Decompressive Craniectomy versus Craniotomy for Acute Subdural Hematoma.

BACKGROUND: Traumatic acute subdural hematomas frequently warrant surgical evacuation by means of a craniotomy (bone flap replaced) or decompressive craniectomy (bone flap not replaced). Craniectomy may prevent intracranial hypertension, but whether it is associated with better outcomes is unclear. METHODS: We conducted a trial in which patients undergoing surgery for traumatic acute subdural hematoma were randomly assigned to undergo craniotomy or decompressive craniectomy. An inclusion criterion was a bone flap with an anteroposterior diameter of 11 cm or more. The primary outcome was the r

Outcome in neurologically impaired patients with craniovertebral junction tuberculosis: results of combined anteroposterior surgery.

OBJECT: The authors studied the immediate and long-term outcome after transoral decompression, occipitocervical fusion, and antituberculous therapy (ATT) in patients who had neurological deficits due to craniovertebral junction (CVJ) tuberculosis. METHODS: In this retrospective study, the authors reviewed the management and outcome in nine consecutive patients in whom features of spinal cord compression were observed and CVJ tuberculosis was diagnosed between 1993 and 1999. They ranged in age from 9 to 55 years. Onset of symptoms was acute or subacute and rapidly progressive (median 4 months,

Sequential endoscopic and robot-assisted surgical solutions for a rare fungal spondylodiscitis, secondary lumbar spinal stenosis, and subsequent discal pseudocyst causing acute cauda equina syndrome: a case report.

BACKGROUND: Fungal spondylodiscitis is a rare infectious disease. The secondary lumbar spinal stenosis and postoperative discal pseudocyst were even rarer. The surgical interventions were disputed, yet endoscopic and robot-assisted techniques may be helpful under different circumstances. CASE PRESENTATION: A 62-year-old female was diagnosed as infectious spondylodiscitis at the L4/5 level and a posterolateral endoscopic debridement was performed after invalid conservative therapy. Causative organism culture revealed a rare fungus, Candida tropicalis. A secondary spinal stenosis with refractor

Endoscopic endonasal approach to the cavernous sinus Epstein-Barr virus-positive B cell non-Hodgkin lymphoma in a child: case report.

Cavernous sinus (CS) lymphoma without paranasal sinuses involvement is extremely rare in pediatric population and remains a diagnostic challenge due to its similarity to other tumors located in this area. An 8-year-old boy presented with a 6-day history of gradually developing ptosis in the right eyelid. After admission, his symptoms progressed within 24 h to include right-sided ophthalmoplegia consisting of oculomotor and abducens nerve palsies. Endoscopic endonasal approach (EEA) was performed urgently to decompress the CS and to obtain a diagnosis. The postoperative course was uneventful, a

Tethered Cord as a Complication of Chronic Cerebral Spinal Fluid Diversion.

INTRODUCTION: The etiology and treatment of tethered cord syndrome remains controversial, especially in adults. We present an unusual case of tethered cord syndrome with an associated epidural lipomatosis likely due to chronic over shunting of cerebrospinal fluid (CSF). METHODS: A 25-year-old woman had a history of Chiari malformation, hydrocephalus, and a ventriculoperitoneal shunt. She demonstrated progressively worsening gait, which led to a diagnosis of myelopathy and a posterior cervical decompression and fusion was performed. Imaging revealed engorgement of the epidural veins ventral to

Intradural Neurocysticercosis of Lumbar Spine: A Case Report.

Study Design Case report. Objective Cysticercosis (parasitic infection caused by Taenia solium) is the most common parasitic infection of the nervous system. However, spinal neurocysticercosis is rare. It can present as an extraspinal or intraspinal lesion, with intramedullary being the rarest location. The symptoms can vary from vague backache and radiculopathy to cauda equine syndrome. Methods We report a 32-year-old man who presented with neurocysticercosis in the lumbar spine and cauda equine syndrome. He had low backache for 1 month, hesitancy in micturition, and decreased perianal sensat

Guideline update for the performance of fusion procedures for degenerative disease of the lumbar spine. Part 9: lumbar fusion for stenosis with spondylolisthesis.

Patients presenting with stenosis associated with a spondylolisthesis will often describe signs and symptoms consistent with neurogenic claudication, radiculopathy, and/or low-back pain. The primary objective of surgery, when deemed appropriate, is to decompress the neural elements. As a result of the decompression, the inherent instability associated with the spondylolisthesis may progress and lead to further misalignment that results in pain or recurrence of neurological complaints. Under these circumstances, lumbar fusion is considered appropriate to stabilize the spine and prevent delayed

Compression of the deep branch of the ulnar nerve in Guyon's canal by a ganglion: two cases.

