Medulloblastoma metastatic to the suprasellar region at diagnosis: a report of six cases with clinicopathologic correlation.

The presence of metastatic disease in patients newly diagnosed with medulloblastoma remains one of the most important prognostic factors that determines event-free survival. In the present study, anatomic distribution and the signal characteristics and enhancement patterns of subtle anterior third ventricular recess metastases were compared with those of the original tumor; medical records were reviewed for clinical presentation, surgical stage, treatment and long-term outcomes. All foci were clinically occult; 5 out of 6 had negative cerebrospinal fluid cytology, and in 4 out of 6, the only e

Spinal fractures in patients with ankylosing spondylitis.

Thirty-one consecutive patients with ankylosing spondylitis and spinal fractures were reviewed. There were 6 women and 25 men with a mean age of 60 +/- 11 years; 19 had cervical and 12 had thoracolumbar injuries. Of the patients with cervical fracture, two had an additional cervical fracture and one had an additional thoracic fracture. Three trauma mechanisms were identified: high-energy trauma in 13 patients, low-energy trauma in 13 and insufficiency fracture in 5. One-third of the patients suffered immediate neurological impairment, a further one-third developed neurological impairment befor

[Epidemiology, treatment and long-term results of spinal fractures at a non-university affiliated central hospital with special reference to fractures of the lumbar spine].

In a retrospective study epidemiology, treatment, clinical and radiological results of 283 patients with spine fractures in a five-year period are presented. The operation rate ranges from 42% of the cervical to 9% of the thoracical and 24% of the lumbar spine. We found good radiological results concerning the correction of the wedge compression and the collapse of the lumbar vertebral body by fixation with an internal fixator. After a follow-up of 2-5 years nearly 80% of conservative as well as operative treated patients had more or less residual back pain. Because there is a correlation betw

Spinal neurofibromas: a report of 66 cases and a comparison with meningiomas.

A series of 66 spinal cord neurofibromas was analyzed for history, signs, surgical approach, and outcome. The tumors presented primarily with sensory symptoms. Plain films were abnormal in 1/2 of cases and 1/2 had a complete block. They were primarily intradural, and primarily thoracic. A conservative exam system was used for follow-up and 85% with pain had complete relief; 50% with motor loss had normal motor function, and 88% had normal sensation who had prior sensory loss. In comparison to meningiomas, the principal differences were that neurofibromas had an even sex distribution, a lower i

Irregular Cervicothoracic Nerve Sheaths on Digital Subtraction Myelography: A CSF-Venous Fistula Mimic.

BACKGROUND AND PURPOSE: Spontaneous intracranial hypotension (SIH) can be caused by CSF-venous fistulas (CVFs), which often require a specialized lateral decubitus examination such as digital subtraction myelography (DSM) for diagnosis. DSM interpretations can be confounded by irregular nerve sheath diverticula at the cervicothoracic junction, potentially mimicking a true CVF. This study aimed to characterize anatomic variations of nerve sheaths at the cervicothoracic junction, in an effort to reduce the risk of misdiagnosis. MATERIALS AND METHODS: We retrospectively identified 35 patients wi

Deep learning algorithm for identifying osteopenia/osteoporosis using cervical radiography.

Due to symptomatic gait imbalance and a high incidence of falls, patients with cervical disease-including degenerative cervical myelopathy-have a significantly increased risk of fragility fractures. To prevent such fractures in patients with cervical disease, treating osteoporosis is an important strategy. This study aimed to validate the diagnostic yield of a deep learning algorithm for detecting osteopenia/osteoporosis using cervical radiography and compare its diagnostic accuracy with that of spine surgeons. Samples were divided into training (n = 200) and test (n = 30) datasets. The deep l

Maintenance of construct integrity with subaxial cervical pedicle screws when crossing the cervicothoracic junction in complex pediatric deformity: an international multicenter study.

OBJECTIVE: In cases of complex pediatric spinal deformity, posterior spinal instrumentation crossing the cervicothoracic junction (CTJ) may be required. This is most frequently encountered for revision surgery to address proximal junctional kyphosis (PJK). In the cervical spine, lateral mass screws are most commonly used, although they may result in biomechanically weak proximal constructs and predispose to instrumentation failure. The use of supplemental anterior constructs has recently been reported with promising results, but this necessitates an additional surgical procedure. Subaxial cerv

Preoperative T1 Slope More Than 40° as a Risk Factor of Correction Loss in Patients With Adult Spinal Deformity.

STUDY DESIGN: A retrospective study of surgical outcomes of adult spinal deformity (ASD) cases. OBJECTIVE: The aim of the study was to investigate the effects of high T1 slope (T1S) on surgical outcomes in patients with ASD. SUMMARY OF BACKGROUND DATA: Few studies have evaluated the surgical outcomes of patients with ASD with cervical deformities. METHODS: Eighty-eight patients with ASD who underwent posterior spinal corrective fusion were assigned to either group A (T1S <40°) or group B (T1S ≥40°). Whole-spine standing radiographs of both groups were preoperatively assessed: at first stand

Fatal child abuse presenting with multiple vertebral and vascular trauma.

This case study involves the unexplained death of a previously healthy 30-month-old child. Reportedly, she was found unresponsive by her foster father following a 2-hour nap. She was transported to the hospital and died in the emergency room of unknown causes. Blood drawn in the ER showed hemoglobin of 4.3 mg/dL. Postmortem skeletal survey, magnetic resonance imaging (MRI) of the brain, and retinal examinations were negative. The medical examiner assumed jurisdiction of the body. The autopsy showed massive retroperitoneal hemorrhage, transections of the abdominal aorta, and inferior vena cava,

[Type AO/ASIF B3 fractures of the thoracic and lumbar spine].

PURPOSE OF THE STUDY: Type B3 thoracic and lumbar fractures are often found in spines with previous hyperossification processes such as ankylosing spondylarthritis (AS) or diffuse idiopathic skeletal hyperostosis (DISH). They occur most frequently due to high-energy trauma in a healthy spine and as fall-related domestic injury in a spine affected by hyperossification. Generally, they are less frequent than type B3 cervical spine fractures. In this retrospective study involving two centres, the incidence of these fractures, their characterisation, therapy and complications associated with them

Recurrent spine surgery patients in hospital administrative database.

INTRODUCTION: Hospital patient databases are typically used by administrative staff to estimate loss-profit ratios and to help with the allocation of hospital resources. These databases can also be very useful in following rehospitalization. This paper studies the recurrence of spine surgery patients in our hospital population based on administrative data analysis. METHODS: Hospital data on 4,958 spine surgery patients operated between 2002 and 2009 were retrospectively reviewed. After sorting the cohort per ascending discharge date, the patient official name, consisting of first, middle and
