Integrated treatment for the survivor of childhood trauma who is chemically dependent.

A substantial percentage of individuals who are chemically dependent also suffer from posttraumatic stress disorder and other survivor syndromes related to childhood abuse. The synergism of these multiple conditions creates unique treatment challenges. An enhanced, integrated treatment approach increases the chances of a successful clinical outcome. This article discusses a treatment model for chemical dependence and trauma-related syndromes that blends mental health and 12-Step and other chemical dependence treatment notions into an integrated approach that treats both disorders simultaneousl

Comorbidity of psychiatric and alcohol/drug disorders: interactions and independent status.

Dual diagnosis as interpreted in clinical psychiatric practice is often not what the term means-two independent disorders. Commonly, addiction disorders are attributed to self medicating of "underlying" psychiatric disorders, there by, a contingency status. However, studies and clinical addiction practice show that drug and alcohol addiction must be afforded an independent status before accurate assessment and assignment of interactions in dual diagnosis can be made in clinical conditions. Studies also show rates for psychiatric comorbidity are low in addiction populations and rates for addict

Drug and alcohol dependence and psychiatric populations: the need for diagnosis, intervention, and training.

The comorbidity of psychiatric disorders and alcohol and drug dependence is high. The prevalence rate for each disorder is high as determined by studies in general and patient populations. Moreover, the severity of the course is likely to be greater when the two disorders are combined, prompting the attention of clinicians and researchers. Of importance, the intoxication and withdrawal from alcohol and drugs produce psychiatric symptoms and syndromes that can mimic psychiatric disorders. The prevalence rates for comorbidity of both categories of disorders vary according to the populations stud

Opportunities for treatment of psychoactive substance use disorders with serotonergic medications.

The authors review both the preclinical and the clinical evidence for a role of serotonin (5-HT) systems in the regulation of drug-taking behavior. Animal studies show that pharmacologic treatments that enhance 5-HT function, notably selective reuptake inhibitors, reduce the self-administration of a variety of substances of abuse, including ethanol and cocaine. These treatments also tend to suppress consummatory behavior in general. In contrast to the broad spectrum of suppression following 5-HT enhancement, selective antagonists at the 5-HT3 receptor subtype have been reported to reduce ethan

Binge-eating, self-induced vomiting and laxative abuse: a community study.

Following a television documentary on bulimia nervosa, people who thought that they had this type of eating problem were asked to complete a confidential questionnaire. 579 women who fulfilled self-report diagnostic criteria for bulimia nervosa were thereby identified. These women closely resembled patients with bulimia nervosa, although the age range was wider. They had grossly disturbed eating habits and almost half vomited at least daily. Laxative abuse was also common. Although almost two-thirds had been overweight in the past, the majority had a weight within the normal range. A minority

A survey of attempted suicide in Shropshire.

The number of patients who are referred for a psychiatric opinion as cases of attempted suicide seems to be increasing. One hundred and seven patients, 70 females and 37 males, were referred to the author during the period of the one year study, all but three were cases of deliberate self-poisoning. Only a minority of the group, three males and 11 females, regretted that the attempted had not been successful and all were admitted to the psychiatric hospital. A large majority of the patients were provisionally diagnosed as suffering from a reactive depression, consequent in many instances upon

Recent developments in opiate research and their implications for psychiatry.

Considerable progress in opiate research has been made during the last few years regarding the identification and localization of opiate receptors in vitro and in vivo, the analysis of drug-receptor interactions and the characterization of an endogenous ligand of the opiate receptor. There is little evidence that effects induced by chronic exposure to opiates - development of tolerance and dependence -are due to changes in opiate receptor mechanisms; it is supposed that the adaptive changes occur mainly in the chain of events triggered by the drug-receptor interaction. Such changes may be dire
