Antegrade intramedullary K-wire fixation for distal radial fractures.

Manual reduction and antegrade intramedullary K-wire fixation were performed for 29 selected Colles-type distal radial fractures in 29 patients. Severely comminuted intra-articular fractures and Barton's fracture were not included in this study. The tips of the K-wires supported an articular surface of the distal radius. Except for 1 case of skin irritation in the forearm, there were no complications related to tendon or nerve injuries or reflex dystrophy. There was no pin loosening, but protrusion of sharp tips of K-wires into the wrist joint occurred in 3 patients. Average volar tilt, radial

Dynamic extension assist splinting of acute central slip lacerations.

The management of zone III extensor tendon injuries remains a challenging clinical problem for both the surgeon and therapist. Current treatment protocols include prolonged immobilization, controlled mobilization, or limited early dynamic splinting, but overall results are often less than optimal. This retrospective study describes an alternative treatment approach for acute central slip lacerations consisting of initial immobilization of the PIP joint in absolute 0 degrees extension for an average of 3.5 weeks followed by active flexion of the PIP joint in a finger-based dynamic extension ass…

Staged major limb replantation: a concept to minimize the risk in replantation of the lower extremity.

The replantation of large limb segments presents two major problems: first, a general danger to survival because of major accompanying injuries and additional ischemia-reperfusion injury; second, local soft-tissue damage at the amputation site. Successful replantation can be compromised by infection, vessel thrombosis, and disturbed bone healing. Possible risk reduction may be accomplished by the concept of a two-staged replantation. A brief primary emergency procedure involving bone resection, osteosynthesis, and revascularization (with the goal of limb survival) is followed by a second proce

[Preventive antibiotic treatment in cases of complicated injuries of the extremities. A questionnaire study].

A questionnaire about prophylactic antibiotic treatment of open bone, tendon or joint lesions in the extremities was sent to the 83 casualty departments in Denmark. The questionnaire was answered by 56 casualty departments which treated 72% of the orthopaedic surgical injuries in Denmark. In cases of lesions of the hands and feet, 23% of the patients were treated in casualty departments where prophylactic antibiotic therapy was rarely employed while 24% were treated in casualty departments where prophylactic antibiotic therapy was always employed. In cases of lesions proximal to the wrist or a

Palmar dislocation of the proximal interphalangeal joint.

Fifteen patients with palmar dislocations of the proximal interphalangeal (PIP) joint were reviewed 6 to 49 months after treatment (average 17.8 months). Disruption of the extensor mechanism, palmar plate, and one collateral ligament was found in all patients. The loss of static and dynamic joint support caused palmar subluxation, malrotation, and a boutonnière deformity. Two dislocations were irreducible, and three were associated with dorsal avulsion fractures from the middle phalanx. The serious nature of the injuries from this dislocation was initially unrecognized, and most patients were

Arthroscopic-Assisted Suture Tape-Reinforced Triangular Fibrocartilage Complex Repair and Distal Radioulnar Ligament Augmentation Using a Modified Adams-Berger Technique.

Distal radioulnar joint instability secondary to triangular fibrocartilage complex (TFCC) injury remains a major challenge in wrist surgery. Traditional tendon graft-based reconstruction techniques, such as the Adams-Berger procedure, though effective, require extensive soft tissue dissection and tendon harvesting, resulting in significant surgical morbidity. Meanwhile, isolated arthroscopic TFCC repairs often fail to achieve durable stabilization in chronic or degenerative cases due to poor tissue quality and insufficient fixation strength. To overcome these limitations, we present a modified

Diagnostic and Interventional US of the Wrist and Hand: Quadrant-based Approach.

Diagnostic and interventional US of the wrist and hand can be challenging due to the small size and superficial location of structures and various disorders that need to be considered. A quadrant-based approach (volar, ulnar, dorsal, and radial) provides a rational method for performing a focused examination and joint positioning during both diagnostic imaging and intervention. Volar wrist disorders primarily involve the median nerve and the digital flexor system comprised of the flexor tendons and pulleys. The ulnar nerve and extensor carpi ulnaris tendon are chiefly responsible for ulnar-sid

Glenoid osteotomy with various tendon transfers for brachial plexus birth palsy: clinical outcomes.

BACKGROUND: Posterior shoulder dislocation is one of the disabling complications of brachial plexus birth injury (BPBI), and various treatment options including capsule and surrounding muscles release for open reduction, humeral derotational osteotomy, and tendon transfers have been recommended to manage it. In the present study, we aimed to determine the clinical outcome of open reduction with soft tissue release, tendon transfer, and glenoid osteotomy in patients with BPBI and posterior shoulder dislocation or subluxation. METHODS: From 2018 to 2020, 33 patients who underwent open reduction

Anti-Claw Procedure with Modified Dynamic Flexor Digitorum Superficialis Transfer for Ulnar Nerve Palsy: Our Experience.

INTRODUCTION: The ulnar nerve palsy is a distressing injury, resulting in clawing of hand. The cause of clawing is due to paralysis of the interosseous muscles in the presence of functioning long extensor and long flexors of fingers. Various methods have been proposed to correct this deformity which include both static and dynamic procedures. In this study, we share our experience with flexor digitorum superficialis tendon transfer using Zancolli's modification for anti-claw correction. MATERIALS AND METHODS: It was a retrospective case series study. A record of 53 patients was included in th

Point-of-care ultrasound in musculoskeletal field.

