Adult flatfoot/posterior tibial tendon dysfunction: outcomes analysis of surgical treatment utilizing an algorithmic approach.

A new classification scheme is presented along with an algorithmic surgical approach to the treatment of posterior tibial tendon dysfunction (PTTD). This classification scheme treats as separate entities the soft-tissue and osseous components of PTTD. The soft-tissue pathology is staged from tendinitis to tendinosis to that of complete rupture. The osseous element is graded as no planar deformity, reducible planar deformity, or rigid planar deformity. The stage of soft-tissue pathology is then combined with the grade of the osseous condition, leading to a comprehensive surgical treatment plan.

[Functional results after suture repair in ruptures of the long biceps tendon with special consideration of subacromial impingement].

Operative treatment for ruptures of the long biceps tendon still is discussed controversially. In the present literature the keyhole-technique is recommended according to favourable biomechanical conditions. In recent years refixation to the short biceps tendon was preferred. Now it is supposed that this technique may provoke subacromial impingement considering the loss of depression function of the long biceps tendon to the humeral head. Between 1980 to 1991 83 patients with rupture of the long biceps tendon were treated operatively by refixation to the short head. 28 patients were investigat

High-field and low-field MR imaging of superior glenoid labral tears and associated tendon injuries.

OBJECTIVE: The purpose of this study was to determine the performance characteristics of high-field and low-field MR imaging for the diagnosis of a glenoid superior labral anteroposterior (SLAP) tear. MATERIALS AND METHODS: High-field (n = 46) or low-field (n = 21) MR imaging was performed on 41 patients with SLAP tears and 26 patients with normal superior labra. The superior labrum was classified into one of four types on the basis of patterns of intralabral signal intensity. The relative frequency of rotator cuff tears and long head of the biceps tendinopathy was also assessed. RESULTS: Fo

[Simultaneous bilateral spontaneous rupture of the quadriceps tendon. Apropos of a case of a hemodialysis patient with hyperparathyroidism and review of the literature].

The bilateral simultaneous spontaneous rupture of the quadriceps tendons is a rare condition. Of the pathologies associated with this condition, in the literature, chronic renal failure with hyperparathyroidism is of special interest. In this context, the frequency of this lesion is related to the chronicity of the renal disease. We report a case treated surgically with a good functional result in a patient under dialysis with hyperparathyroidism.

Reconstruction for chronic lateral ankle instability using the palmaris longus tendon: is reconstruction of the calcaneofibular ligament necessary?

The palmaris longus tendon was used to reconstruct the anterior talofibular ligament (ATFL) in 27 ankles with chronic lateral instability. The mean age of the patients at surgery was 23 years, and the follow-up was more than 2 years. The functional evaluation showed excellent or good results in all ankles. Twenty-seven ankles were divided into two groups according to operative findings: group A consisted of 11 ankles with old isolated injury of the ATFL, and group B consisted of 16 ankles with old combined injuries of the ATFL and the calcaneofibular ligament. There were no significant differe

The posterior tibial tendon and the tarsal sinus in rheumatoid flat foot: magnetic resonance imaging of 40 feet.

OBJECTIVE: To investigate the role of the posterior tibial tendon in the flat foot deformity of rheumatoid arthritis (RA). METHODS: Eleven patients with hyperpronated feet and 9 without hyperpronation underwent magnetic resonance imaging (MRI) of the feet and ankles. Radial oblique double echo images provided cross sectional views of the posterior tibial tendon as it coursed behind and under the medial malleolus and inserted beneath the midfoot. RESULTS: Complete tears (Type III) of the posterior tibial tendon were seen in one patient with hyperpronation and in one without hyperpronation. Ho

Why do some Friedreich's ataxia patients retain tendon reflexes? A clinical, neurophysiological and molecular study.

Among 101 patients homozygous for GAA expansion within the X25 gene, 11 from 8 families had Friedreich's ataxia with retained reflexes in the lower limbs (FARR). These patients had a lower occurrence of decreased vibration sense, pes cavus, and echocardiographic signs of left ventricular hypertrophy than the 90 FA patients with areflexia. The mean age at onset was significantly later (26.6+/-11.4 vs. 14.2+/-6.9 years), and the mean size of the smaller allele was significantly less (408+/-252 vs. 719+/-184 GAA triplets) in FARR patients. The neurophysiological findings were consistent with mild

Factors affecting the rectus femoris-patellar tendon Q-angle, measured using a computed tomographic scan.

We devised a method to measure the Q-angle (the angle between the rectus femoris and the patellar ligament), using computed tomography (CT), in 1993. In this study, I investigated the lateral shift at each point forming the angle as factors affecting the Q-angle. The study group consisted of 83 patients with recurrent patellar dislocation (83 lower limbs) and a control group of 55 healthy people (55 lower limbs). A lateral shift was found in the anterosuperior iliac spine (ASIS) in the dislocation group compared with the control group (P < 0.01). A lateral shift was also found in the anteroinf

Early onset cerebellar ataxia with retained tendon reflexes: foot deformity in a first grade family member.

