Composite TyG-Inflammation Index as a Preoperative Biomarker of Metabolic-Inflammatory Vulnerability in Colorectal Cancer Surgery: Associations With Postoperative Complications and Myosteatosis.

BACKGROUND AND AIMS: Systemic inflammation and insulin resistance are increasingly recognized as key determinants of perioperative vulnerability in patients with colorectal cancer. The Composite TyG-Inflammation Index (CTI), which integrates the triglyceride-glucose index and C-reactive protein, has recently emerged as a marker of metabolic-inflammatory dysfunction. However, its clinical relevance in colorectal cancer surgery has not been established. This study aimed to evaluate the association of preoperative CTI with postoperative complications and myosteatosis in patients undergoing electi

Subclinical anxiety and depression and postoperative complications after major colorectal surgery: A 2-center study.

BACKGROUND: The prevalence of anxiety and depression in patients with cancer and inflammatory bowel diseases is reported to be 25%-40% and 25%-30%, respectively, and has been associated with multiple postoperative complications in different surgery types. METHODS: This is an observational, prospective, and case-control study conducted at 2 referral centers. Adult patients undergoing elective colorectal surgery for colorectal cancer or inflammatory bowel disease completed the Hospital Anxiety and Depression Scale before surgery, and postoperative outcomes were registered. The primary objective

Continuous fascial closure using absorbable barbed sutures lowers the risk of incisional hernia after minimally invasive colorectal surgery: a consecutive single-surgeon retrospective cohort study.

BACKGROUND: Incisional hernia (IH) at extraction sites remains a common late complication of minimally invasive colorectal surgery. Closure technique is a potentially modifiable factor; however, evidence comparing continuous and interrupted fascial closure in this setting is limited. METHODS: We conducted a retrospective single-center cohort study involving 397 patients who underwent laparoscopic or robotic colorectal cancer resection by a single surgeon between April 2019 and May 2025. Interrupted fascial closure was routinely performed until March 2022, after which continuous closure was ex

How Is Colorectal Cancer Care Impacted by Global Crisis in Contrasting Healthcare Systems?-A Descriptive Study From Scotland and Switzerland During the COVID-19 Pandemic.

BACKGROUND: Healthcare across Europe was affected by COVID-19 pandemic lockdowns. How different national healthcare systems coped with this impact remains unclear. Healthcare in Switzerland differs significantly from that in Scotland, for example, in terms of centralization. The aim of this study was to assess the impact of the COVID-19 pandemic on the diagnosis and surgical treatment of colorectal cancer (CRC) in contrasting healthcare systems. PATIENTS AND METHODS: This retrospective cohort study was conducted in south-east Scotland and in the extended north-west of Switzerland from January

Impact of Intravenous Lidocaine, Dexmedetomidine, and Intrathecal Morphine on Metastasis-Related Biomarkers and Cellular Immune Profiles in Colorectal Surgery: A Prospective, Randomized Controlled Trial.

BACKGROUND: Anesthetic adjuvants used in multimodal analgesia-including intravenous lidocaine, dexmedetomidine, or intrathecal morphine (ITM)-may differentially affect immune responses and metastasis-related pathways in colorectal cancer surgery. Their comparative effects on these pathways remain poorly understood. METHODS: In this prospective, randomized, patient- and assessor-blinded trial, adults undergoing elective laparoscopic or robotic colorectal cancer resection were allocated to receive intravenous lidocaine, dexmedetomidine, or ITM. The primary outcome was plasma matrix metalloprote

Postoperative day 3 geriatric nutritional risk index as an early warning marker for anastomotic leakage after colorectal cancer surgery: A retrospective cohort study.

Anastomotic leakage (AL) is a serious complication after colorectal cancer (CRC) surgery, and early postoperative risk stratification remains clinically challenging. The Geriatric Nutritional Risk Index (GNRI) integrates serum albumin and body weight information, but its role as a postoperative marker and its incremental value beyond albumin alone remain uncertain. This single-center retrospective cohort study included 302 patients who underwent laparoscopic radical colorectal cancer surgery at Yanbian University Hospital between January 2022 and October 2024. Postoperative day 3 (POD3) GNRI a

Ability of a polygenic risk score to refine colorectal cancer risk in Lynch syndrome.

BACKGROUND: Polygenic risk scores (PRSs) have been used to stratify colorectal cancer (CRC) risk in the general population, whereas its role in Lynch syndrome (LS), the most common type of hereditary CRC, is still conflicting. We aimed to assess the ability of PRS to refine CRC risk prediction in European-descendant individuals with LS. METHODS: 1465 individuals with LS (557 MLH1, 517 MSH2/EPCAM, 299 MSH6 and 92 PMS2) and 5656 CRC-free population-based controls from two independent cohorts were included. A 91-SNP PRS was applied. Molecular biology from bench-to-bedside - which colorectal cancer patients should be referred for genetic counselling and risk assessment.

