We report a patient with a spectrum of clinical features simulating toxic epidermal necrolysis, bullous erythema multiforme and later, dermatitis herpetiformis (DH). The histological features were suggestive of DH, bullous pemphigoid (BP) and epidermolysis bullosa acquisita (EBA). Direct immunofluorescence results suggested BP or EBA. Indirect immunofluorescence on salt-split skin and immunoblotting analysis on normal human dermal extracts gave results that were diagnostic for EBA.

Protein contact with the skin can be associated with 2 major clinical conditions: contact urticaria and protein contact dermatitis. This article reviews the pathogenesis, clinical pictures and the literature on the proteins having caused immunological occupation-related skin problems, i.e. fruits, vegetables, spices, plants, and woods; animal proteins; grains and enzymes, all affecting a wide variety of jobs. This is illustrated with some cases observed in the contact-allergy unit in Leuven.

Dyes are a category of substances capable of inducing allergic contact dermatitis found in a variety of foods, drugs, textiles, cosmetics, and hair dyes. This study tested 33 dyes in guinea pigs using a modified Buehler and Klecak method for open epicutaneous testing.