Early onset cerebellar ataxia with retained tendon reflexes (EOCA) is a clinical syndrome characterised by progressive cerebellar ataxia with an onset before the age of 25 years and a wide spectrum of associated features. It is distinguished from Friedreich's ataxia (FA) mainly by the preservation of tendon reflexes, a better prognosis, and the absence of GAA expansion in the frataxin gene. Although EOCA is thought to be a hereditary disorder with an autosomal recessive mode of inheritance, genetic heterogeneity might underlie the spectrum of clinical features. In this case report we describe

Passive tension in the supraspinatus musculotendinous unit after long-standing rupture of its tendon: a preliminary report.

Incomplete functional recovery after rotator cuff surgery can be caused by rerupture or incomplete restoration of the contractile properties of the muscle-tendon-bone unit. We measured the passive tension generated in the supraspinatus musculotendinous unit at the time of repair of the supraspinatus tendon performed for the treatment of long-standing rupture in four patients and compared our results with the values of an intact musculotendinous unit. In stepwise elongation from 10 to 20 mm, passive tension increased by a factor of 2.2 +/- 0.4 in the study group. In the control case passive ten

Stabilization of the subluxed second metatarsophalangeal joint: flexor tendon transfer versus primary repair of the plantar plate.

Surgical treatment of the subluxed second metatarsophalangeal joint (MTPJ) has been a consistently frustrating problem for the foot and ankle surgeon. The plantar plate is the principal stabilizing structure of the second MTPJ and compromise to its integrity has been implicated as the cause of the subluxed second toe. Flexor tendon transfer has been reported as the mainstay of treatment to stabilize the subluxed second MTPJ. Recently, primary repair of the plantar plate has been advocated, yet no research exists comparing it to flexor tendon transfer. Eight freshly frozen lower extremity cadav

A review of the role of magnetic resonance imaging in the evaluation of shoulder impingement syndrome and rotator cuff tendon tears.

The objective of this paper is to present a review of the role of magnetic resonance imaging in the evaluation of shoulder impingement syndrome and rotator cuff tendon tears. This imaging modality is effective in demonstrating both the soft tissue and bony abnormalities associated with structural shoulder impingement which include subacromial-subdeltoid bursitis, supraspinatus tendinopathy, rotator cuff tendon tears, subacromial osteophytic spurs and acromioclavicular joint capsular hypertrophy and osteophytosis. The detection of a subacromial osteophytic spur is considered specific for should

Resorbable and non-resorbable augmentation devices for tenorrhaphy of xenografts in extensor tendon deficits: 12 week study.

Resorbable (poly-L-lactide) and non-resorbable (polyethylene terephathalate) tendon augmentation devices (TAD) in conjunction with a pericardial adhesion barrier, were designed to strengthen tenorrhaphies and were evaluated in an ovine extensor tendon deficit model in a short term study. Fifteen centimetres of tendon were resected and replaced with kangaroo tail tendon xenografts that had been cross-linked with 0.075% glutaraldehyde (GA) at 4 degrees C for one or seven days. Compared with tenorrhaphies performed with Kessler sutures alone, both types of TAD were more effective at preventing te

Management of patella fractures associated with central third bone-patella tendon-bone autograft ACL reconstructions.

Patella fractures are an acknowledged complication of cruciate ligament reconstructions that use bone-tendon-bone autografts. Two fracture patterns are seen: a nondisplaced longitudinal patellar fissure fracture recognized interoperatively and the classic displaced transverse fracture that disrupts the extensor mechanism. Rigid fixation techniques are described for each fracture type that minimally affect rehabilitative efforts.

Rupture of the flexor pollicis longus tendon after 30 years due to migration of a retained foreign body.

Delayed rupture of a flexor tendon in the hand due to the presence of a retained foreign body is rare. We present the case of a late flexor pollicis longus rupture 30 years after traumatic implantation of a glass fragment. The foreign body had migrated distally a distance of 4.5 cm from the site of the original injury and eroded into the flexor tendon sheath. Thumb function was restored with a flexor digitorum superficialis tendon transfer from the ring finger with excellent results.

Popliteal tendon transposition for stabilization of the cranial cruciate ligament deficient stifle joint in dogs: an experimental study.

Popliteal tendon transposition was performed in five dogs with surgically induced cranial cruciate ligament rupture. After a lateral approach to the stifle joint, the popliteal tendon was severed distal to the sesamoid bone and transposed cranially onto the tibial crest to mimic the sagittal orientation of the cranial cruciate ligament. The origin of the popliteal tendon on the lateral femoral condyle was preserved. Lameness was not clinically detectable 2 months after surgery. At 6 months postoperatively, there was minimal radiographic and histopathologic evidence of degenerative joint diseas
