Outcomes of unrelated cord blood transplants and allogeneic-related hematopoietic stem cell transplants in children with high-risk acute lymphocytic leukemia.

Acute lymphocytic leukemia (ALL) is a common indication for hematopoietic stem cell transplantation (HSCT) in children. Use of unrelated cord blood (UCB) has become increasingly popular as a stem cell source, given the rapid availability and decreased GVHD potential. Publications describing outcomes of children with leukemia who underwent UCB transplants have compared them to those having received unrelated donor marrow transplants. Results are similar. We compared our outcomes using UCB vs allogeneic-related hematopoietic stem cells in pediatric ALL patients since 1992. A total of 49 patients…

Rapid in vivo loss of non-T-cell binding capacities of ATLG Grafalon® and ATG Thymoglobulin® explains selective effect on T-cell reconstitution after hematopoietic stem cell transplantation.

Anti-thymocyte globulin (ATG Thymoglobulin®) and anti-T-lymphocyte globulin (ATLG Grafalon®) are commonly used in hematopoietic stem cell transplantation (HSCT) for in vivo T-cell depletion, aiming to reduce graft failure and graft-versus-host disease (GvHD). Despite their widespread use, the precise mechanisms by which ATG/ATLG affect immune recovery post-transplantation remain partially understood. In this study, in 289 pediatric HSCT patients treated with ATG (n = 213) or ATLG (n = 76), the relationship between longitudinal serum levels of the active T-lymphocyte-binding component of ATG/AT…

Stem Cell Mobilization Performed with Different Doses of Cytarabine in Plasma Cell Myeloma Patients Relapsing after Previous Autologous Hematopoietic Cell Transplantation-A Multicenter Report by the Polish Myeloma Study Group.

Salvage autologous hematopoietic cell transplantation (auto-HCT) may be used to treat relapse of plasma cell myeloma occurring after previous auto-HCT. When an insufficient number of hematopoietic stem cells have been stored from the initial harvest, remobilization is necessary. Here, we aimed to analyze the efficacy and safety of different doses of cytarabine (total 800 vs. 1600 vs. 2400 mg/m2) for remobilization. Sixty-five patients, 55% male, with a median age at remobilization 63 years, were included. Remobilization was performed with cytarabine_800 in 7, cytarabine_1600 in 36, and cytarab…

Cardiac involvement in a patient with B-cell lymphoblastic lymphoma/acute lymphoblastic leukemia and a history of allogeneic hematopoietic stem cell transplantation and CAR T-cell therapy: A case report.

Cardiac involvement in hematological malignancies is uncommon, with only a few cases reported to date, and it often leads to a poor prognosis. Here, we report a case of a 42-year-old woman with a history of allogeneic hematopoietic stem cell transplantation (allo-HSCT) and anti-CD19 chimeric antigen receptor (CAR) T-cell therapy for B-cell lymphoblastic lymphoma/acute lymphoblastic leukemia in whom cardiac mass and myocardial infiltration were detected. Prior to this presentation, massive pericardial effusion had occurred 6 months after CAR T-cell therapy, which was improved via ultrasound-gui…

Anti-CD19 chimeric antigen receptor-modified T-cell therapy bridging to allogeneic hematopoietic stem cell transplantation for relapsed/refractory B-cell acute lymphoblastic leukemia: An open-label pragmatic clinical trial.

Chimeric antigen receptor-modified T-cell (CAR-T) therapy is effective and safe for patients with relapsed/refractory B-cell acute lymphoblastic leukemia (r/r B-ALL), but its value has been limited in terms of long-term leukemia-free survival. New strategies that can help CAR-T therapy achieve lasting effect are urgently warranted. This non-randomized interventional pragmatic clinical trial had a particular aim. It explored whether consolidative allogeneic hematopoietic stem cell transplantation (allo-HSCT) could improve the long-term prognosis of the minimal residual disease-negative complete…

Cytomegalovirus-Specific CD8+ T-Cells With Different T-Cell Receptor Affinities Segregate T-Cell Phenotypes and Correlate With Chronic Graft-Versus-Host Disease in Patients Post-Hematopoietic Stem Cell Transplantation.

Virus-specific T-cell responses are crucial to control cytomegalovirus (CMV) infections/reactivation in immunocompromised individuals. Adoptive cellular therapy with CMV-specific T-cells has become a viable treatment option. High-affinity anti-viral cellular immune responses are associated with improved long-term immune protection against CMV infection. To date, the characterization of high-affinity T-cell responses against CMV has not been achieved in blood from patients after allogeneic hematopoietic stem cell transplantation (HSCT). Therefore, the purpose of this study was to describe and a…

The cells involved in cell-mediated and transplantation immunity in the normal outbred rabbit. XIII. The identity of the responder cells and the role of phagocytic cells in the mixed leucocyte culture reaction.

The ability of rabbit spleen cells depleted of thymus, bone marrow or appendix-derived cells to respond in the one-way mixed leucocyte reaction (MLR) has been investigated. The specific subpopulations of lymphoid cells were eliminated by lysis of these cells in the presence of specific antisera and complement. Phagocytic cells (monocytes and heterophils) were removed with a strong magnet after incubation with carbonyl iron particles. The results indicate that the MLR-responding cells are thymus-derived and that neither bone marrow-derived cells nor appendix-derived cells are essential for the

An EBMT registry matched study of allogeneic stem cell transplants for lymphoma: allogeneic transplantation is associated with a lower relapse rate but a higher procedure-related mortality rate than autologous transplantation.

