordered breathing by using the average volume-assured pressure support mode of a Respironics V60 Ventilator (Philips Respironics: United States). This allows a target tidal volume to be set based on automatic changes in inspiratory positive airway pressure. This removed the need to change the noninvasive positive pressure ventilation settings during the day and during sleep. The Respironics V60 Ventilator, in the average volume-assured pressure support

ATS Core Curriculum 2021. Adult Sleep Medicine: Sleep Apnea.

The American Thoracic Society Sleep Core Curriculum updates clinicians on important sleep topics, presented during the annual meeting, and appearing in summary here. This year's sleep core theme is sleep-disordered breathing and its management. Topics range from pathophysiological mechanisms for the association of obstructive sleep apnea (OSA) and metabolic syndrome, surgical modalities of OSA treatment, comorbid insomnia and OSA, central sleep apnea, and sleep practices during a pandemic. OSA has been associated with metabolic syndrome, independent of the role of obesity, and the pathophysiol

[Respiratory arrest during sleep. Facts and hypothyses].

The writers analyze 29 nyctohemeral recordings in 20 patients with sleep apneas. In addition to the standard polygraphy, use is made of pneumotachography and capnography. The results (central apnea with open or secondarily closed glottis; obstructive apnea with closed glottis or with expiratory valve phenomenon) raise the hypothesis of a central physiopathological mechanism common to the various types of apneas through a more or less dissociated inhibition of the activity of the respiratory, laryngeal, intercostal, and diaphragmatic muscles:--in central apnea: inhibition of the thoracoabdomina

A comparison of tracheal breath sounds, airflow, and impedance pneumography in the detection of childhood apnea.

Impedance respiratory monitoring is not capable of detecting obstructive apneas. We compared a microphone breath sound detector, coupled to the chest wall, with a standard impedance device in 10 sleeping infants and children in order to determine the ability of the breath sound detector to detect normal respirations and central and obstructive apneas. Airflow was used as a standard for all measurements. No difference was found between the breath sound detector and impedance device techniques in the detection rate of either normal respirations or central apneic intervals. There was no statistic

Effects of CPAP on Language Recovery in Post-Stroke Aphasia: A Review of Recent Literature.

Obstructive sleep apnea is highly prevalent in the post-stroke population, and has been shown to affect cognitive, neurological, and functional status. Continuous positive airway pressure (CPAP) treatment is one of the most effective interventions for obstructive sleep apnea, but compliance is often low due to confounding effects of co-occurring conditions, side effects of treatment titration procedures, and individual patient personality characteristics, perceptions, and social factors. Current research suggests that CPAP treatment for obstructive sleep apnea is not associated with significan

The patients were observed for a period of 2 to 84 months (average: 20,8 months). Postoperatively, 80 % showed a disappearance or great reduction of snoring intensity. Other symptoms of obstructive sleep apnea syndrome such as apneas, tiredness during the day and deterioration of sleep quality also improved markedly. In ten out of 13 patients with a demonstrated sleep apnea syndrome, the apneas disappeared or became noticeably reduced (in sev

Central sleep apnea in pregnant women with sleep disordered breathing.

PURPOSE: Physiologic changes in the cardiac, respiratory, and renal systems in pregnancy likely impact ventilatory control. Though obstructive sleep apnea and snoring are common in the pregnant population, the predisposition to central respiratory events during sleep and the prevalence of such events is less well studied. The aim of this study was to assess the presence of central apneas during sleep in pregnant women and non-pregnant controls suspected of sleep disordered breathing. METHODS: Twenty-five pregnant women referred for polysomnography for sleep disordered breathing were compared

Walking the fine line between OSA and aging.

Obstructive Sleep Apnea (OSA) is the most common sleep- related breathing disorder. In recent years, evidence have shown that patients with OSA may have this disorder for different reasons, with different symptoms and comorbidities. Therefore, treatment should be individualized [1]. This has led to a growing interest in the characterization of the disease into phenotypes. OSA in elderly patients is often a challenge for clinician in terms of diagnosis, as symptoms may be masked, but also for treatment in terms of efficacy and adherence. However, aging is a pathophysiological factor that predis

Obesity hypoventilation syndrome: a review of epidemiology, pathophysiology, and perioperative considerations.

Obesity hypoventilation syndrome (OHS) is defined by the triad of obesity, daytime hypoventilation, and sleep-disordered breathing without an alternative neuromuscular, mechanical, or metabolic cause of hypoventilation. It is a disease entity distinct from simple obesity and obstructive sleep apnea. OHS is often undiagnosed but its prevalence is estimated to be 10-20% in obese patients with obstructive sleep apnea and 0.15-0.3% in the general adult population. Compared with eucapnic obese patients, those with OHS present with severe upper airway obstruction, restrictive chest physiology, blunt

Sleep deficiency and motor vehicle crash risk in the general population: a prospective cohort study.