Lynch syndrome is associated with deficiency of the mismatch repair genes MLH1, MSH2, MSH6 or PMS2. However, most MLH1 deficient tumours are sporadic in origin, and they can be identified if harbouring a BRAF V600E mutation or hypermethylation of the MLH1 gene promoter. The aim of this study was to validate our previously suggested clinically applicable strategy based on molecular characteristics for identifying which patients to refer for genetic counselling. The strategy was validated in an unselected cohort of 287 colorectal cancer patients. All tumours were tested for MLH1, PMS2, MSH2 and

Circulating Tumor DNA Dynamics in Patients with Liver-Limited Metastatic Colorectal Cancer Resected after First-Line Systemic Treatment.

PURPOSE: Resection of liver metastases improves survival in patients with metastatic colorectal cancer (mCRC) with liver-limited disease (LLD), but relapse remains common. Postresection circulating tumor DNA (ctDNA) is a valid prognostic biomarker; however, no data are available regarding the prognostic effect of presurgery ctDNA after upfront chemotherapy. EXPERIMENTAL DESIGN: Patients (N = 116) with initially unresectable mCRC with LLD who underwent resection after upfront chemotherapy were included. Patients were sequenced with a tumor-naïve ctDNA-based minimal residual disease (MRD) assay

Association of differential miRNA expression with hepatic vs. peritoneal metastatic spread in colorectal cancer.

BACKGROUND: Though peritoneal carcinomatosis reflects a late stage of colorectal cancer (CRC), only few patients present with synchronous or metachronous liver metastases alongside their peritoneal carcinomatosis. It is hypothesized that this phenomenon may be causally linked to molecular characteristics of the primary CRC. This study used miRNA profiling of primary CRC tissue either metastasized to the liver, to the peritoneum or not metastasized at all thus to identify miRNAs potentially associated with defining the site of metastatic spread in CRC. METHODS: Tissue of the primary tumor stem

Incidence of Second Primary Malignancies in Patients with Neuroendocrine Tumours.

BACKGROUND: An association between neuroendocrine tumours (NET) and increased risk of developing second primary malignancies (SPM) has been recognised. METHODS: This was a retrospective review of our institutional prospectively maintained database of NET patients. We identified patients who had been diagnosed with both neuroendocrine and any additional malignancies via examination of patient notes. RESULTS: Clinical data for 169 patients were analysed. After exclusion of patients known to have hereditary tumour predisposition syndromes, 29 SPM were identified in 26 patients (15.38%), the com

Mucoepidermoid carcinoma: A yet unreported cancer associated with familial adenomatous polyposis.

Turcot's syndrome is a rare clinical syndrome, characterized by the association between familial adenomatous polyposis (FAP) and a primary central nervous system tumour. Gardner's syndrome is characterized by the association between FAP and several tumour types such as multiple osteomas, fibromas, epidermoid cysts and desmoid tumours. We report here the case of a twenty-six year-old woman with a history of both Turcot's and Gardner syndromes. She had a family history of adenomatous polyposis with a mutation in the APC (Adenomatous Polyposis Coli) gene. At the age of 26, she presented a mucoepi

The immune response to sporadic colorectal cancer in a novel mouse model.

Current mouse models do not reflect the sporadic nature of colon cancer and do not allow the analysis of antitumor immune response because of the lack of known tumor-specific antigens. Two transgenic mouse models with spontaneous tumor development were generated, directing the expression of SV40T antigen (Tag) either constitutively (Vil-Cre × LoxP-Tag-transgenic mice) or stochastically (Vil-Cre-ER(T2) × LoxP-Tag-transgenic mice) into the putative stem cell region of the crypt of Lieberkühn. Tumor development and antitumor immune response were monitored. Vil-Cre × LoxP-Tag mice developed multip

Effect of misreported family history on Mendelian mutation prediction models.

People with familial history of disease often consult with genetic counselors about their chance of carrying mutations that increase disease risk. To aid them, genetic counselors use Mendelian models that predict whether the person carries deleterious mutations based on their reported family history. Such models rely on accurate reporting of each member's diagnosis and age of diagnosis, but this information may be inaccurate. Commonly encountered errors in family history can significantly distort predictions, and thus can alter the clinical management of people undergoing counseling, screening

Prostate Cancer Screening in the Prostate, Lung, Colorectal and Ovarian (PLCO) Cancer Screening Trial: findings from the initial screening round of a randomized trial.

BACKGROUND: The benefit of screening for prostate cancer using prostate-specific antigen (PSA) testing and digital rectal examination (DRE) is uncertain and is under evaluation in a randomized prospective trial, the Prostate, Lung, Colorectal and Ovarian (PLCO) Cancer Screening Trial. Although the final results are several years away, the initial round of screening is complete. We describe the population enrolled in the PLCO trial, their baseline PSA and DRE screening results, and diagnostic follow-up results during the first year of follow-up. Family history and molecular features of children, adolescents, and young adults with colorectal carcinoma.

BACKGROUND: Colorectal cancer is extremely rare in childhood. Published case series reporting children and adolescents with colorectal cancer have not focused on the underlying genetic aspects of the tumour or genetic susceptibility of the families. AIMS: We examined a cohort of patients with early onset colorectal cancer to determine whether a specific genetic predisposition could be elucidated. In particular, we focused on whether DNA mismatch repair gene deficiency which causes hereditary non-polyposis colorectal cancer (HNPCC) could be elucidated. METHODS: Patients with colorectal cancer