INTRODUCTION: Ulnar nerve compression at the wrist can be caused by a variety of intrinsic and extrinsic factors. Isolated compression of only the deep branch of ulnar nerve by a ganglion is very uncommon. Ultrasound examination can clearly show the cystic lesion compressing the nerves. MATERIALS AND METHODS: We present two cases of compression of deep branch of ulnar nerve by a ganglion in the Guyon's canal. Two male patients presented with history of progressive weakness and paraesthesia in the medial 1(1/2) digits of the non-dominant hand. Interestingly, both the patients noticed sudden on

Complications during anterior surgery of the lumbar spine: an anatomically based study and review.

Procedures involving anterior surgical decompression and fusion are being performed with increasing frequency for the treatment of a variety of pathological processes of the spine including trauma, deformity, infection, degenerative disease, failed-back syndrome, discogenic pain, metastases, and primary spinal neoplasms. Because these operations involve anatomy that is often unfamiliar to many neurological and orthopedic surgeons, a significant proportion of the associated complications are not related to the actual decompressive or fusion procedure but instead to the actual exposure itself. Does surgical approach affect headache relief in patients undergoing cervical fusion for subaxial cervical spondylotic myelopathy? A Spine CORe™ analysis of QOD data.

OBJECTIVE: Headache is a common complaint associated with cervical spondylotic myelopathy (CSM), presenting in more than one-third of patients. Surgical treatment of CSM has been linked to improved headache symptoms. The etiology of headache associated with subaxial cervical spine disorders is not well understood, nor is the mechanism of surgery in relieving symptom intensity. The authors hypothesized that surgical treatment of CSM has a positive impact on patients with headache symptoms, and that anterior cervical discectomy and fusion (ACDF) provides simpler access to foraminal decompression

Clinical Efficacy of Unilateral Biportal Endoscopic Transforaminal Lumbar Interbody Fusion.

Unilateral biportal endoscopic transforaminal lumbar interbody fusion (UBE-TLIF) is an advanced, minimally invasive spinal surgical technique characterized by the use of 2 discrete portals-one for clear visualization and continuous irrigation and the other for the dexterous manipulation of surgical instruments. This configuration not only affords an expanded view (0° or 30°) but also enhances the freedom of movement for instruments, thereby augmenting the precision and flexibility of the surgery. The superiority of UBE-TLIF lies in its capacity to facilitate rapid postoperative recovery with m

Perioperative Multidisciplinary Collaboration Is Essential for Successful Complex Multi-staged Oncological Surgical Procedures in Military Treatment Facilities: A Case Report of a Giant Cell Tumor of the Thoracic Spine.

Although giant cell tumors are generally benign, locally aggressive growth can cause profound pathologic tissue destruction and, in some cases, transform into metastatic neoplasms. Here, we present the case of a 37-year-old female with a previously decompressed giant cell tumor of the thoracic spine who presented with acute neurologic symptoms concerning for metastatic transformation requiring a multi-staged surgical resection. The clinical course highlights the potential for late metastatic changes and underscores the importance of long-term surveillance. Our case demonstrates that complex re

Three-dimensional printing for surgical planning of a double aortic arch case.

BACKGROUND AND AIM: Over the past years, three-dimensional (3D) models of patient-specific anatomical conditions are being used to improve the comprehension and surgical management of a variety of diseases. It is an additional diagnostic tool that aids clinical decision-making. Furthermore, this technology is still not routinely used in the medical field since its availability is limited by cost and complex process. METHODS AND RESULTS: We describe a patient with a balanced-type double aortic arch encircling trachea and esophagus. Considering the clinical symptoms, surgical decompression of t

Progressive dysphagia and neck pain due to diffuse idiopathic skeletal hyperostosis of the cervical spine: a case report and literature review.

Diffuse idiopathic skeletal hyperostosis (DISH) is considered an underdiagnosed and mostly asymptomatic nonprimary osteoarthritis. The etiology of DISH remains unknown and the validated diagnostic criteria are absent. This condition is still recognized radiologically only. Rarely, large projecting anterior osteophytes result in esophageal impingement and distortion leading to dysphagia. We report the case of progressive dysphagia and neck pain due to DISH of the cervical spine in a 70-year-old man, which was surgically removed with excellent postoperative results and complete resolution of sym

[Single and temporally displaced second nerve lesions in an animal model and their clinical significance].

BACKGROUND: Surgical optic decompression after trauma has been discussed controversially. The surgical trauma is supposed to produce an additional nerve lesion with the danger of complete loss of vision. Alternatively, conservative high dose cortisone therapy has been recommended. METHODS: The functional and morphological consequences of a lesion after calibrated optic compression in one or two sessions were examined in an animal model using 29 Wistar rats. RESULTS: Depending on the duration and intensity of the lesion, we observed a linear decline in the number of neurons in the RGC (retina
