Self-inflicted partial epiphyseolysis of the distal femur-a case report.

INTRODUCTION: Injuries to the epiphyseal plate are of great concern as they can affect bone growth. Although epiphyseal fractures are common in adolescents, fractures of the distal femoral epiphyseal plate are rare. CASE PRESENTATION: We present a case of a Salter-Harris type 1 fracture of the distal epiphysis of the femur that was self-inflicted by a patient with paraplegia due to spina bifida. The patient was brought to the pediatrician's attention during a routine checkup with an apparent swelling of the right thigh. Upon presentation, we performed a radiograph and an additional MRI, which

Clinical study on the application effect of improved polyetheretherketone cranial plate in cranioplasty.

This study evaluates the clinical outcomes associated with the use of an improved polyetheretherketone (PEEK) cranial plate in cranioplasty surgery. A total of 104 patients were involved, with significant findings revealing a reduced incidence of postoperative adverse reactions in the improved PEEK group (28.85%) compared to the conventional PEEK group (50.00%, P = 0.027). Patient satisfaction rates were markedly higher in the improved PEEK cohort (P < 0.05). Although the medical expenses for the enhanced PEEK group were greater (¥ 144 600 ± 21 200 vs ¥ 127 400 ± 20 100, P < 0.05), there were

Ameloblastic Fibroma: A Case Report of a Rare Presentation Illustrating a Diagnostic Challenge.

This case report emphasizes the diagnostic role of cone-beam computed tomography (CBCT) in identifying a rare pediatric case of ameloblastic fibroma (AF), comprising approximately 2% of odontogenic tumors. The 10-year-old patient initially presented with painless left mandibular posterior gingival swelling, resembling radicular or dentigerous cysts in clinical and conventional radiographic assessments. However, CBCT revealed a distinct periapical radiolucency linked to deciduous molar roots and an unerupted premolar, exposing mandibular lingual plate thinning and buccal plate disruption. The l

Methotrexate use in juvenile idiopathic arthritis and its impact on growth plate development: A prospective observational cohort study.

Juvenile Idiopathic Arthritis (JIA) is a chronic inflammatory disease that can disrupt normal skeletal growth in children. Methotrexate (MTX) is widely used as a disease-modifying therapy in JIA, has uncertain effects on the developing growth plate and bone elongation. Hence, we enrolled 40 children with JIA, dividing them equally into MTX-exposed and non-MTX groups and evaluated growth plate morphology (via imaging), growth velocity (using height-for-age Z-scores) and biochemical markers (IGF-1, ALP, Vitamin D) over 12 months. The MTX group showed significantly reduced growth velocity (ΔZ = 0

Biocreative Alveolar Molding Plate Treatment (BioAMP) for neonatal unilateral cleft lip and palate with excessively wide alveolar cleft and maxillary arch width.

Since its inception in Europe in the 1950s, alveolar molding treatment for neonates with complete cleft lip and palate has undergone significant evolution in both design and application methodology, demonstrating effectiveness in normalizing the alveolar cleft and nasal shape. However, excessively wide alveolar clefts accompanied by disproportionately wide total maxillary arch pose significant challenges when utilizing conventional alveolar molding methods involving cyclical adding and grinding of acrylic on molding plates. The current report introduces a novel alveolar molding method named Bi

Peri-Implant Distal Radius Fracture: Proposal of a New Classification.

A peri-implant fracture near the volar plate of the distal radius represents a rarity and can be associated with a mechanical failure of the devices. A literature review was conducted including all fractures that occurred around a volar wrist plate, which could be associated with an ulna fracture. All articles published until December 2021 were considered according to the guidelines presented in the PRISMA Statement. The search was conducted with the PubMed electronic database, Cochrane Database of Systematic Reviews, Medline, Embase, and Google Scholar. Only nine cases of these fractures were

Atypical Ulnar Fracture in an Older Woman with Osteoporosis with a Five-Year History of Denosumab Treatment: A Case Report.

Atypical ulnar fracture (AUF) is a rare condition that may occur in patients treated with bisphosphonates. We describe the clinical course of a patient who had an AUF during treatment with denosumab. An 86-year-old woman treated with denosumab for 5 years experienced a complete transverse fracture of the proximal left ulna after falling from a standing height. Intraoperative photographs showed cortical thickening around the fracture site. Therefore, we diagnosed the patient with AUF and performed internal fixation with a locking plate. However, a re-fracture occurred 2.5 months after surgery,

Valproic Acid Impacts the Growth of Growth Plate Chondrocytes.

A range of bone abnormalities including short stature have been reported to be associated with the use of antiepileptic drugs (AEDs) in children. Exactly how AEDs impact skeletal growth, however, is not clear. In the present study, rat growth plate chondrocytes were cultured to study the effects of AEDs, including valproic acid (VPA), oxcarbazepine (OXA), levetiracetam (LEV), lamotrigine (LTG), and topiramate (TPM) on the skeletal growth. VPA markedly reduced the number of chondrocytes by apoptosiswhile other AEDs had no effect. The apoptosis associated noncleaved and cleaved caspase 3, and ca

Respiratory chain inactivation links cartilage-mediated growth retardation to mitochondrial diseases.