The role of allogeneic bone marrow transplantation in lymphoma remains uncertain. We have analyzed 1185 allogeneic transplants for lymphoma reported to the EBMT registry between 1982 and 1998 and compared the results with those of 14687 autologous procedures performed over the same period. Patients receiving allogeneic transplants were subdivided according to histology: low-grade non-Hodgkin's lymphoma (NHL) 231 patients; intermediate-grade NHL 147 patients; high-grade NHL 255 patients; lymphoblastic NHL 314 patients; Burkitt's lymphoma 71 patients; and Hodgkin's disease 167 patients. Efficacy of Unrelated Allogeneic Hematopoietic Stem Cell Transplantation in Severe Aplastic Anemia: A Single-Center Retrospective Study.

To evaluate the efficacy of unrelated allogeneic hematopoietic stem cell transplantation (URD-HSCT) in severe aplastic anemia (SAA), this study retrospectively analyzed the clinical data of 206 patients at a single center. The median age was 28 years (range: 5-64 years), and 13.6% had received immunosuppressive therapy prior to transplantation. The median time to neutrophil and platelet engraftment was 12 days (range: 9-22 days / 8-55 days), respectively, with a graft failure rate of 3.4%. The cumulative incidence of grade II-IV acute graft-versus-host disease (aGVHD) was 21.7% (95% CI: 16.6%-…

Comparative prognostic analysis of allogeneic hematopoietic stem cell transplantation regimens in the treatment of severe aplastic anemia.

BACKGROUND: Severe aplastic anemia (SAA) is a fatal bone marrow failure disorder. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is the definitive curative treatment for SAA. This study evaluated the prognosis of patients with severe aplastic anemia (SAA) receiving different allo-HSCT regimens. METHODS: A retrospective analysis was performed on 62 patients with aplastic anemia who underwent allo-HSCT between January 2015 and August 2024. The cohort comprised 32 males and 30 females, with a median age of 12 years (range, 1.5-44 years) at transplantation; 28 patients were ≥14 ye…

Liberal versus restrictive transfusion in haemato-oncological patients undergoing autologous stem cell transplantation: A single-centre experience.

OBJECTIVES: Assess the safety and efficacy of restrictive transfusion in haemato-oncological patients undergoing autologous haematopoietic stem cell transplantation (AHSCT). BACKGROUND: Restrictive transfusion is increasingly advocated within transfusion medicine to prevent over-treatment, lower transfusion-related adverse events and decrease healthcare costs. Yet there is a paucity of evidence regarding safe restrictive transfusion strategies in AHSCT patients. METHODS: Double-unit transfusion at a haemoglobin threshold of 8 g/dL (i.e., liberal transfusion) is compared to single-unit transf…

Parameters characteristic of upper dermal microvasculature in patients after hematopoietic cell transplantation by noninvasive reflectance confocal videomicroscopy.

We noninvasively visualized the upper dermal microvasculature of 31 patients after hematopoietic cell transplantation (HCT) by reflectance confocal videomicroscopy. The microvessel diameter and number and diameter of adherent and rolling leukocytes for patients after HCT were similar to historically published values in healthy subjects. We also observed "Приостановлено" leukocytes i.e. leukocytes that temporarily stop, coinciding with the simultaneous stopping of the rest of the blood flow. The number and diameter of paused leukocytes, and the duration of leukocyte being paused for patients after HCT …

Who Am I? Eyebrow Follicles Minimize Donor-Derived DNA for Germline Testing After Hematopoietic Stem Cell Transplantation.

Germline genetic testing plays a critical role in diagnosing inherited predispositions and increasingly guides therapeutic and surveillance choices-but becomes technically challenging after allogeneic hematopoietic stem cell transplantation (HSCT), when donor-derived DNA contaminates host tissues. To address this, we compared donor-derived DNA across three accessible tissues-buccal swab, nail, and eyebrow follicles-in recipients after hematopoietic stem cell transplantation using two orthogonal assays (34-SNP next-generation sequencing and a 27-marker short tandem repeat panel) and modeled cli…

Early fecal metabolomic profiling for predicting acute graft-versus-host disease following allogeneic hematopoietic stem cell transplantation.

Acute graft-versus-host disease (GVHD) is a major cause of morbidity and mortality after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Reliable early, noninvasive biomarkers for GVHD prediction remain limited. We conducted targeted fecal metabolomic profiling using liquid chromatography-tandem mass spectrometry in 77 allo-HSCT recipients at two time points: before conditioning (S1) and immediately after conditioning but prior to donor T-cell engraftment (S2). We assessed changes in metabolite profiles and their association with subsequent GVHD development. A multivariate logi…

Early Peripheral Blood WT1 Expression Predicts Relapse After Allogeneic Hematopoietic Stem Cell Transplantation in Acute Myeloid Leukemia.

Relapse remains the leading cause of treatment failure after allogeneic hematopoietic stem cell transplantation (allo-HSCT) in acute myeloid leukemia (AML). Early identification of patients at increased post-transplant relapse risk is essential to enable intensified surveillance and pre-emptive therapeutic strategies. Wilms' tumor 1 (WT1) is overexpressed in most AML cases and represents a broadly applicable molecular marker; however, its utility as a peripheral blood (PB) measurable residual disease (MRD) marker after allo-HSCT remains incompletely defined. In this prospective multicenter coh…