The musculoskeletal field is relatively new in point-of-care ultrasound (POCUS), a modality that has been gaining significance with the evolution of ultrasound (US) equipment and high-frequency probes. Images have become progressively clearer over the past decade, making it possible to diagnose many pathological conditions without exposure to radiation. Ultrasonography reveals many soft tissue pathologies that cannot be visualized using plain radiographs, which have historically been the first choice for the evaluation of musculoskeletal disorders. US saves money and time compared to magnetic

Dual Acting Carbon Monoxide Releasing Molecules and Carbonic Anhydrase Inhibitors Differentially Modulate Inflammation in Human Tenocytes.

Sustained oxidative stress and inflammation have been reported as the major factors responsible for the failure of tendon healing during rotator cuff tears (RCTs) and rotator cuff disease (RCD). Although, their therapeutic management remains still challenging. Carbonic anhydrases (CAs) are involved in many pathological conditions, and the overexpression of both CA9 and 12 in inflamed joints has been recently reported. Consequently, a selective CA9/12 inhibition could be a feasible strategy for improving tendon recovery after injury. In addition, since carbon monoxide (CO) has been proven to ha

Mechanobiology in Tendon, Ligament, and Skeletal Muscle Tissue Engineering.

Tendon, ligament, and skeletal muscle are highly organized tissues that largely rely on a hierarchical collagenous matrix to withstand high tensile loads experienced in activities of daily life. This critical biomechanical role predisposes these tissues to injury, and current treatments fail to recapitulate the biomechanical function of native tissue. This has prompted researchers to pursue engineering functional tissue replacements, or dysfunction/disease/development models, by emulating in vivo stimuli within in vitro tissue engineering platforms-specifically mechanical stimulation, as well

A multi-chamber tissue culture device for load-dependent parallel evaluation of tendon explants.

BACKGROUND: Injuries in the musculoskeletal system, such as tendon and ligament ruptures, are challenging to manage and often require surgical reconstructions with limited long-term success. Thus, characterizations of these tissues are urgently needed to better understand cellular mechanisms that regulate tissue homeostasis and healing. Explant culturing systems allow for ex vivo analysis of tissues in an environment that mimics the native microenvironment in vivo. METHODS: Collaborative efforts within our institution facilitated the establishment of a novel explant culturing system. Tissue s

Functional outcomes of distal triceps tendon repair comparing transosseous bone tunnels with suture anchor constructs.

BACKGROUND: Distal triceps tendon ruptures are relatively rare. Few studies have investigated functional outcomes after repair. There is no consensus on fixation methods for this injury. The purpose of this study was to compare the functional outcomes and the reoperation rates after distal triceps tendon repairs using transosseous tunnels and suture anchors. METHODS: A multicenter, retrospective review of all primary triceps repairs done between 2006 and 2015 was performed. Patients were included if they had a minimum of 2 years of follow-up. Intraoperative data recorded included repair metho

Traumatic Supraspinatus Tears in Patients Younger Than 25 Years.

Traumatic rotator cuff tears in patients younger than 25 years are rare events, with few reports in the literature. When compared with the more mature shoulder, the young, healthy supraspinatus tendon is a robust tendon that is able to absorb a significant amount of energy before tendon failure. Therefore, the diagnosis of a rotator cuff tear can be often overlooked in this population due to the patient's age. This is a report of traumatic supraspinatus repairs in patients younger than 25 years. Nine patients younger than 25 years were identified with a posttraumatic supraspinatus tear as visu

Surgical management of chronic boutonniere deformity.

PURPOSE: Boutonniere deformity is caused by damage to the central slip of the extensor tendon hood with secondary palmer migration of the lateral bands. Accordingly, patients complain of disfigurement and impairment of function due to hyperextension of their DIP. The aim of this study is to evaluate the results of surgical treatment of chronic boutonniere deformity by using a modified technique. PATIENTS AND METHODS: Twelve patients with posttraumatic boutonniere deformity were available for follow up as a retrospective study. They were treated by release of the extensor expansion proximal to

[Occupationally-induced carpal tunnel syndrome].

In Western Europe the Carpal-tunnel syndrome (CTS) is rarely regarded as a work injury, but in the USA this syndrome is usually considered to be work-related compression of a peripheral nerve, and the labour unions are already concerned about it. The median nerve can be squeezed by the flexor tendons of the index and middle fingers. These tendons pass dorsally from the median nerve through the carpal canal, where the nerve is subject to pressure when the tendons stretch whilst the wrist is flexed. Measurements in the carpal canal confirm this mechanism and show that, whilst grasping with the m

Endoscopic Flexor Hallucis Longus Transfer for Chronic Noninsertional Achilles Tendon Rupture.

BACKGROUND:: Operative management of chronic Achilles tendon ruptures is challenging, and numerous techniques have been described. Risk of infection and wound breakdown have been described after open techniques, and minimally invasive methods have been proposed to avoid them. The aim of this study was to describe the clinical and radiological results obtained after endoscopic flexor hallucis longus (FHL) tendon transfer in patients with chronic Achilles tendon rupture. METHODS:: Between 2012 and 2015, a total of 22 patients were endoscopically treated for chronic Achilles tendon rupture. Mean
