Geographic disparities in MRI access and patterns of mobile MRI deployment across Michigan counties.

BACKGROUND: Rural populations face higher risks of poor health outcomes in part due to limited access to diagnostic imaging. Geographical barriers may be exacerbated by longer travel distances, limited public transportation, and structural and policy factors such as Certificate of Need (CON) regulations. Mobile MRI units have been deployed across Michigan; however, it remains unclear whether they are deployed in a manner that meaningfully reduces geographic barriers to MRI access in rural populations. PURPOSE: To examine geographic disparities in access to MRI machines across Michigan countie

Mortality trends of pressure ulcers and non-healing wounds in the United States, 1999-2024.

BACKGROUND: Pressure ulcers (PUs) and chronic non-healing wounds (NHWs) are important causes of morbidity and mortality, particularly among older adults with multimorbidity. The two conditions arise through different mechanisms but are frequently reported together, which can conceal their separate trajectories. This study assessed 26 year national trends in wound-associated mortality in the United States, analyzing each condition on its own. METHODS: We conducted a retrospective analysis of CDC WONDER mortality data from 1999 to 2024. PUs were defined by ICD-10 code L89, and chronic NHWs by L

Spoken language and risk of nulliparous, term, singleton, vertex cesarean births in California, 2016 to 2021.

BACKGROUND: Non-English-speaking individuals have noted disparities in health outcomes in the United States, but the role of language in obstetric settings is not well characterized. OBJECTIVE: We examined the association between primary spoken (preferred) language and the risk of nulliparous, term, singleton vertex cesarean delivery (CD). STUDY DESIGN: We conducted a retrospective cohort study of nulliparous, term, singleton vertex deliveries in California (2016-2021) using birth certificates linked to hospital discharge records. The primary outcome was the risk of CD. Poisson log-linear re

Patients' Use of e-Consultations as an Alternative to Other General Practitioner Services: Cross-Sectional Survey Study.

BACKGROUND: e-Consultations, defined as asynchronous text-based messaging, have transformed how patients interact with their general practitioner (GP). While e-consultations can improve patient access to GP care, concerns about increased workload for GPs are raised. OBJECTIVE: This study aimed to address three research questions: (1) For what purpose and with what expectations do patients initiate e-consultations? (2) If e-consultations had not been available, what alternative actions would the patient have taken? and (3) How are the alternative actions associated with patient and e-consultat

Recommendations for Designing a Digital Health Tool for Blindness Prevention Among High-Risk Diabetic Retinopathy Patients: Qualitative Focus Group Study of Adults With Diabetes.

BACKGROUND: Diabetic retinopathy (DR) is a leading cause of preventable blindness among working-aged adults. Black, Latine, and low-income individuals are screened less for DR, diagnosed later, treated less often, and go blind more than White individuals. OBJECTIVE: This study aimed to engage members to co-design a digital health tool that is accessible, user-friendly, and culturally relevant, through a community-led research approach,. METHODS: Using a qualitative approach, we conducted 4 semistructured focus group interviews with 19 individuals from the Greater New Haven area, aged 18 year

Episiotomy and severe perineal laceration among Asian American, Native Hawaiian, and Pacific Islander nulliparous individuals in California.

BACKGROUND: Asian American, Native Hawaiian, and Pacific Islander individuals have increased risks for episiotomy and severe perineal laceration during vaginal delivery. The Asian American, Native Hawaiian, and Pacific Islander population in the US is diverse yet few studies disaggregate results within specific ethnicity populations. OBJECTIVE: This study investigated the variability in risks for episiotomy and severe perineal laceration among 16 disaggregated Asian American, Native Hawaiian, and Pacific Islander groups, compared to Non-Hispanic White nulliparous individuals, and assessed wha

Familial risk of placental abruption.

BACKGROUND: Placental abruption (PA), the partial or complete separation of the placenta before delivery, occurs in 1-2 % of pregnancies and is associated with severe maternal and perinatal morbidity and mortality. The etiology of PA is complex, and the increased recurrence risk in subsequent pregnancies suggests a potential genetic contribution. Investigating PA in large families may reveal important insights into inherited risk and the extent to which genetic factors contribute to PA. To guide future gene-discovery studies, we investigated the familial risk of PA using data from 1.2 million

Variations in Postpartum Readmission by Individual Race, Ethnicity, and Rurality in South Carolina.