BACKGROUND: Insufficient sleep duration and obstructive sleep apnea, two common causes of sleep deficiency in adults, can result in excessive sleepiness, a well-recognized cause of motor vehicle crashes, although their contribution to crash risk in the general population remains uncertain. The objective of this study was to evaluate the relation of sleep apnea, sleep duration, and excessive sleepiness to crash risk in a community-dwelling population. METHODS: This was a prospective observational cohort study nested within the Sleep Heart Health Study, a community-based study of the health con

Multicomponent Analysis of Sleep Using Electrocortical, Respiratory, Autonomic and Hemodynamic Signals Reveals Distinct Features of Stable and Unstable NREM and REM Sleep.

A new concept of non-rapid eye movement (NREM) and rapid eye movement (REM) sleep is proposed, that of multi-component integrative states that define stable and unstable sleep, respectively, NREMS, NREMUS REMS, and REMUS. Three complementary data sets are used: obstructive sleep apnea (20), healthy subjects (11), and high loop gain sleep apnea (50). We use polysomnography (PSG) with beat-to-beat blood pressure monitoring, and electrocardiogram (ECG)-derived cardiopulmonary coupling (CPC) analysis to demonstrate a bimodal, rather than graded, characteristic of NREM sleep. Stable NREM (NREMS) is

Age related disparities in sleep apnea diagnosis using a wearable device: Implications of 4% vs. 3% hypopnea scoring criteria.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) diagnosis relies on the Apnea-Hypopnea Index (AHI), with discrepancies arising from the 3% and 4% desaturation criteria. This study investigates age-related variations in OSA severity classification, utilizing data from 1201 adult patients undergoing Home Sleep Apnea Testing (HSAT) with SleepImage Ring@. METHODS: The study employs Bland-Altman analysis to compare AHI values obtained with the 3% and 4% desaturation criteria. Age-stratified analysis explores discrepancies across different age groups. RESULTS: The analysis reveals a systematic bia

Bilevel positive airway pressure worsens central apneas during sleep.

STUDY OBJECTIVES: While most patients with sleep-disordered breathing are treated with continuous positive airway pressure (CPAP), bilevel positive airway pressure (BLPAP) is often used. Having observed that BLPAP therapy increased central apneas in some of our patients undergoing sleep studies, we conducted this study to evaluate the effects of BLPAP. DESIGN: Retrospective analysis of all sleep studies performed in an outpatient sleep center that used BLPAP over a 2-year period. We assessed the incidence and frequency of events during rapid eye movement (REM) sleep and non-REM sleep during b

Interhemispheric sleep depth coherence predicts driving safety in sleep apnea.

Obstructive sleep apnea is associated with increased risk of car crashes; however, conventional measures of sleep apnea severity do not clearly identify those individuals who are at greatest risk. Here we tested whether, among individuals with sleep apnea, those with reduced interhemispheric sleep depth coherence, measured by correlation between right and left hemisphere odds ratio product, are at greater risk. The sample was derived from the Sleep Heart Health Study, a prospective observational cohort study, and included 1,378 adults with sleep apnea. The occurrence of a car crash was ascerta

Sleep-stage- and event-specific interpretation during concurrent natural sleep endoscopy and polysomnography.

PURPOSE: Polysomnography (PSG) defines sleep stage, respiratory event type, and respiratory effort, but does not directly identify the visible anatomic correlate of individual events. We examined whether concurrent natural sleep endoscopy (NSE) with PSG could support event-level interpretation of selected sleep-disordered breathing events. METHODS: Four adults were selected from archived synchronized NSE-PSG recordings acquired between 2007 and 2009. Cases were chosen to illustrate interpretive scenarios in which PSG context changed, refined, or constrained visible airway interpretation. RES

Sleep health in the older adults: Architecture, circadian changes, and common sleep disorders.

Population aging accelerates globally, increasing sleep-related complaints among older adults. These disorders substantially impact cognitive function, physical health, and quality of life. This narrative review examines age-related changes in sleep architecture, alterations in circadian rhythm, common sleep disorders, and modifiable behavioral factors in older populations. We searched PubMed, Web of Science, and Google Scholar through December 2025 for studies involving adults aged ≥ 65 years focusing on sleep patterns, architecture, circadian rhythms, and disorders. Normal aging involves red

How to study sleep apneas in mouse models of human pathology.

Sleep apnea, the most widespread sleep-related breathing disorder (SBD), consists of recurrent episodes of breathing cessation during sleep. This condition can be classified as either central (CSA) or obstructive (OSA) sleep apnea, with the latest being the most common and toxic. Due to the complexity of living organisms, animal models and, particularly, mice still represent an essential tool for the study of SBD. In the present review we first discuss the methodological pros and cons in the use of whole-body plethysmography to coupling respiratory and sleep measurements and to characterize CS
