The distribution of laminin, a basement membrane glycoprotein, was studied by an immunohistochemical technique in six samples of benign sclerosing lesions of extrahepatic bile ducts and in 11 sclerosing cholangiocarcinomas. The expression of laminin showed that benign glandular structures were surrounded by a mainly intact basement membrane. In sclerosing cholangiocarcinomas laminin was irregularly distributed, and in large areas totally absent. The findings suggest that deposition of basement membrane material is defective. Sclerosing cholangitis may be extremely difficult to distinguish from…

Endoscopic papillotomy is nine years old. During this time it obtained a well defined place in the treatment of common bile duct stones and of papillary stenosis. In about 85% this method proved to be successful without any problems. To lower the risk and to improve the results now smaller steps must be done. Within the last 4 years we have learned to manage even big stones with a reduced risk by stone dissolution and by a new lithotripsy device. Biliary pancreatitis has become an indication for an early duodenoscopy and even papillotomy. Acute obstructive suppurative cholangitis may be treate…

Stenosing papillitis is a recently recognized cause of obstructive jaundice. Although many liver disorders are associated with a polyclonal increase in serum immunoglobulin levels, a monoclonal increase is relatively rare. A patient with monoclonal gammopathy associated with idiopathic stenosing papillitis is reported. The gammopathy has persisted for 3 years after surgical relief of the obstructive jaundice.

Under carefully controlled conditions in a clinical research ward, conjugated primary bile-acid concentrations were measured by a 125I-radioimmunoassay every eight hours for four days in seven healthy control subjects and ten patients with hepatic disease (five with cirrhosis, three with primary biliary cirrhosis and two with sclerosing cholangitis). Two meals of approximately equal composition were consumed daily at 10 A.M. and 6 P.M., and blood was drawn at 4 A.M., 12 noon and 8 P.M.. With the exception of one patient, all subjects had greater postprandial than fasting serum bile-acid concen…

The authors review the indications of their 146 cases of supraduodenal choledochoduodenostomy (SDC) from 1970 to 1977. The conditions for which the procedure was carried out were dilatation of the choledochus (100%), multiple calculosis (83%), and primitive or secondary stenosing odditis (17%). After a detailed discussion of these indications, the authors report their immediate and long-term results, which were good in 96% of the patients. In the matter of surgical technic the authors give preference to laterolateral SDC, stressing that effective anastomosis requires a choledochal diameter of

A 76-year-old man presented with liver dysfunction and intrahepatic bile duct dilatation. Imaging studies showed two large stones that had become impacted in the common hepatic duct, which was fused with the gallbladder. The patient was diagnosed with Mirizzi syndrome type IV. Hepaticojejunostomy and stone removal failed due to dense gallbladder adhesions involving the right hepatic artery. The bile flow was temporarily restored; however, the patient experienced cholangitis 16 months later. The stones were extracted via peroral single-operator cholangioscopy (SOC)-guided electrohydraulic litho…

INTRODUCTION: Pernicious anemia (PA) is a risk factor for gastric cancer. Other autoimmune conditions may also contribute.
METHODS: In a case-control study, we evaluated 47 autoimmune conditions among 39,125 gastric cancers and 200,000 cancer-free controls.
RESULTS: Six conditions were associated with increased gastric cancer risk (range of adjusted odds ratios: 1.28-1.93, P < 0.05): PA, membranous nephropathy, primary biliary cirrhosis, pure red cell aplasia, primary sclerosing cholangitis, and Graves disease. PA was associated with 8 other autoimmune conditions (adjusted odds ratios: 1.57-…

A 72-year-old woman with primary biliary cholangitis was admitted to our hospital with heart failure with a preserved ejection fraction. An accidental right ventricular perforation that occurred during an endomyocardial biopsy precipitated cardiogenic shock. Despite successful surgical treatment, she demonstrated progressive hemodynamic deterioration, which was resistant to the administration of high-dose catecholamines. She was diagnosed with acute adrenal insufficiency, which was attributed to the discontinuation of Celestamine® (betamethasone/dextrochlorpheniramine combination) just after t…

A 65-year-old male patient with a one year history of liver transplantation was referred for an 18F-fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) to rule out post transplant lymphoproliferative disease. Multiple foci of intense abnormal 18F-FDG uptake were seen in the transplanted liver which were concerning for malignancy. Explantation of the liver approximately 1 month following the PET/CT revealed multiple inflammatory and ischemic changes including large bile duct necrosis, acute cholangitis, bile duct obstruction changes and periportal fibros…

Caroli disease (CD) is a rare congenital malformation of the liver characterized by non-obstructive, segmental, cystic dilatation of the intrahepatic bile ducts (IHDs). The clinical course is usually asymptomatic for the first 5-20 years, and symptoms may seldom occur throughout the patient's life. Bile stagnation leads to recurrent episodes of cholangitis, stone formation, or liver abscesses, and biliary cirrhosis usually occurs years later. Here we report on a 42-year-old man diagnosed with diffuse-type CD with a characteristic central dot sign, who had multiple intrahepatic and common bile …