IMPORTANCE: Racial and ethnic disparities in postpartum readmission (PPR) remain a critical public health concern, with non-Hispanic Black individuals experiencing rates up to 3 times as high as non-Hispanic White individuals. PPR is often associated with mental health disorders (MHDs) and substance use disorders (SUDs), and barriers to care are particularly acute in rural communities. OBJECTIVE: To investigate intersections in the associations of individual residential rurality and race and ethnicity with all-cause, MHD-, and SUD-related PPR throughout 1 year post partum. Nonspecific Effects of the Bacillus Calmette-Guérin Vaccine in Portuguese Children Under 5 Years of Age: Protocol for a Population-Based Historical Birth Cohort Study.

BACKGROUND: The Bacillus Calmette-Guérin vaccine (BCG) against tuberculosis (TB) shows beneficial nonspecific effects, which are likely related to innate immune training. Until 2016, a single BCG dose was administered to all newborns in Portugal. In July 2016, a clinical guideline established that only children under 6 years belonging to high-risk groups should receive BCG. This might have prevented nonvaccinated children from developing trained immunological responses as effectively as BCG-vaccinated children. OBJECTIVE: This study aims to investigate if there is variation in TB-related and

Racial and ethnic disparities in severe maternal morbidity from pregnancy through 1-year postpartum.

BACKGROUND: Previous studies examining racial and ethnic disparities in severe maternal morbidity (SMM) have mainly focused on intrapartum hospitalization. There is limited information regarding the racial and ethnic distribution of SMM occurring in the antepartum and postpartum periods, including SMM occurring beyond the traditional 6 weeks postpartum period. OBJECTIVE: To examine the racial and ethnic distribution of SMM during antepartum, intrapartum, and postpartum hospitalizations through 1-year postpartum, overall and stratified by maternal sociodemographic factors, and to estimate the

Alcohol-related mortality in 15-24-year-olds in Denmark (2010-2019): a nationwide exploratory study of circumstances and socioeconomic predictors.

BACKGROUND: Adolescents and young adults aged 15-24 years are disproportionately affected by unnatural deaths, including accidents, suicide and interpersonal violence for which alcohol is a leading risk factor. We aimed to explore the extent of and circumstances surrounding alcohol-related deaths in young people aged 15-24 years and whether socioeconomic background and ethnicity differ in those who died due to alcohol-related causes as compared to the background population. METHODS: All deaths of 15-24-year-olds occurring in Denmark from 2010 to 2019 were investigated. We manually reviewed de

Comorbidity and Cancer Disease Rates among Those at High-Risk for Alzheimer's Disease: A Population Database Analysis.

(1) Importance: Alzheimer's disease (AD) is complex and only partially understood. Analyzing the relationship between other more treatable or preventable diseases and AD may help in the prevention and the eventual development of treatments for AD. Risk estimation in a high-risk population, rather than a population already affected with AD, may reduce some bias in risk estimates. (2) Objective: To examine the rates of various comorbidities and cancers in individuals at high-risk for AD, but without a clinical diagnosis, relative to individuals from the same population with normal AD risk. (3) D

BACKGROUND: Federal Ministry of Health (FMoH) Ethiopia achieved significant declines in malaria mortality and incidence and has recently launched malaria elimination in selected low transmission settings. Successful malaria elimination calls for rapid and accurate diagnosis of cases so that the patients can promptly be treated before the occurrence of transmission. Therefore, this study assessed the competency of malaria microscopists using panal slides, and laboratory service availability and readiness in terms of supplies and equipments in malaria elimination targeted districts in Ethiopia.

Effectiveness of Rubella vaccine in a rubella outbreak in Guangzhou city, China, 2014.

BACKGROUND: WHO recommends the use of rubella-containing vaccine (RCVs) to prevent rubella and congenital rubella syndrome (CRS). Most licensed rubella vaccines in use globally are based on RA27/3 strains and have estimated vaccine effectiveness (VE) rates of 95-100%. In contrast, China uses a BRD-II strain-based rubella vaccine. Few field studies have been conducted that estimate VE of China's RCV. On March 17, 2014, a rubella outbreak was reported in a middle school in Guangzhou city, China. We conducted an investigation to understand reasons for the outbreak, and we used that investigation