In childhood, skeletal growth is driven by transient expansion of cartilage in the growth plate. The common belief is that energy production in this hypoxic tissue mainly relies on anaerobic glycolysis and not on mitochondrial respiratory chain (RC) activity. However, children with mitochondrial diseases causing RC dysfunction often present with short stature, which indicates that RC activity may be essential for cartilage-mediated skeletal growth. To elucidate the role of the mitochondrial RC in cartilage growth and pathology, we generated mice with impaired RC function in cartilage. These mi

Unilateral, Multifocal Fracture of Radial with Ipsilateral Midshaft Ulna Fracture - Two Such Unusual and Rarely Reported Cases.

INTRODUCTION: Diaphyseal fractures of both bones of the forearm are commonly encountered in clinical practice. Segmental radius shaft fractures are, however, less commonly seen. We hereby report two cases of segmental complex radius fracture with mid-shaft ulna fracture. Such type of cases are reported previously in children but rarely in adults. CASE REPORT: In case-1, comminuted radius shaft fracture was fixed with square nails and ulna was fixed with Recon plate. In case-2, plating was done for radius mid-shaft, K-wiring for distal radius, and ulna was stabilized with a square nail. Detail

Use of a retrograde femoral nail in a patient with McCune-Albright syndrome.

McCune-Albright Syndrome is a rare condition characterized by endocrine abnormalities, precocious puberty, pigmented skin lesions and polyostotic fibrous dysplasia with consequent fractures and limb deformity. Patients with this syndrome might have had multiple operations on a limb and might also have extensive internal fixation in-situ. We review the case of a 41-year-old woman with McCune-Albright syndrome, who presented with a pathological fracture of her left femur below a long plate and screws. Our management of this challenging problem included the use of a retrograde femoral nail, which

Projection geometry and stress-reduction techniques in craniofacial surgery.

Since 1981, we have been able to modify the mathematical patterns of projection geometry to reshape the skull in craniofacial surgery. Unlike burring, morcellization, rotation, and plate switching, this technique actually changes the shape of individual sections of the skull by changing their radius of curvature. The technique is an adaptation of the principles used by engineers to build complex structures such as ships' hulls, airfoils, and domes. The result is a rigid form of the desired shape that becomes permanent with healing. This has several advantages: 1. An increase in the level of sa

The Figure-8 Crossing Line Technique for Arthroscopic Treatment of Acromioclavicular Joint Dislocation: A Technical Note.

For high-grade dislocation of the acromioclavicular joint, surgical intervention is typically recommended. In comparison to clavicle hook plate fixation, arthroscopic suspension fixation offers several advantages, including reduced trauma, fewer complications, and improved postoperative joint function. However, the procedure poses significant technical challenges, particularly regarding the creation of bone tunnels under arthroscopy. This article presents an effective technique for crossing lines, termed the "figure-8 crossing technique." This method provides numerous opportunities for line cr

Indications for and Risks Associated With Implant Removal After Pediatric Trauma.

A wide range of implants are used in the treatment of pediatric fractures, including wires, plates, screws, flexible rods, rigid rods, and external fixation devices. Pediatric bones differ from adult bones both mechanically and biologically, including the potential for remodeling. Implants used in pediatric trauma patients present a unique set of circumstances regarding indications, risks, timing of implant removal, weight-bearing restrictions, and long-term sequelae. Indications for implant removal include wire/pin fixation, when substantial growth remains, and infection. When considering imp

Refinements in the reconstruction of bisphosphonate-related osteonecrosis of the jaw.

The recommended treatment strategy for stage 3 bisphosphonate-related osteonecrosis of the jaw (BRONJ) is currently rigid plate fixation without bone reconstruction. However, a recent systematic review indicated the utility of microsurgical reconstruction after resection of BRONJ. Several types of flaps have been described but their applications are controversial. Here we present a detailed reconstruction plan for obtaining better outcomes in patients with maxillary and mandibular BRONJ. Given that progressive maxillary BRONJ is often invasive to the skin, including the eyelid, leading to func

Cerebral fat embolism syndrome (FES): similar cases with different outcomes.

Fat embolism syndrome (FES) is a rare multisystem, clinical syndrome occurring in 0.9%-2.2% of long-bone fractures. The severity of FES can vary from subclinical with mild respiratory changes and haematological aberrations to a fulminant state characterised by sudden onset of severe respiratory and neurological impairment. Here we present two patients with cerebral FES secondary to femur fracture. Both patients exhibited profound neurological impairment with varied outcomes. Our cases highlight the importance of a high clinical suspicion of FES in patients with long-bone fractures and neurolog
