Don't Miss the Blind Spots: Incidental Detection of Choroidal Melanoma During Primary Care Evaluation of Visual Field Loss.

Identifying choroidal melanoma in the primary care setting represents a diagnostic challenge. We present a rare case of choroidal melanoma in a patient who presented with progressive superolateral quadrantanopia. A CT of the head identified an intraocular hyperdensity within the left globe. Fundoscopy confirmed a classic dome-shaped choroidal melanoma. Gene expression profiling demonstrated a class 1b uveal melanoma with PRAME positivity and mutations in GNAQ and SF3B1. Staging scans were negative for metastatic disease. Vision improved following four sessions of proton beam therapy. This case…

Axial and choroidal changes as key predictors of myopia control in red-light therapy: Evidence from machine learning models.

BACKGROUND: Repeated low-level red-light (RLRL) therapy has emerged as a promising non-invasive intervention for myopia control. However, the predictive factors underlying its efficacy remain insufficiently explored. METHODS: This multicenter cohort study included 538 pediatric patients who underwent RLRL treatment with a minimum follow-up of one year. Baseline ocular parameters and dynamic changes in axial length (AL) and choroidal thickness (ChT) over three months were collected. Multiple feature selection approaches-LASSO regression, Boruta, recursive elimination, and multivariate regressi…

Wide-field choroidal thickness changes after loading-phase anti-VEGF therapy in treatment-naïve neovascular AMD.

PURPOSE: While it is known that anti-vascular endothelial growth factor (anti-VEGF) treatment for neovascular age-related macular degeneration (nAMD) leads to a reduction in choroidal thickness in the macula, changes in peripheral choroidal thickness remain unclear. This study aimed to evaluate choroidal thickness changes following three monthly intravitreal injections of aflibercept and faricimab in treatment-naïve nAMD. METHODS: Treatment-naïve nAMD patients receiving three consecutive monthly injections as the loading phase were enrolled in this study. Wide-field swept-source OCT was used …

Choroidal neovascularization secondary to punctate inner choroidopathy after resolution of multiple evanescent white dot syndrome: A case report.

RATIONALE: White dot syndromes include multiple evanescent white dot syndrome (MEWDS) and punctate inner choroidopathy (PIC), which are rarely reported to coexist or occur sequentially in a single eye. The co-occurrence increases diagnostic complexity, and there are no previous reports of choroidal neovascularization (CNV) secondary to PIC following the resolution of MEWDS. This case report aims to supplement clinical evidence for the pathogenesis and treatment of such sequential ocular diseases. PATIENT CONCERNS: A 42-year-old healthy female presented with blurred vision in her right eye for…

Maternal retinal and choroidal microvascular alterations in late-onset fetal growth restriction assessed by optical coherence tomography angiography.

BACKGROUND: Fetal growth restriction(FGR) is a common pregnancy complication leading to adverse perinatal outcomes and long-term developmental risks. Beyond placental insufficiency, increasing evidence suggests that maternal systemic and microvascular alterations may contribute to its pathophysiology. Optical coherence tomography angiography (OCTA) enables non-invasive evaluation of retinal microvasculature and may provide insights into maternal microvascular alterations in pregnancies complicated by FGR. OBJECTIVE: To assess maternal retinal and choroidal microvascular changes in pregnancies…

Gradual Occlusion of an Anterior Choroidal Artery Aneurysm Using Overlapping Flow Diversion and Short-term Anticoagulation: A Case Report.

Anterior choroidal artery aneurysms, in which the anterior choroidal artery arises from the aneurysmal sac, pose significant therapeutic challenges, as treatment risks include branch occlusion and devastating ischemic complications, including hemiplegia and hemianopia. We report a patient in their 70s with an incidentally diagnosed 12.2 mm unruptured right anterior choroidal artery aneurysm, in which three-dimensional angiography demonstrated that the anterior choroidal artery originated from the center of the aneurysm dome. During conservative observation, the aneurysm enlarged to 14.1 mm, ne…

Genome-wide discovery reveals 30 loci for choroidal thickness and uncovers potential causal links with angle-closure glaucoma.

The choroid is critical for maintaining vision and implicated in several ocular diseases, being the sole source of nutrients and waste removal for the outer retina. Genetic discovery can help elucidate the pathways through which choroidal features influence disease risk. Our meta-analysis of genome-wide association studies (n= 78,682 participants) identified 30 genomic regions, including 20 novel loci, associated with choroidal thickness. Findings suggest inflammatory and vascular processes drive choroidal thickness, with overlapping mechanisms shared with refractive error. Genome-wide indepen…

From Somatic Driver to Molecular Therapy in Retinal, Choroidal and Periocular Vascular Disease: Mosaicism, Targetable Pathways and Ocular Toxicity.

The identification of somatic drivers has turned a field once based on morphology into a set of molecular pathways that can potentially be t … concern. Gq/11 signaling accounts for most capillary malformations, Sturge-Weber syndrome and both choroidal hemangiomas, which are separated by the mutated codon rather than by histology, and it remains a preclinical target. Assessment of choroidal vessels in healthy eyes using 3-dimensional vascular maps and a semi-automated deep learning approach.

To assess the choroidal vessels in healthy eyes using a novel three-dimensional (3D) deep learning approach. In this cross-sectional retrospective study, swept-source OCT 6 × 6 mm scans on Plex Elite 9000 device were obtained. Automated segmentation of the choroidal layer was achieved using a deep-learning ResUNet model along with a volumetric smoothing approach. Phansalkar thresholding was employed to binarize the choroidal vasculature. The choroidal vessels were visualized in 3D maps, and divided into five sectors: nasal, temporal, superior, inferior, and central. Choroidal thickness (CT) an…
